Background: Statistics from the Ministry of Health and Welfare reported oral cancer as one of the most prevalent malignant cancers, with the third highest incidence and the fifth leading cause of death among men in Taiwan. Lymph node metastasis in oral cancer usually has a low survival rate, with no significant improvement in the past 30 years. Therefore, a more effective survival predictor is warranted. Many cancer studies revealed that monitoring tumor thickness and lymph node density, in addition to tumor, node, and metastasis (TNM) stages, can provide more accurate predictions.Methods: This retrospective study analyzed data from 612 patients with oral cancer who had the habit of chewing betel nuts. The study focused on tumor thickness, lymph node density, and the regional distribution of lymph node metastasis to determine their effectiveness as predictors.Results: The results revealed that a tumor thickness of 6 mm indicated cervical lymph node metastasis and was the optimal cutoff point for overall survival. The optimal cutoff value for lymph node density was 0.04. Patients with a tumor thickness of >6 mm and a lymph node density of >0.04 had significantly lower overall survival rates. Additionally, patients with >1 lymph node metastasis level and lower cervical metastasis exhibited a relatively worse prognosis.Conclusion: Therefore, in addition to TNM staging, tumor thickness, lymph node density and metastasis level are suitable as a parameter for predictors that can be used as references for adjuvant therapies for better therapeutic effects.

Background A limited lymphadenectomy may make patients with esophageal squamous cell carcinoma (ESCC) at risk for missing occult nodal disease. This study aimed to determine an effective threshold of lymph nodes (LNs) for evaluation of the quality of lymphadenectomy and discuss the impact of adjuvant chemotherapy on pN0 patients. Methods Patients treated surgically for ESCC between 2010 and 2018 in Sichuan Cancer Hospital were included. NPV (Negative Predictive Value) based on a beta-binomial distribution was developed to assess the confidence of node-negative disease. Kaplan-Meier curves were plotted to compare survival differences among groups. Results When patients had >21 LNs examined, the probability of false-negative findings was estimated at <10%, and NPV was estimated at a high level (>86.5%). For patients with LNs removed ≤21, 5-year OS (or DFS) in postoperative chemotherapy group vs that in surgery group was 65.3% vs 54.6%, p = 0.041 (or 59.2% vs 46.6%, p = 0.033). But for patients with >21 LNs, there were no statistical differences (p = 0.683 for OS or p = 0.942 for DFS). Conclusion Only pN0 patients who were at high risk of occult lymph node metastasis (examined LNs ≤21) could benefit from postoperative chemotherapy.

Introduction: The aim of the study was to test and validate the performance of the 2012 Briganti nomogram as a predictor for pelvic lymph node invasion (LNI) in men who underwent radical prostatectomy (RP) with extended pelvic lymph node dissection (PLND), to examine their performance and to analyse the therapeutic impact of using different nomogram cut-off . Material and methods: The study group consisted of 222 men with clinically localized prostate cancer (PCa) who underwent RP with ePLND between 01/2012 and 10/2018. Measurements included: preoperative PSA, clinical stage (CS), primary and secondary biopsy Gleason pattern and percentage of positive cores. The area under curve (AUC) of the receiver operator characteristic analysis was appointed to quantify accuracy of the primary nomogram model to predict LNI. The extent of estimation associated with the use of this model was graphically depicted using calibration plots. Results: The median number of removed lymph nodes was 16 (IQR 12-21). A total of 53 of 222 patients (23,9%) had LNI. Preoperative clinical and biopsy characteristics differed significantly (all p&lt;0.005) between men with and without LNI. A nomogram-derived cut-off of 7% could lead to a reduction of 43% (95/222) of lymph node dissection, while missing 19% (10/53) of patients with LNI. The sensitivity, specificity, and negative predictive value associated with the 7% cut-off were 81.1%, 50.3%, and 96.3%, respectively. Conclusions: Analysed nomogram demonstrated high accuracy for LNI prediction. A nomogram-derived cutoff of 7% confirmed good performance characteristics within a first external validation cohort from Poland.

Previous bio-mapping studies of pork processing facilities have indicated a need for mitigation strategies in trim and ground products due to Salmonella prevalence. Lymph nodes have been identified as a source of Salmonella in pork products. The objective of this study was to determine if the removal of glands and lymph nodes in pork would reduce Salmonella and indicator organisms in comminuted products. Samples from a commercial pork facility were divided into three treatment groups; 1) untreated control, 2) topical glands removed before final processing, and 3) topical, jowl, and internal lymph nodes and glands removed before final processing. Samples were evaluated for indicator organisms and quantity of Salmonella using the BioMérieux TEMPO® system and BAX® System Real-Time Salmonella SalQuant™ methodology, respectively. The results of this study indicate that the removal of the topical, internal, and jowl lymph nodes was effective at significantly (P &gt; 0.05) reducing the prevalence of both Salmonella and indicator organisms. Salmonella was reduced from 2.5-Log CFU/Sample and 3.8-Log CFU/Sample of Salmonella in control group ground and trim samples, to less than 1-Log CFU/sample in both matrices from treatment 3. These data indicate that samples from treatment 3 had an average of a 3-Log reduction, and treatment 2 had a 2.5-Log reduction, of Salmonella when compared to the control samples. This indicates that the physical removal of glands and lymph nodes can influence Salmonella and indicator organism prevalence in the final product.

Mitochondrial alternative oxidase 1a (AOX1a) plays an extremely important role in critical node of seed viability. However, the regulatory mechanism is still poorly understood. The study aims to identify regulatory mechanisms by comparing between OsAOX1a-RNAi and wild type (WT) rice seed during artificial aging treatment. Gain weight and P50 significantly decreased in OsAOX1a-RNAi rice seed, indicating that there might be impaired in seed development and storability. Compared to WT seeds in the 100%, 90%, 80%, and 70% germination, respectively, NADH and succinate-dependent O2 consumption, the activity mitochondrial malate dehydro-genase and ATP contents were decreased in OsAOX1a-RNAi seeds in the 100%, 90%, 80%, and 70% germination, respectively, indicating that mitochondrial status in the OsAOX1a-RNAi seeds after imbibition was weaken than the WT. In addition to, the reduced abundance of complex I N and P module subunits might showed that the capacity of mitochondrial electron transfer chain was significantly inhibited in the OsAOX1a-RNAi seed at critical node of seed viability. Above results might indicated that the ATP production was impaired in OsAOX1a-RNAi seeds during ageing. Therefore, we conclude that the activities of mitochondrial metabolism and alternative pathways were severely inhibited in OsAOX1a-RNAi seeds at critical node of viability, which was leading to accelerate the collapse of seed viability. The precise regulatory mechanism of the alternative pathway at the critical node of viability still needs to be further analyzed.

Lymph nodes are secondary lymphoid organs that appear as bean-like nodules usually <1 cm in size, and they are localized throughout the body. Many antigen-presenting cells such as dendritic cells and macrophages reside in lymph nodes, where they mediate host defense responses against pathogens such as viruses and bacteria. In cancers, antigen-presenting cells induce cytotoxic T lymphocytes (CTLs) to react to cancer cell–derived antigens. Macrophages located in the lymph node sinus are of particular interest in relation to anti-cancer immune responses because many studies using both human specimens and animal models have suggested that lymph node macrophages play a key role in activating anti-cancer CTLs. The regulation of lymph node macrophages therefore represents a potentially promising novel approach in anti-cancer therapy.

With the development of satellite load technology and very-large-scale integrated (VLSI) circuit technology, on-board real-time synthetic aperture radar (SAR) imaging systems have facilitated rapid response to disasters. Limited by severe size, weight, and power consumption constraints, a key challenge of on-board SAR imaging system design is to achieve high real-time processing performance. In addition, with the rise of multi-mode SAR applications, the reconfiguration of the on-board processing system is beginning to receive widespread attention. This paper presents a multi-mode SAR imaging chip with SoC architecture based on the reconfigurable double-operation engines and multilayer switching network. We decompose the commonly used extend chirp scaling (CS) SAR imaging algorithm into 8 types of double-operation engines according to the computing orders, and design a three-level switching network to connect these engines for data transition. The CPU is responsible for engine scheduling based on data flow driven with instructions to implement each part of the CS algorithm. Thus, multi-mode floating-point SAR imaging processing can be integrated into a single Application-Specific Integrated Circuit (ASIC) chip instead of relying on distributed technologies. As a proof of concept, a prototype measurement system with chip-included board is implemented, and the performance of the proposed design is demonstrated on Chinese Gaofen-3 stripmap continuous imaging. A chip requires 9.2 s, 50.6 s and 7.4 s for a stripmap with 16,384×16,384 granularity, multi-channel stripmap with 65.536×8192 granularity and multi-channel scan mode with 32,768×4096 granularity and 6.9 W for the system hardware to process the SAR raw data.

