Chest-X-ray Based Deep Learning Model for Anemia Detection Insights from a Large Hospital Cohort

Background Anemia is a prevalent global health issue, affecting individuals across all age groups and leading to significant morbidity. Traditional methods for diagnosing anemia rely on laboratory assessments of hemoglobin concentration, which are invasive and costly. This study investigates the potential of a chest X-ray (CXR)-based deep learning model (DLM) to predict anemia, offering a non-invasive and efficient diagnostic alternative. Patients and methods We conducted a retrospective cohort study using data from Tri-Service General Hospital, Taipei, Taiwan, collected between June 2016 and February 2022. A total of 305,793 patients aged 20 years and older who had at least one CXR were included. The dataset was divided into development, tuning, internal validation, and external validation sets. All data originated from Tri-Service General Hospital, except for the external validation set, which was obtained from the Jingjhou branch. A DLM based on a 50-layer SE-ResNeXt architecture was developed to predict anemia from CXR images. The model's performance was compared to predictions made using traditional clinical data to assess its predictive accuracy. Results The DLM for predicting anemia showed strong performance, with an AUC of 0.845 in the internal validation set (sensitivity 68.5%, specificity 84.7%) and 0.852 in the external set (sensitivity 71.5%, specificity 83.2%). Subgroup analysis revealed better diagnostic accuracy with computed radiography, posteroanterior (PA) view, and in males aged 55 to 64, while younger females had lower accuracy. The DLM, especially when combined with patient data, outperformed other clinical models. Conclusion Our study introduces a deep learning tool for detecting anemia from chest images, offering faster decision-making and interventions in clinical settings. With further refinement, this model aims to align more closely with real-time clinical conditions, enhancing its utility. Ultimately, it has the potential to improve patient care through quicker anemia detection and better clinical outcomes.

Bone fractures are common injuries that require quick and accurate diagnosis to provide the right medical care. The “convolutional neural network” technique relies mostly on manual inspection, by radiologists and can be laborious and prone to error. The objective of this work was to increase the efficiency and accuracy of fracture identification by automating the examination of X-ray images using convolutional neural networks. A “convolutional neural network” model created to detect bone fractures in X-ray images was used in the present study. The model was trained and validated using an extensive dataset of X-ray images, which included both fractured and nonfractured bones. Multiple convolutional layers were used in the “convolutional neural network” architecture for feature extraction, and pooling and fully connected layers were added for classification. The main measures used to assess the model's performance were sensitivity, specificity, and accuracy. In regard to identifying bone fractures, the “convolutional neural network”-based model outperformed the conventional technique. With significant gains in sensitivity and specificity, this approach achieved a high accuracy rate and decreased the frequency of false positives and false negatives. We used “convolutional neural network” tool in PyTorch for bone fracture detection and we outlined the important considerations that must be considered when attempting to achieve this goal Additionally, we contrasted every study with our baseline. Here our maximum accuracies were 99.05%, 98.60%, 99.45% and 99.74% for epochs 8, 9, 10 and 11, respectively. These findings highlight the model's ability to improve the diagnostic efficiency and accuracy in clinical settings. Using “convolutional neural network” to identify bone fractures from X-ray images is a promising development in medical imaging. In the end, our method improves patient outcomes by ensuring faster and more reliable fracture diagnosis while also lessening the diagnostic burden on radiologists. Subsequent investigations will focus on incorporating this system into clinical procedures and investigating its utilization in real-time emergency situations. Since there was no medical intervention on human subjects in this investigation, trial registration regulations were not applied.

Post-menopausal women are at risk of osteoporotic vertebral fractures (OVFs), which if undetected and untreated, can lead to significant pain and morbidity. However, OVFs are often not reported by radiologists on routine chest radiographs. Artificial Intelligence (AI) is increasingly used in medical applications with improvements in detection and diagnosis. This study aims to investigate the clinical value of a newly developed AI tool, Ofeye 1.0 for automated detection of OVFs on lateral chest radiographs in post-menopausal women (>60 years) who were referred to undergo chest x-rays for other reasons. A total of 510 de-identified lateral chest radiographs from three clinical sites were retrieved and analysed by Ofeye1.0 tool. These images were then reviewed by a consultant radiologist to decide whether there is any fracture present or not with findings served as the reference standard for determining the diagnostic performance of the AI tool. The percentage of radiologist reports which included the OVF was analysed by comparing diagnostic reports with AI findings, while the accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive values (NPV) of the AI tool were calculated. Out of all the original radiologist reports, missed OVFs were found in 14% images but were detected by the AI tool. The AI tool demonstrated a relatively high specificity 92.8% (95% CI: 89.6, 95.2%), moderate accuracy 80.3% (95% CI: 76,3, 80.4%), PPV 73.7% (95% CI: 65.2, 80.8%) and NPV 81.5% (95%CI: 79, 83.8%), but low sensitivity 49% (95%CI: 40.7, 57.3%). The high false negative results were mainly due to mild OVFs with <20% vertebral height loss. The new AI software tool has high specificity with a low false positive rate of 5.1%, showing that it can be used as a complimentary tool to the routine diagnostic reports for reduction of missed OVF in elderly women. The low sensitivity with high false negative rates indicate the necessity of radiologist’s in identifying and reporting early OVFs.

The Origin of the Most Energetic Galactic Cosmic Rays: Supernova Explosions into Massive Star Plasma Winds

We propose that the high energy Cosmic Ray particles around the spectral turn-down commonly called the {\it knee} and up to the upturn, commonly called the {\it ankle}, mostly come from Blue Super Giant star explosions. At the upturn, i.e. the {\it ankle}, Cosmic Rays probably switch to another source class, most likely extragalactic sources. To show this we recently compiled a set of Radio Supernova data to list the magnetic field, shock speed and radius scale (Biermann et al. 2018) \cite{Biermann18}. Using particle acceleration theory at shocks, those numbers can be transformed into characteristic {\it knee} and {\it ankle} energies. Without adjusting any free parameters both of these observed energies are directly indicated by the supernova data. We now proceed to the next step in the argument, and use the Supernova Remnant data of the starburst galaxy M82. Assuming that they are Blue Supergiant star explosions, the shock will race to their outer edge with a magnetic field that follows ${B (r) \, r \, \sim \, const.}$. We argue that the shock runs through the entire magnetic plasma wind region at full speed all the way out to the wind-shell, which is of order parsec scale. The speed is observed to be $\sim \, 0.1 \, c$ at about ${10^{16} \, {\rm cm}}$ radius in the plasma wind. This demonstrates how Blue Supergiant star explosions can provide the Cosmic Ray particles across the {\it knee} and up to the {\it ankle} energy range. The data from the CREAM (Cosmic Ray Energetics and Mass Experiment) mission will test this cosmic ray concept which is reasonably well grounded in two independent radio supernova data sets. The next step in developing our understanding is to obtain accurate Cosmic Ray data near to the {\it knee}, and use unstable isotopes of Cosmic Ray nuclei at high energy to probe the "piston" driving the explosion. We plan to combine these data with the physics of the budding black hole which is probably forming in each of these stars to learn more.