Dendritic cells (DCs) are the most specialized antigen-presenting cells and lymph nodes (LNs) play an important role in the DC-mediated T-cell response. We evaluated the infiltration of CD1a-positive DCs (CD1a-DCs) i.e. immature DCs, and S100-positive dendritic cells (S100-DCs), a mixture of immature- and mature DCs, in 73 cases of laryngeal cancer and its regional LNs. Among them, 31 patients underwent radiotherapy (RT) or chemoradiotherapy (CRT) prior to surgery. No significant difference was found for CD1a-DC infiltration in the primary tumor, metastatic LNs and non-metastatic LNs, while S100-DCs were significantly fewer in number in the primary tumor and metastatic LNs compared to non-metastatic LNs. Cases which showed a high infiltration of S100-DCs in metastatic LNs appeared to show a favorable prognosis although statistical significance was not reached. In the RT/CRT group, infiltration of CD1a-DCs and S100-DCs were fewer in the primary tumor and metastatic LNs compared to the treatment naive group. Conversely, the RT/CRT group showed higher CD1a-DCs and S100-DCs numbers in non-metastatic LNs compared to the treatment naïve group. Thus, DC maturation in metastatic LNs plays an important role in tumor immunity in laryngeal cancer, and infiltration of DCs into the primary tumor and metastatic LNs is impaired by RT/CRT.

Background: The management of penile cancer patients has some grey zones, one of them being the timing of prophylactic pelvic lymph node dissection. Here, we review the impact of timing of prophylactic inguinal and pelvic lymph node dissection on patients’ outcome. Methods: Relevant databases were searched according to recommendations by the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. Before the systematic review we present a narrative review on the indications for lymph node dissection and the impact of pathological anatomical considerations on the timing of prophylactic lymph node dissection and patients’ outcome. The primary research questions were disease-free and overall survival rate in patients who underwent early or late lymph node dissection after penile cancer diagnosis. Results: Four clinical trials were included in the systematic review. Conclusions: The survival rate of patients with penile cancer is strictly associated with the timing of prophylactic pelvic lymph node dissection. Even though there are no randomized controlled trials we conclude that there is evidence for a strong recommendation that lymph node dissection should be performed as soon as possible after diagnosis with3 months as a realistic cut-off time between early and late. According to the present evidence base, lymph node dissection should be offered the sooner the better to all patients at high risk of lymph node metastasis.

Lymph node invasion in prostate cancer is a significant prognostic factor indicating worse prognosis. While it affects significantly both survival rates and recurrence, proper management remains a heated issue. Thorough evaluation of risk factors associated with nodal involvement, such as lymph node density or extracapsular extension, is crucial to establish potential expansion of the disease and to substratify patients clinically. There are multiple strategies that may be taken into consideration for patients with positive lymph nodes. Nowadays therapeutic methods are generally based on observation, radiotherapy, and androgen deprivation therapy. However current guidelines are incoherent in terms of indication of the most effective management approach. Future management strategies will be expected to reach for novel diagnostic tools and therapies, such as photodynamic therapy or diagnostic imaging with prostate specific membrane antigen. Nevertheless, this heterogeneous group of men remains a vast therapeutic concern, and both clarification of the guidelines and optimal substratification of patients is required.

This work introduces the mathematical framework of the novel “First-Order Comprehensive Adjoint Sensitivity Analysis Methodology for Neural Ordinary Differential Equations (1st-CASAM-NODE)” which yields exact expressions for the first-order sensitivities of NODE decoder-responses to the NODE parameters, including encoder initial conditions, while enabling the most efficient computation of these sensitivities. The application of the 1st-CASAM-NODE is illustrated by using the Nordheim-Fuchs reactor dynamics/safety phenomenological model, which is representative of physical systems that would be modeled by NODE while admitting exact analytical solutions for all quantities of interest (hidden states, decoder outputs, sensitivities with respect to all parameters and initial conditions, etc.). This work also lays the foundation for the ongoing work on conceiving the “Second-Order Comprehensive Adjoint Sensitivity Analysis Methodology for Neural Ordinary Differential Equations (2nd-CASAM-NODE)” which aims at yielding exact expressions for the second-order sensitivities of NODE decoder-responses to the NODE parameters and initial conditions while enabling the most efficient computation of these sensitivities.

Dendritic cells (DCs) are known as major antigen-presenting cells, and lymph nodes (LNs) play an important role in the DC-mediated immune response. This study focused on infiltration of CD1a-positive DCs (CD1a-DCs) in regional LNs in 70 cases of gallbladder cancer (GBC). The results showed that LNs infiltration by CD1a-DCs was associated with unfavorable clinical outcomes in patients with GBC in univariate analyses, but all cases categorized as CD1a-DCs high group at regional LNs had nodal metastasis. Therefore, LNs infiltration by CD1a-DCs was not an independent prognostic factor identified by multivariate analyses. The subgroup analyses in cases with LN metastasis group (n = 32), significant impact of CD1a-DCs infiltration into metastatic LNs was not observed. In contrast, CD1a-DCs infiltration into primary tumor had a significant impact on surgical outcomes. As the results of previous reports that focused on CD1a-DCs infiltration in regional LNs of other organs were varied, the role and significance of CD1a-DCs infiltration in regional LNs may be different according to the tumor histology or primary site. Accumulation of further studies is needed to clarify the role and significance of CD1a-DCs infiltration into regional LNs of solid cancers.

This paper outlines the detection procedure of intrusion as provided by Tripwire tool along-with the enhancement to be made in design and implementation to achieve the optimum performance level of Tripwire. It operates on effective and swift performance mechanism in order to report the system admin about the possible intrusion detected in the system with security assurance in cutting edge computer structures, which is very important in order to provide the integrity and reliability of information. In order to deal with this emerging issue in the historical research, the basic level of security is provided in the system by making enhancement in the existing tripwire tool for UNIX file system. The proposed methodological working of tripwire makes it more effective by adopting reliable and instantaneous mechanism to deal with specifically anomalies and unwanted access in the UNIX file system.

The heterogeneous structure implies that a few nodes may be crucial in maintaining network structural and functional properties. Identifying these crucial nodes correctly and quickly is a primary issue as contemporarily may face the mushrooming of large-scale datasets. Besides, the ‘weight issue’ is always ignored in this field which edge weight may play a positive/negative role in contributing to the node importance in different weighted networks. This paper provides a novel algorithm, Weighted Expectation Algorithm (WEA), which aims to ensure accuracy and speed of computation by taking advantage of dynamic programming to better handle the task of large-scale networks. Additionally, the weight issue that edge weights may contribute differently is addressed by a simply quantitative definition. Two standard experiments show WEA can maintain the network structure in connectivity well (by the lowest average robustness 0.192) and identify the node importance better in the spreading function test of spreading dynamics (by the highest average Kendall’s tau-b 0.678). In addition, the time complexities of different algo-rithms are evaluated, and their time-consuming are tested, proving that WEA consumes a rela-tively short time.

The purpose of this review is to summarize current knowledge on lymph node dissection (LND) in prostate cancer (PCa) patients undergoing radical prostatectomy (RP). Despite a growing body of evidence, utility, therapeutic and prognostic value of such approach as well as optimal extent of LND, remain unsolved issues. Although LND is the most accurate staging procedure, the direct therapeutic effect is still not evident from the current literature which limits the possibility of establishing clear recommendations. This indicates the need for further robust and adequately designed high quality clinical trials.

Penile cancer (PeCa) is a rare urological malignancy characterized by significant geographic variations in both incidence and mortality rates. Due to its rarity and the consequent lack of randomized trials, current management is based on retrospective studies and small prospective trials. In addition, the diagnostic pathways and treatment strategies exhibit substantial heterogeneity, differing significantly between less developed and well-developed countries. The prognosis of PeCa is determined by the presence and extent of regional lymph node (LN) involvement. Therefore, early detection and treatment of LN metastasis is paramount to ensure better outcomes. In recent decades, the overall survival for PeCa has increased, mainly due to advancements in imaging techniques and risk stratification. We aim to provide an overview of the current role of PET CT imaging in the management of patients with PeCa.

The growing of Brain-Computer Interface (BCI) applications is closely related to the increasing accessibility of Electroencephalography (EEG) hardware (EEG headsets), which are noninvasive, portable, wireless and often with open software. However, there is a limited number of BCI software platforms tailored to help inexperienced programmers in the development of BCI applications. Only few of them integrate BCI applications with IoT devices and services. To address these challenges, a model for visual node-based programming has been designed utilizing BrainFlow library within the Node-RED platform, which can be applied to more than 20 biosensors. New Nodes for Node-RED Library within OpenBCI Category (openBCI toolkit for Node-RED) have been developed for design of EEG-Based Brain-Machine Interface and Integration in IoT. The proposed toolkit have been implemented and validated through a case study for controlling a robotic arm by OpenBCI headset. The results from the pilot experiment demonstrated that through concentration levels classified by BrainFlow performance metrics, the control of a TinkerKit Braccio robot arm is possible.

Although first described in the final decade of the 19th century, the axis responsible for atrioventricular conduction has long been the source of multiple controversies. Some of these continue to reverberate. When first described by His, for example, many doubted the existence of the bundle we now name in his honour, while Kent suggested that multiple pathways crossed the atrioventricular junctions in the normal heart. It was Tawara who clarified the situation, although many of his key definitions have not universally been accepted. In key studies in the third decade of the 20th century, Mahaim then suggested the presence of ubiquitous connections that provided "paraspecific" pathways for atrioventricular conduction. In this review, we show the validity of these original investigations, based on our own experience with a large number of datasets from human hearts prepared by serial histological sectioning. Using our own reconstructions, we show how the atrioventricular conduction axis can be placed back within the heart. We emphasise that newly emerging techniques will be key in providing the resolution to map cellular detail to the gross evidence provided by the serial sections

This study aimed to evaluate the optimal extent of lymphadenectomy in patients with nonfunctioning pancreatic neuroendocrine neoplasms. We retrospectively analyzed the clinicopathological data of patients with nonfunctioning pancreatic neuroendocrine neoplasms who underwent surgical resection. We investigated the frequency of metastases at each lymph node station according to tumor location and analyzed the factors contributing to poor overall survival (OS) and disease-free survival (DFS). Overall, data of 84 patients were analyzed. For pancreatic head tumors, metastases at stations 8, 13, and 17 were found in one (3.1%), four (12.5%), and three (9.3%) patients, respectively. However, none of the other stations showed metastases. For pancreatic body and tail tumors, only metastases at station 11 were found in two (5.1%) patients. Additionally, multivariate DFS and OS analyses showed that lymph node metastasis was the only independent prognostic factor. Lymph node metastasis was found near the primary tumor and was the only independent factor of poor prognosis in patients with nonfunctioning pancreatic neuroendocrine neoplasms after undergoing curative surgery. Peri-pancreatic lymphadenectomy might be recommended for nonfunctioning pancreatic neuroendocrine neoplasms.

Epithelioid hemangioendothelioma (EHE) is an extremely rare vascular sarcoma with variable aggressive clinical behavior. In this retrospective study we aimed to investigate prognostic factors based on clinicopathologic findings in a molecular/immunohistochemical confirmed nationwide multicenter cohort of 57 EHE. Patients had unifocal disease (n=29), multifocal disease (n=5), lymph node metastasis (n=8) and/or distant metastasis (n=15) at the time of diagnosis. The overall survival rate was 69.6% at 1 year and 50.5% at 5 years. Survival did not correlate with sex, age or histo-pathological parameters. No survival differences were observed between multifocal and metastatic disease suggesting that multifocality represents early metastases and treatment options are limited in comparison to unifocal disease. In unifocal tumors, survival could be predicted using the risk stratification model of Shibayama et al. dividing the cases into low (n=4), intermediate (n=15) and high (n=3) risk-groups. Larger tumor size (>3cm) and higher mitotic count (>1/10HPF) was asso-ciated with progressive unifocal disease course. Lymph node metastases at time of diagnosis oc-curred in 14.0% of the cases and was mainly associated with tumor localization in the head and neck area proposing lymph node dissection. In conclusion, our results demonstrate the aggressive behavior of EHE, emphasize the prognostic value of a previous described risk stratification model and may provide new insights regarding tumor focality, therapeutic strategies and prognosis.

Histologically poor differentiation is associated with lymph node metastasis. Thus, pathological evaluation of biopsy specimens is crucial when treating stomach cancers. Deep learning of WSIs is challenging because the images are enormous. Given the computing limitations, patch-level supervised learning methods have been proposed. However, valuable information is lost when dividing WSIs into smaller patches. Another drawback is the need for pixel-level annotation by a pathologist. It is acceptable to differentiate, i.e., grade, gastric cancer at the holistic tissue level (i.e., under low magnification). We developed a weakly supervised learning technique for tissue-level gastric adenocarcinoma histological differentiation (well-to-moderately or poorly differentiated) and applied global reasoning to tissue-level features. The tissue-level AUROCs of the histological differentiation classifiers were 0.953, 0.969, and 0.943, respectively when data from five hospitals were subjected to threefold cross-validation. Comparison of the Grad-CAM heatmaps of the trained classifier and the pathologists’ annotations confirmed that our weakly supervised model exhibited performed well.

Stem cells are nurtured and regulated by a specialized microenvironment known as stem cell niche. While the functions of the niches are well defined, their structure and location remain unclear. We have identified in rat bone marrow, the seat of hematopoietic stem cells, extensively vascularized node-like compartments that fit the requirements for stem cell niche and which we called hemmules. Hemmules are round or oval structures of about one millimeter in diameter that are surrounded by a fine capsule, have afferent and efferent vessels, are filled with the extracellular matrix and mesenchymal, hematopoietic, endothelial stem cells, and contain cells of the megakaryocyte family, which are known for homeostatic quiescence and contribution to the bone marrow environment. We propose that hemmules are the long sought hematopoietic stem cell niches and that they are prototypical of stem cell niches in other organs.

The incidence of non-palpable breast cancer is increasing due to widespread screening and neoadjuvant therapies. Among the available tumor localization techniques, radio-guided occult lesion localization (ROLL) has largely replaced wire-guided localization (WGL). This study aimed to compare ROLL and WGL techniques in terms of the effectiveness of isotopic marking of axillary sentinel lymph nodes and to assess patient perspectives along with surgeon and radiologist preferences. A single-center, prospective randomized study enrolled 110 patients with non-palpable breast lesions (56 ROLL, 54 WGL). Breast type, tumor volume, location, histological and radiological features, and localization/surgical duration were evaluated in the context of sentinel lymph node marking using isotope (Technetium-99m labelled human serum albumin) and blue dye. Statistical analysis was performed with significance set at p&lt;0.05 and strong significance at p&lt;0.01. Both techniques were successful in localizing non-palpable lesions (100%). However, patients reported WGL as more painful. A notable difference was observed in axillary lymph node marking, with subareolar isotope injection (used in WGL) showing better results compared to ROLL. While ROLL provided advantages in terms of patient comfort, and logistical simplicity, WGL was superior for axillary sentinel lymph node marking, particularly in inner quadrant tumors, suggesting WGL may be preferred for these cases.

The 6TiSCH protocol (IEEE 802.15.4e) is crucial for the Industrial Internet of Things (IIoT), utilizing a time-slotted channel hopping (TSCH) mode based on node distribution. In our experiment, we explored node position clustering and adjusted the capacity of existing cells in congested areas to understand traffic patterns and optimize cell selection for TSCH resource allocation, particularly in slot frames. Efficient cell allocation prevents packet loss, manages congestion, and enhances data transmission reliability. Node clustering increases cell usage efficiency and reduces communication latency between nodes, with each cell allocation tailored to its traffic needs through the sink node from the clustering result. We evaluated the clustering method using the K-means algorithm through experiments using the 6TiSCH simulator. Additionally, we considered using the routing protocol for low power and lossy networks (RPL) as a comparison, such as OF0 and MRHOF, as the objective function to assess clustering results and their impact on throughput and packet delivery. Our method significantly improved average performance metrics: under the OF0 routing protocol, we achieved a 30.01% latency reduction, 15.95% faster joining time, 8% higher packet delivery ratio, and 13.82% throughput increase. Bitcoin has been launched for over a decade and made an increasing impact on the world’s financial order, which attracted extensive attention of researchers. Bitcoin system runs on a dynamic P2P network, containing tens of thousands of nodes including reachable nodes and unreachable nodes. In this article, a detection system BNS (Bitcoin Network Sniffer) was prososed, which could collect as many Bitcoin nodes as possible. For reachable nodes, the authors designed an algorithm BRF (Bitcoin Reachable-nodes Finding) based on node activity evaluation, which reduced the nodes to be detected and greatly shortened the detection time. For unreachable nodes, the authors trained a dicision tree model BUF(Bitcoin Unreachable-nodes Finding) to identify unreachable nodes based on attribute features from massive node addresses. Experiments showed that BNS performed better than the website "Bitnodes" in total number and efficiency. Based on the experimental results, the authors analyzed the real network size, node "churn" and geographical distribution.

Network neuroscience delves into comprehending the intricate arrangement and interactions within the neural networks of the brain. By merging insights from both neuroscience and network theory, it investigates the brain's structure and function through the lens of interconnected nodes (symbolizing brain regions) and edges (symbolizing the connections among these regions) forming a dynamic network. Graph metrics play a pivotal role in neuroscience by providing quantifiable insights into the intricate connectivity patterns within the brain's neural networks. Frequently, nodes are extracted from functional or structural atlases, which can lead to diverse shapes and sizes. Nevertheless, our understanding of how these differing node characteristics and definitions impact the computed graph metrics in neuroimaging data remains limited. This study adopts a data-driven methodology to delineate functional nodes, subsequently examining the influence of their sizes on resultant graph metrics. By employing the Neuromark framework, an entirely automated independent component analysis (ICA) is applied to resting state fMRI data. Through this, functional network connectivity (FNC) matrices are computed—capturing Pearson correlations between component-time courses—while employing a proportional threshold to streamline node connections based on these correlations. Subsequently, various global and local graph metrics are computed, offering insights into network characteristics. Global metrics provide an overarching summary of the network structure, while local measures delve into structural intricacies at the individual component level. Node sizes are computed based on voxel counts surpassing a designated threshold. Next, we calculated the Pearson correlation between the obtained node sizes and the graph metrics, which we term 'node-metric coupling' (NMC). Our findings revealed consistent and noteworthy correlations between the values of graph metrics and the dimensions of brain nodes. To delve deeper into the implications of this correlation, we examined the node-metric coupling within a dataset comprising Alzheimer's disease, mild cognitive impairment, and control subjects. The disparities observed in node-metric coupling among these groups underscore the necessity of accounting for this factor during analysis. The two principal outcomes of this study are as follows: Firstly, the substantial interplay between varying node sizes within a given atlas and the resultant graph metrics; and secondly, the potential utility of node-metric coupling as a viable biomarker for brain disorders. These discoveries hold significant ramifications. While comparing studies employing diverse atlases is already a challenge, our work highlights an additional, critical source of variability: accounting for node sizes. Consequently, the association between intra-atlas node size and graph metric should be thoughtfully acknowledged in future neuroimaging investigations.

In view of the Practical Byzantine Fault Tolerance (PBFT) consensus algorithm all nodes participate in the consensus, the three-stage process has problems such as large communication overhead, random selection of master nodes and lack of reward and punishment mechanism. A Speculative Practical Byzantine Fault Tolerance (SP-PBFT) is proposed. Firstly, the consensus protocol was optimized, and a timeout mechanism was proposed to divide the consensus process into optimistic mode and pessimistic mode, which reduced the communication overhead under the premise of resisting Byzantine node attacks .Secondly, a node evaluation mechanism was proposed to calculate the node reputation value according to the node's historical behavior and give the corresponding reward or punishment, which reduced the possibility of malicious nodes participating in the consensus process. Finally, the Fibonacci grouping mechanism was used to reduce the number of nodes participating in the consensus process, fundamentally improving the consensus efficiency, avoiding the problem of centralization of the consensus process caused by the cumulative reputation value of nodes, and improving the enthusiasm of consensus nodes. Simulation experiments using Docker containers to simulate multiple nodes show that the SP-PBFT consensus algorithm proposed in this paper has better performance than PBFT consensus algorithm and other improved algorithms in terms of consensus delay, throughput, fault tolerance and communication complexity.

We aimed to assess the prognostic value of BRAFV600E mutation in a series of 127 papillary thyroid carcinoma (PTC) cases as a single factor, as well as in synergic interaction with other standard risk factors. All cases were tested for BRAFV600E mutation by real-time PCR. Event-free survival (EFS) was calculated between the date of the first evaluation and the date of occurrence of an adverse event (tumor recurrence/distant metastasis) or the date of the last known status. The prevalence of BRAFV600E mutation was 57.2%. The Kaplan-Meyer analysis showed a significant reduction of EFS among cases harboring BRAFV600E mutation compared to non-mutated cases (p=0.010). In addition, BRAFV600E mutation was found to better predict adverse outcomes when associated with the following risk factors: age≥55 years-old (p<0.001), male gender (p<0.001), conventional (p=0.005) and tall cell (p=0.014) histology, tumor size&gt;40mm (p=0.001), extrathyroidal extension (p=0.001), multifocality (p=0.001) and lymph node metastasis (p&lt;0.001). In univariate analysis, a 3.74-fold increased risk for a reduced EFS (p=0.018) was found for BRAFV600E mutated cases. Our results highlight the prognostic value of BRAFV600E mutation in PTCs. Moreover, the synergic interaction between BRAFV600E mutation and other risk factors seems more valuable in terms of prognosis, compared to BRAFV600E mutation alone.

Background: The lymph nodes staging system can predict the prognosis of gastric signet ring cell carcinoma (SRCC), but the optimal system for early and advanced SRCC remains unknown. Methods: This study retrospectively analyzed 693 SRCC patients who underwent radical resection in the Department of Gastrointestinal Surgery, Harbin Medical University Cancer Hospital. The predicted performance of three lymph node staging systems, including pN staging, lymph node metastasis rate (LNR), and log odds of positive lymph nodes (LODDS), was compared using the receiver characteristic operating curve (ROC). The kaplan⁃meier method and the log⁃rank test analyze the overall survival of patients. The Cox risk regression model identifies independent risk factors associated with patient outcomes. The nomogram was made by R studio. Results: The 693 SRCC included 165 early SRCC and 528 advanced SRCC. ROC showed that LODDS had better predictive performance than pN and LNR in predicting prognosis regardless of early or advanced SRCC. LODDS can be used to predict the prognosis of early and advanced SRCC and was an independent risk factor associated with patient outcomes (P=0.002, P<0.001). Furthermore, the nomogram constructed by LODDS and clinicopathological features had good predictive performance. Conclusion: LODDS showed a clear prognostic superiority over both pN and LNR in early and advanced SRCC.

The error between both is less than 1%, better than the empirical adjustment to issue a set of drive current models.

As Heterogeneous Wireless Sensor Network (HWSN) fulfill the requirements of researchers in the design of real life application to resolve the issues of unattended problem. But, the main constraint face by researchers is energy source available with sensor nodes. To prolong the life of sensor nodes and hence HWSN, it is necessary to design energy efficient operational schemes. One of the most suitable routing scheme is clustering approach, which improves stability and hence enhances performance parameters of HWSN. A novel solution proposed in this article is to design energy efficient clustering protocol for HWSN, to enhance performance parameters by EECPEP-HWSN. Propose protocol is designed with three level nodes namely normal, advance and super node respectively. In clustering process, for selection of cluster head we consider three parameters available with sensor node at run time, i.e., initial energy, hop count and residual energy. This protocol enhance the energy efficiency of HWSN, it improves performance parameters in the form of enhance energy remain in the network, force to enhance stability period, prolong lifetime and hence higher throughput. It is been found that proposed protocol outperforms than LEACH, DEEC and SEP with about 188, 150 and 141 percent respectively.

Water pollution is a major global problem and the deterioration of water quality in rivers in many situations is becoming more widespread day by day, this causes a serious threat to all living beings and is eventually leading to the extinction of many aquatic species and others that depend on them. Therefore, with the advancement of Internet of Things (IoT) and Wireless Sensor Networks (WSN), monitoring the state of river water, for taking quick action to safeguard rivers becomes a pressing need, and is the main objective. An investigation was made into some existing IoT technologies and other related work to help achieve the objective of this project. In this paper we develop a low-cost solution based on Long Range (LoRa) to check in real-time, with specific sensors, some water parameters such as temperature, pH, conductivity, and turbidity. The data collected in a sensor node is sent via LoRa modulation to a gateway, where it is processed, forwarded to a local Message Queuing Telemetry Transport (MQTT) server, displayed on a local Node-RED dashboard, and alternatively it can be sent to the Internet. The developed prototype system is tested in a real scenario and proves that it can reliably monitor the water quality of a river in a reasonably sized area.

Criticism has been directed towards the traditional methods of management accounting employed by organizations thus far, as they fail to adequately address the growing need for competitiveness and sustainable development in a globalized environment. This criticism stems from both economic factors, such as insufficient monitoring of material, energy, and escalating overhead costs, and environmental concerns, where inadequate attention is given to vital information regarding significant environmental aspects, impacts, risks, and damages arising from the organization's activities and processes. Additionally, these methods do not sufficiently support the implementation of a functional environmental management system (EMS). Drawing upon our research in EMS standardization following the international standards ISO 1400X, as well as our knowledge gained during the development of the Slovak Technical Standard (STN) for ISO 14051, this paper presents an innovative model of environmental management accounting. This model is based on the cost accounting of material and energy flows within organizations. It serves as an enhancement framework for organizations with an existing EMS built according to the EN ISO 14001 model, as well as for organizations lacking a formalized EMS based on this standard. Within this paper, we specify, decompose, and apply the new model, focusing on the cost accounting of material-energy flows in a hypothetical organization. We meticulously examine the ten key steps of the model's structure, particularly in terms of costing and the allocation of costs to material, energy, system, and waste management.

As a new distributed machine learning (ML) approach, federated learning (FL) shows the great potential to preserve data privacy by enabling distributed data owners to collaboratively build a global model without sharing their raw data. However, the heterogeneity in terms of data distribution and hardware configurations make it hard to select participants from the thousands of nodes. In this paper, we propose a multi-objective node selection approach to improve time-to-accuracy performance while resisting malicious nodes. We firstly design a deep reinforcement learning assisted FL framework. Then the problem of multi-objective node selection under this framework is formulated as a Markov decision process (MDP), which aims to reduce the training time and improve model accuracy simultaneously. Finally, a deep Q-netwok (DQN) based algorithm is proposed to efficiently solve the optimal set of participants for each iteration. Simulation results show that the proposed method not only significantly improves the accuracy and training speed of FL, but has stronger robustness to resist malicious nodes.

Marangoni flow in foam fractionation in the lamellar film for the interior and exterior of a micro-foam was investigated. The three-dimensional node-film-Plateau Border system was modeled using computational fluid dynamics. The importance of the surfactant concentration of the foam fractionation column and air-liquid interface mobility on the Marangoni velocity in the film was emphasized. The study found that an increase in surfactant concentration in the reflux column significantly increases the Marangoni velocities. Additionally, a mobile interface results in a higher Marangoni flow, while a rigid interface leads to less intensive flow. The behavior of the Marangoni flow in both interior and exterior foam was explored, revealing that the flow in exterior foam has different behavior due to the presence of the wall, which reduces the Marangoni velocity compared to interior films.

Introduction: The aim of our study was to evaluate the efficacy and toxicity of a daily-adaptive MR-guided SBRT on 1.5 T MR-linac in patients affected by lymphnode oligometastases from PCa. Materials and Methods: The present study is a prospective observational study conducted in a single institution (protocol n°: MRI / LINAC n. 23748). Patients with oligometastatic lymphnodes from PCa treated with daily-adaptive MR-guided SBRT on 1.5T MR-linac were included in the study. Minimum required follow-up of 3 months after SBRT. Primary end-point was local progression-free survival (LPFS). Secondary end-points were: nodal progression-free survival (NPFS), and progression-free survival (PFS), and toxicity. Results: 118 lymphnode oligometastases from PCa were treated with daily-adaptive 1.5T MR-guided SBRT in 63 oligometastatic patients. 63.5% patients were oligoprogressive and 36.5% oligoprogressive. Two-year LPFS was 94.5%. Median NPFS was 22.3 months, and the 2-year NPFS was 46.5%. Having received hormone therapy before SBRT was correlated with lower NPFS at the multivariate analysis (1-y NPFS 87.1% versus 42.8%; p= 0.002 - HR 0.199, 95% CI 0.073-0.549). Furthermore, the oligorecurrent state during ADT was correlated with a lower NPFS than the oligoprogressive state. Median PFS was 10.3 months, the 2-year PFS was 32.4%. Patients treated with hormone therapy before SBRT had a significantly lower 1-year PFS the others (28% versus 70.4%; p= 0.01 - HR 0.259, 95% CI 0.117-0.574). No acute and late toxicities occurred during treatment. Conclusion: the present is the largest prospective study of 1.5T lymphnode SBRT on MR-linac in patients with PCa. Lymphnode SBRT by 1.5T MR-linac provides high local control rates with an excellent toxicity profile.

Sentinel lymph node dissection (sLND) using a magnetometer and superparamagnetic iron oxide nanoparticles (SPIONs) as a tracer was successfully applied in prostate cancer (PCa). Radioisotope-guided sLND combined with extended pelvic LND (ePLND) achieved better node removal, increasing the number of affected nodes or the detection of sentinel lymph nodes outside the established ePLND template. We determined the diagnostic value of additional magnetometer-guided sLND after intraprostatic SPION-injection in high-risk PCa. This retrospective study included 104 high-risk PCa patients (PSA >20 ng/ml and/or Gleason score ≥8 and/or cT2c) from a prospective cohort who underwent radical prostatectomy with magnetometer-guided sLND and ePLND. The diagnostic accuracy of sLND was assessed using ePLND as a reference standard. Lymph node metastases were found in 61 of 104 patients (58.7%). sLND had a 100% diagnostic rate, 96.6% sensitivity, 95.6% specificity, 96.6% positive predictive value, 95.6% negative predictive value, 3.4% false negative rate, and 4.4% false positive rate (detecting lymph node metastases outside the ePLND template). These findings demonstrate the high sensitivity and additional diagnostic value of magnetometer-guided sLND, exceeding that of ePLND through the individualized extension of PLND or the detection of sentinel lymph nodes / lymph node metastases outside the established node template in high-risk PCa.

Elevated blood glucose levels, known as glycemia, play a significant role in sudden cardiac arrest, often resulting in sudden cardiac death, particularly among those with diabetes. Understanding the internal mechanisms has been a challenge for healthcare professionals, leading many research groups to investigate the relationship between blood glucose levels and cardiac electrical activity. Our hypothesis suggests that glucose-sensing biophysics mechanisms in cardiac tissue could clarify this connection. To explore this, we adapted a single-compartment, computational model of the human atrial node's action potential. We incorporated glucose-sensing mechanisms with voltage-gated sodium ion channels using ordinary differential equations. Parameters for the model were based on existing experimental studies to mimic the impact of glucose levels on atrial node action potential firing. Simulations using voltage clamp and current clamp techniques showed that elevated glucose levels decreased sodium ion channel currents, leading to a reduction in the sinoatrial node action potential frequency. In summary, our mathematical model provides a cellular-level understanding of how high glucose levels can lead to bradycardia and sudden cardiac death.

Complex atypical endometrial hyperplasia (CAH) carries a high probability of cancer. Intraoperative evaluation of endometrial cancer in cases of CAH has not been reliable. The safety and sensitivity of sentinel lymph node (SLN) sampling has been validated. We aim to evaluate the efficacy and safety of SLN sampling in CAH managed by the da Vinci robotic platform. A total of 113 patients with a preoperative diagnosis of CAH were included in this retrospective cohort study. All underwent a robot assisted total laparoscopic hysterectomy and bilateral salpingo-oophorectomy with 69 patients undergoing SLN sampling. Statistical analysis calculated the probability of cancer, SLN map rate, and surgical complications. Predictors of cancer were evaluated. Forty-seven percent of the entire cohort were diagnosed with endometrial cancer. Median age was 63 years in the SLN cohort (N = 69) and 61 in the No SLN cohort (N = 44) (p= 0.363).  Median BMI was 34 Kg/m2 in the SLN cohort and 40 in the No SLN cohort (p= 0.004). Bilateral SLN map was 92.8% and unilateral SLN map rate was 7.2%. There were no grade 3-4 complications in the SLN cohort, and 4 grade 3-4 complications in the No SLN group (p=0.021). A preoperative diagnosis of CAH bordering on or cannot rule out cancer was the only predictor of cancer.

Megalin (LRP2) is a rapidly recycling multiligand endocytic receptor, primarily expressed in polarized epithelial cells. Despite megalin could promote tumor growth and invasiveness through several mechanisms, it has been understudied in the field of molecular oncology so far. The present study aimed to evaluate the impact of megalin expression in oral squamous cell carcinoma (OSCC) on disease progression. Megalin expression was evaluated immunohistochemically in 63 OSCC specimens. Data obtained were retrospectively compared with patients’ clinicopathological features and survival. The proportion of megalin-expressing cells in primary OSCC tissue was significantly associated with metastatic spreading to lymph nodes, vascular invasion and lower overall survival rate. Results obtained by the study suggest that megalin can be considered as a novel molecule involved in OSCC pathogenesis, but also useful as a potential biomarker for cancer progression.

In heterogeneous networks, the random walks based on meta-path requires prior knowledge and lacks flexibility. And the random walks based on non-meta-path only considers the number of node types, but does not consider the influence of schema and topology between node types in real networks. To solve the above problems, this paper proposes a novel model HNE-RWTIC (Heterogeneous Network Embedding Based on Random Walks of Type & Inner Constraint). Firstly, to realize the flexible walks, we design a Type strategy, which is the node type selection strategy based on the co-occurrence probability of node types. Secondly, to achieve the uniformity of node sampling, we design an Inner strategy, which is the node selection strategy based on the adjacency relationship between nodes. The Type & Inner strategy can realize the random walks based on meta-path, the flexibility of the walks, and can sample the node types and nodes uniformly in proportion. Thirdly, based on the above strategy, a transition probability model is constructed; then, we obtain the nodes embedding based on the random walks and Skip-Gram. Finally, in classification and clustering tasks, we conducted a thorough empirical evaluation of our method on three real heterogeneous networks. Experimental results shown that F1-Score and NMI of HNE-RWTIC outperform state-of-the-art approaches.

The use of unmanned aerial vehicle (UAV) during emergency response of a disaster has been widespread in recent years and the terrain images captured by the cameras on board these vehicles are significant sources of information for such disaster monitoring operations. Thus, analyzing such images are important for assessing the terrain of interest during such emergency response operations. Further, these UAVs are mainly used in disaster monitoring systems for the automated deployment of sensor nodes in real time. Therefore, deploying and localizing the wireless sensor nodes optimally, only in the regions of interest that are identified by segmenting the images captured by UAVs, hold paramount significance thereby effecting their performance. In this paper, the highly effective nature-inspired Plant Growth Simulation Algorithm (PGSA) has been applied for the segmentation of such terrestrial images and also for the localization of the deployed sensor nodes. The problem is formulated as a multi-dimensional optimization problem and PGSA has been used to solve it. Furthermore, the proposed method has been compared to other existing evolutionary methods and simulation results show that PGSA gives better performance with respect to both speed and accuracy unlike other techniques in literature.

Background: We investigated the significance of the para-aortic node (PALN) and other regional node metastases (RLN), reflecting malignant histological findings or poor patient prognosis, in patients with biliary duct cancers (BDC) and pancreatic carcinomas (PC) who underwent radical resections at two institutes. Methods: Conventional clinicopathological factors, including metastasis to the PALN and RLN, and surgical outcomes or long-term prognosis in 181 PC and 116 BDC patients between 1994 and 2022 were examined. Results: In patients with PC, cancer metastasis was observed in the RLN and PALN in 54% and 9% of patients, respectively. In patients with BDC, cancer metastasis was observed in the RLN and PALN in 39% and 9% of patients, respectively. Disease-free survival (DFS) and overall survival (OS) in patients with BDC and PALN metastasis were significantly lower than those without PALN metastasis; however, several patients survived over three years. Multivariate analysis revealed that, in patients with BDC, PALN metastasis was an independently related factor of OS, and RLN was an independently related factor for DFS, respectively (p

Background: Colorectal cancer (CRC) is prevalent globally, constituting 11.9% of cases in Mexico. Lymph node metastases are established prognostic indicators, with extracapsular lymph node extension (ENE) playing a crucial role in modifying prognosis. While ENE is associated with adverse factors, certain aspects, like lymph node conglomerates, are underexplored. Matted nodes, clusters of lymph nodes infiltrated by cancer cells, are recognized as an independent prognostic factor in other cancers. This study investigates the prognostic implications of lymph node conglomerates, particularly matted nodes, in colon cancer. Methods: A retrospective analy-sis of 502 colon cancer cases (2005–2018) identified 255 with lymph node metastasis. Cases were categorized into two groups: 1) lymph node metastasis alone (n=208), and 2) lymph node metas-tasis with matted nodes (n=47). A comparative survival analysis was performed. Results: Of the 255 patients, 38% had lymph node metastasis. Patients with matted nodes (18.4%) showed an association with higher pN stage and lymphovascular invasion. The 5-year survival rate for pa-tients with matted nodes was 47.7%, compared to 60% without; however, this association demonstrated only a statistical tendency. Multivariate analysis identified clinical stage and ad-juvant chemotherapy use as independent factors contributing to survival. Conclusion: This study underscores matted nodes as potential prognostic indicators in colon cancer, emphasizing their association with higher pN stage and reduced survival. Further research is essential for validat-ing these findings and integrating matted nodes into the broader context of colorectal cancer management.

The implementation of sentinel lymph node (SLN) biopsy is changing the scenario in the surgical treatment of early-stage cervical cancer and the oncologic safety of replacing bilateral pelvic lymphadenectomy with SLN biopsy is currently under investigation. One of the undisputed values of SLN biopsy is its diagnostic accuracy in detecting low-volume metastases (LVM) via pathologic ultrastaging. In early-stage cervical cancer, the reported incidence of LVM ranges from 4 to 20%. The prognostic impact and the role of adjuvant treatment in patients with LVM is still unclear. Some non-prespecified analyses in prospective studies showed no impact on the oncologic outcomes compared to node-negative disease. However, the heterogeneity of the studies, the differences in the disease stage and the use of adjuvant treatment, and the concomitant pelvic lymphadenectomy (PLND) make any conclusion on this topic hard. Current guidelines suggest considering micrometastasis (MIC) as a node-positive disease, while isolated tumor cells (ITC) as a node-negative disease, with a low level of evidence. This review aims to highlight the unanswered questions about the definition, identification, prognostic and therapeutic role of LVM and underline the present and future challenges we are facing. We hope this review will guide further research, giving robust evidence on LVM and their impact on clinical practice.

In wireless sensor networks (WSNs) deployed in smart agriculture systems, accurately determining the locations of sensor nodes is crucial for efficient monitoring and control. However, in many cases, the exact positions of certain sensor nodes may be unknown. To address this challenge, this paper presents a novel method for localizing unknown sensor nodes in WSN-based smart agriculture systems using range measurements. The proposed method utilizes the Levenberg-Marquardt optimization algorithm to solve a nonlinear least squares problem and minimize the error in estimating the unknown node locations. By leveraging the known positions of a subset of sensor nodes and the inexact distance measurements between pairs of nodes, the localization problem is transformed into a nonlinear optimization problem. To validate the effectiveness of the approach, extensive simulations and experiments were conducted. The results demonstrate that the proposed method achieves accurate localization of the unknown sensor nodes, effectively enhancing the overall performance of the WSN-based smart agriculture system. Furthermore, the method demonstrates robustness against measurement noise and scalability for large-scale networks. The practical implications of this research extend beyond smart agriculture systems, as the proposed method can be applied to various WSN applications where precise node localization is essential.

Investigating the network response to node removal and the efficacy of the node removal strategies are related and fundamental questions in network science. Research studies proposed many node centralities based on the network structure ranking node to remove. The random walk (RW) on networks describes a stochastic process in which a walker travels among nodes. RW can be a model of transport, diffusion, and search on networks, and an essential tool for studying the importance of network nodes. In this manuscript, we propose four new measures of node centrality based on RW. Then, we compare the efficacy of the new RW node removal strategies to network dismantle with effective node removal strategies from the literature, such as betweenness and closeness node removal over synthetic and real-world networks. We evaluate the network dismantle along node removal using the size of the largest connected component (LCC). We find that, hence the betweenness nodes attack is the best strategy overall, the new node removal strategies based on RW show the highest efficacy in peculiar network topology. Specifically, RW strategies based on covering times emerge as the most effective strategy on a synthetic lattice network and two real-world road networks. Our results may be useful in selecting the best node attack strategies in a specific class of networks and in building more robust network structures.

The integration of transportation systems with land use through the Transit-Oriented Development (TOD) model can effectively promote sustainable development in cities. By increasing the intensive use of land within the supply capacity of urban public transportation, this model can comprehensively improve the efficiency of urban transportation and land use, while also addressing problems such as traffic congestion and environmental pollution. To implement this model, a "Node-Place-Function" (NPF) framework is proposed based on the coordination degree between rail transit and land use in Qingdao. A comprehensive evaluation system is constructed to quantitatively assess the TOD level and comprehensive development level of land use around rail transit stations from three dimensions. By applying the NSGA-II genetic algorithm, a land use allocation optimization model is developed to maximize rail transit station accessibility, land economic benefits, and environmental comfort. The research results guide other cities in China to implement TOD in land use planning and promote coordinated development between urban rail transit and land use.

Abstract Objective: To review the relevant literature on the use of atrioventricular node ablation and pacing in patients with heart failure and atrial fibrillation. Methods: A PubMed/MEDLINE and SCOPUS search was performed in order to assess the clinical outcomes of atrioventricular node ablation and pacemaker implantation, as well as the complications that may occur. Results: Several clinical trials, observational analyses and meta-analyses have shown that “pace and ablate” strategy not only improves symptoms but also can enhance cardiac performance in patients with heart failure and atrial fibrillation. Although this procedure is effective and safe, some complications may occur including worsening of heart failure, permanent fibrillation, arrhythmias and sudden death. Regarding the implantation of pacemaker, cardiac resynchronization therapy is shown to be the optimal choice compared to right ventricle apical pacing. His bundle pacing is a promising alternative to cardiac resynchronization therapy and has shown beneficial effects, while left bundle brunch pacing is an innovative modality. Conclusions: Atrioventricular node ablation and implantation of pacemaker is shown to have beneficial effects on clinical outcomes of patients with atrial fibrillation ± heart failure who do not respond or are intolerant to medical treatment. Cardiac resynchronization therapy is the treatment of choice and His bundle pacing seems to be an effective alternative way of pacing in these patients.

This paper analyzed the main diseases of the friction high-strength bolt (FHSB) joints in order to study the characteristics of the corrosion products of the joints in steel bridge and the influence factors. Seven representative FHSB connection pairs and eight steel plate samples were selected from the Dongying Shengli Yellow River Bridge, which has been in operation for 33 years. Then Scanning Electron Microscope (SEM), Energy Dispersive Spectroscopy (EDS) and X-Ray Diffraction (XRD) were used to analyze the microscopic morphology and chemical composition of the corroded surface of the samples. Finally, this paper discussed the relationship between the corroded sample surface and the environmental corrosion. The research results showed that construction quality problems, such as non-standard hole expansion, segmental slab joint dislocation and contact surface pollution, accelerated corrosion of the contact surface. With the increase of corrosion degree of joints, the contact surface of aluminum spraying layer gradually changed from a rugged and dense state to a smooth and porous powder state. With the corrosion of the steel plate substrate, the contact surface gradually changed from a fluffy stratified state to a surface bonding state. It was inferred that the friction coefficient of the FHSB connection node on the corroded aluminum sprayed contact surface first decreased and then increased. The corroded node samples detected not only a large number of S and Cl elements, but also the oxides of Mn, Si and other elements as well as FeS. It was speculated that node corrosion was also related to atmospheric (acid rain) and industrial dust and other corrosive environments. Almost all samples detected the component SiO2 of the Yellow River soil, as well as Mg, Ca, K, Na and other elements at the same time, which is basically consistent with the component elements of the Yellow River soil. Therefore, it was inferred that it was related to the inclusion of the Yellow River soil.

Targeted radioisotope-guided sentinel lymph node dissection (sLND) has shown high diagnostic accuracy in prostate cancer (PCa). To overcome the downsides of the radioactive tracers, magnetometer-guided sLND using superparamagnetic iron oxide nanoparticles (SPIONs) was successfully applied in PCa. This prospective study (SentiMag Pro II, DRKS00007671) determined the diagnostic accuracy of magnetometer-guided sLND in intermediate- and high-risk PCa. Fifty intermediate- or high-risk PCa patients (PSA≥10 ng/ml and/or Gleason score ≥7; median PSA 10.8 ng/ml, IQR 7.4–19.2 ng/ml) were enrolled. After intraprostatic SPIONs injection a day earlier, patients underwent magnetometer-guided sLND and eLND, followed by radical prostatectomy. SLNs were detected in vivo and in ex vivo samples. Diagnostic accuracy of sLND was assessed using eLND as the reference. SLNs were detected in all patients (detection rate 100%), with 447 SLNs (median 9, IQR 6–12) being identified and 966 LNs (median 18, IQR 15-23) being removed. Thirty-six percent (18/50) of patients had LN metastases (median 2, IQR 1–3). Magnetometer-guided sLND had 100% sensitivity, 97.0% specificity, 94.4% positive predictive value, 100% negative predictive value, 0.0% false negative rate, and 3.0% additional diagnostic value (LN metastases only in SLNs outside the eLND template). In vivo, one positive SLN/LN-positive patient was missed, resulting in a sensitivity of 94.4%. In conclusion, this new magnetic sentinel procedure has high accuracy for nodal staging in intermediate- and high-risk PCa. The reliability of intraoperative SLN detection using this magnetometer system requires verification in further multicentric studies.

Objective: The diagnostic efficacy of the apparent diffusion coefficient (ADC) on diffusion-weighted magnetic resonance imaging for lymph node metastasis in biliary tract cancer was investigated in the present study. Methods: Surgically resected 112 lymph nodes from 35 biliary tract cancer (BTC) patients were examined in this study. Values of the ADC mean and minimum of lymph nodes, as well as short-axis and long-axis diameters of lymph nodes were assessed by computed tomography (CT). The relationship between these parameters and the presence of histological lymph node metastasis was evaluated. Results: Histological lymph node metastasis was detected in 31 (27.7%) out of 112 lymph nodes. The short-axis diameter in metastatic lymph nodes was significantly larger than that in non-metastatic lymph nodes (P = 0.002), but the long-axis diameter was not significantly different between metastatic and non-metastatic lymph nodes. Both ADC mean and minimum values in metastatic lymph nodes were significantly lower than those in non-metastatic lymph nodes (P < 0.001, respectively). However, the ADC minimum value showed the highest accuracy for the diagnosis of histological lymph node metastasis, with an area under the curve of 0.877, sensitivity of 87.1%, specificity of 82.7%, and accuracy of 83.9%. Conclusions: The ADC minimum value on diffusion-weighted magnetic resonance imaging was very useful for the diagnosis of lymph node metastasis in BTC.

Additive manufacturing enables the realization of complex component designs that can not be achieved with conventional processes, such as the integration of lattice structures for weight reduction. To include lattice structures in component designs, an automated algorithm compatible to conventional CAD is required that is able to handle various lattice topologies as well as variable local shape parameters such as strut radii. Furthermore, smooth node transitions are desired since previous studies indicate advantages regarding reduced stress concentrations and the structural response to loadings such as fatigue performance. The surface patch-based algorithm developed in this work is able to solidify given lattice frames to smooth lattice structures without manual construction steps. The algorithm requires only a few seconds of sketching time for each node and favours paralellization. Automated special case workarounds as well as fallback mechanisms are considered for non-standard inputs. It is demonstrated on irregular lattice topologies and applied for the construction of a lattice infill of an aircraft component that was additively manufactured.

Background: Primary lymphedema is a chronic condition caused by a developmental abnormality of the lymphatic system and its malfunction. Different surgical options such as physiologic and excisional procedures have been proposed. The aim of this study was to present a complete algorithm for the treatment of primary lower extremity lymphedema: Primary LYmphedema Multidisciplinary Approach (P-LYMA). Methods: Nineteen patients were treated according to the P-LYMA. Patients underwent pre- and post-operative complex decongestive therapy (CDT). Different physiologic and excisional procedures were performed independently or in combination. The primary outcome was the circumferential reduction rate (CRR). The Lymphedema Quality of Life Score (LeQOLiS), reduction in the number of episodes of cellulitis, length of stay, and complications were recorded. Results: The mean CRR calculated was 72.8±19.6% after twelve months. The episodes of cellulitis per year decreased from a mean of 1.9±0.8 preoperative to 0.4± 0.6 throughtout follow-up. Two patients experienced minor complications. Mean hospitalization time was 5 days. Patients’ quality of life (LeQOLi score) drastically improved from 70.4±11.6 to 24.2 ±13.9 at twelve months post-op. Conclusion: The P-LYMA algorithm allows to maximize surgical results and improve the quality-of life in patients who suffer from primary lymphedema. CDT is mandatory to optimize results.

LoRa and its upper layers definition LoRaWAN is one of the most promising LPWAN technologies for implementing the Internet of Things (IoT). Although being a popular technology, several works in the literature have revealed various weaknesses regarding the security of LoRaWAN v1.0 (the official 1st draft). By using all these recommendations from the academia and industry, the LoRa-Alliance has worked on the v1.0 to develop an enhanced version and provide more secure and trustable architecture. The result of these efforts ended-up with LoRaWAN v1.1, which was released on Oct 11, 2017. This manuscript aims at demystifying the security aspects and provide a comprehensive Security Risk Analysis related to latest version of LoRaWAN. Besides, it provides several remedies to the recognized vulnerabilities. To the best of authors’ knowledge, this work is one of its first kind by providing a detailed security analysis related to latest version of LoRaWAN. According to our analysis, end-device physical capture, rogue gateway and replay attacks are found to be threating for safety operation of the network. Eventually, v1.1 of LoRaWAN is found to be less vulnerable to attacks compared to v1.0, yet possesses several security implications that need to be addressed and fixed for the upcoming releases.

This paper intoduces a non-standard research technique to clarify how complex phenomena, such as those that are abundantly present in human physiology, can be faithfully described using fractal dynamical models with and without stochastic forces. This method for conducting research involves tracing the his- torical evolution of understaning an empirical medical process faciltated by the fractal calculus perspective. Herein we trace the analysis of the time series for heart rate varability (HRV) developed for diagnosing the cardiovascular health of a patient. This is done herein by introducing four (one empirical which en- tails three theoretical fractal models) distinct but related fractal models each one introduced to solve a particular problem arising from a fundamental defect in the previous model but in generalizing a model at one stage to resolve the problem associated with the defect another is invariably introduced by the re- placement model. It is through the utilization of the fractional calculus that the necessity for rethinking how to systematically incorporate additonal layers of complexity are revealed ultimately resulting in a 'complete' description of its empirical dynamics in fractal terms.

Lymphadenopathy is a common manifestation of both reactive and malignant diseases, and fine needle aspiration (FNA) is an effective and inexpensive screening method. It can prevent unnecessary invasive surgery and excisional biopsy, especially in benign cases. Unfortunately, the lack of universally accepted terminology for reporting results has hindered its widespread support. The Sydney system proposal for lymph node cytopathology categorization and reporting introduced five diagnostic categories to address the lack of universally accepted terminology for reporting results in lymphadenopathy. Our study analyzed 188 lymph node FNC samples from King Abdulaziz University Hospital, Saudi Arabia, examining clinical follow-up data, pathology records, patient information, and final diagnosis from January 2019 to December 2022. Most specimens were from axillary LNs, with 99.5% tissue correlation. The Sydney System Category classification identified 56.9% of cases as malignant, while 26.1% were benign. The final surgical specimen diagnosis revealed a higher percentage of malignant diagnoses, with the highest ROM in malignant/Category V. In conclusion, our study demonstrates that LN-FNAC offers high diagnostic accuracy for LN aspirates, with the Sydney approach potentially aiding risk stratification and achieving consistency in cytologic diagnosis, but further multi-centric research is needed.

Elevated blood glucose levels, known as glycemia, play a significant role in sudden cardiac arrest, often resulting in sudden cardiac death, particularly among those with diabetes. Understanding this link has been a challenge for healthcare professionals, leading many research groups to investigate the relationship between blood glucose levels and cardiac electrical activity. Our hypothesis suggests that glucose-sensing mechanisms in cardiac tissue could clarify this connection. To explore this, we adapted a single-compartment, in-silico model of the human atrial node's action potential. We incorporated glucose-sensing mechanisms with voltage-gated sodium ion channels using ordinary differential equations. Parameters for the model were based on existing experimental studies to mimic the impact of glucose levels on atrial node action potential firing. Simulations using voltage clamp and current clamp techniques showed that elevated glucose levels decreased sodium ion channel currents, leading to a reduction in the sinoatrial node action potential frequency. In summary, our model provides a cellular-level understanding of how high glucose levels can lead to bradycardia and sudden cardiac death.

Background: The accurate diagnosis of regional lymph node metastasis is critical for guiding treatment decisions in head and neck cancer patients. Despite advances in imaging techniques, detecting nodal metastasis remains challenging, leading to potential undertreatment or overtreatment. This systematic review aims to identify molecular markers associated with occult metastasis in Head and Neck Squamous Cell Carcinoma (HNSCC) patients. Methods: The present research was conducted following PRISMA guidelines. Prospero registration nr. CRD42024522985. Results: Through a comprehensive literature search and review process, 634 articles were considered, 103 of them were finally included. Conclusions: Several promising markers were identified, including miR-205, DSG3, pan-CK AE1/AE3, HPV-16, Activin-A, Cyclin D1, and NPL that demonstrated effectiveness across multiple studies. Future research should focus on exploring combination scoring systems to improve diagnostic precision and optimize treatment selection in HNSCC patients.

Introduction: Flow-cytometry (FC) is a powerful tool that can assist in lymphoma diagnosis in lymph node (LN) specimens. Although lymphoma diagnosis and classification are mainly based on tumor cell characteristics, surrounding cells are less employed in this process.Methods: We retrospectively investigated alterations in the ploidy status, proliferative cell fraction (PF) and the percentages of surrounding immune cells in 62 consecutive LN specimens with B-Cell Non-Hodgkin Lymphoma (B-NHL) that were submitted for FC evaluation between 2019-2022.Results: Compared with indolent B-NHLs, aggressive B-NHLs show increased DNA aneuploidy and PF, increased monocytes, immature-granulocytes, mature granulocytes, CD8⁺ T-cells, Double-Negative-T-cells and Double-Positive-T-cells, and decreased total CD45⁺ cells, total lymphocytes, CD4⁺ T-cells and CD4/CD8 ratio. Receiver operating characteristic analysis determined PF > 6.8% and immature-granulocytes > 0.9% as optimal cutoffs with highest specificity and sensitivity in differentiating aggressive and indolent B-NHLs.Conclusions: These findings further strength the diagnostic value of DNA content analysis by FC and suggest the utilization of tumor surrounding immune cells in NHL diagnosis and classification.

For equivalent modeling of mixed wind farms (WFs), existing clustering indicators cannot consider the complex coupling characteristics between different types of wind turbines (WTs). This paper proposes a refined equivalent modeling method for mixed WFs based on artificial intelligence technology. Firstly, the electromechanical transient performance of mixed WFs is analyzed. The WT type, wind speed and direction, and voltage dip are considered the main factors affecting the external dynamic response of mixed WFs. Secondly, the equivalent node model is established, including the selection of independent and dependent variables. Then, the multiple artificial neural networks (ANNs) are trained one by one based on sample data, to explore the nonlinear relationship between the dependent variables and the independent variables. Finally, the electromechanical transient simulations of the power systems with a mixed WF is carried out by using the MATLAB platform. Simulation results show that the proposed model can reflect the external characteristics of the mixed WF in different wind scenarios and voltage dips.

Recently, the Internet of Things (IoT) has attracted wide attention from many fields, especially healthcare, because of its large capacities of information perception and collection. In this paper, we present an IoT-based home telehealth system for providing smart healthcare management for individuals, especially older people. Each client node of the system is mainly composed of an electronic water meter that records the user's daily water usage and an unobtrusive sleep sensor that monitors the user's physiological parameters during sleep such as heart rate (HR), respiratory rate (RR), body movement (BM), and states on the bed or outside the bed. The sleep sensor senses the biosignals and states by a sensor matrix composed of 12 piezoelectric sensors. The collected data can be transmitted to a remote centralized cloud service by a wireless home gateway for analyzing the living pattern and rhythm of users. Furthermore, the periodic feedback of results can be provided to users themselves as well as their family and health advisers. In the present study, data collections from a total of 18 older subjects were made for one year to evaluate the effectiveness of the proposed system. By analyzing living pattern and rhythm, preliminary results indicate the effectiveness of the telehealth system and suggest the potential of the system regarding improvement of quality of life (QoL) of older people and promotion of their health.

The objective of this study was to evaluate the efficacy of one-step nucleic acid amplification (OSNA) for the detection of sentinel lymph node (SLN) metastasis compared to standard pathological ultrastaging in patients with early-stage endometrial cancer (EC). A total of 526 SLNs from 191 patients with EC were included in the study. 379 SLNs (147 patients) were evaluated by both methods, OSNA and standard pathological ultrastaging. The central 1-mm portion of each lymph node was subjected to semi-serial sectioning at 200-μm intervals and examined by hematoxylin-eosin and immunohistochemistry with CK19; the remaining tissue was analysed by OSNA for CK19 mRNA. The OSNA assay detected metastases in 19.7% of patients (14.9% micrometastasis and 4.8% macrometastasis), whereas pathological ultrastaging detected metastasis in 8.8% of patients (3.4% micrometastasis and 5.4% macrometastasis). Using the established cut-off value for detecting SLN metastasis by OSNA in EC (250 copies/μl), the sensitivity of the OSNA assay was 92%; specificity was 82%; diagnostic accuracy was 83%, and the negative predictive value was 99%. Discordant results between both methods were recorded in 20 patients (13.6%). OSNA resulted in an upstaging in 12 patients (8.2%). OSNA could aid in the identification of patients requiring adjuvant treatment at the time of diagnosis.

In zebrafish inner ear, hair cell orientation in anterior and posterior maculae of the embryonic otic vesicle is different (about 30–40 degrees): this is rather unusual in planar polarity mechanism of action, instead suggests that kinocilia may be rotationally polarized. In mice node, the innermost monociliated cells generate a left-ward fluid flow sensed by the immotile primary cilia of Left peri-nodal cells: the Nodal signaling pathway is then expressed asymmetrically, in the Left lateral plate mesoderm, breaking symmetry in visceral organs (situs solitus); however, Right peri-nodal cells also, if artificially excited by a right-ward flow, break symmetry and activate the Nodal cascade, though inverting visceral organ asymmetry (situs inversus); surprisingly, peri-nodal cells prove to be adept at distinguishing flow directionality. Recently, in the Kupffer vesicle (the zebrafish laterality organ), chiral primary cilia orientation has been described: primary cilia, in the left and right side, are symmetrically oriented, showing a mirror average divergence of about 15–20 degrees from the midline. This finding, taken together with the mirror behavior of mouse perinodal cells and zebrafish hair cells, champions the idea of primary cilia enantiomerism.

Our study aimed to identify predictors for the effectiveness of tumor regression in lung cancer patients undergoing neoadjuvant treatment and cancer resections. Patients admitted between 2016 and 2022 were included in the study. Based on the histology of the tumor, patients were categorized into lung adenocarcinoma group (LUAD), and squamous cell carcinoma group (SQCA). Ninety-five patients with non-small cell lung cancer were included in the study. Fifty-eight (61.1%) and 37 (38.9%) patients were included in the LUAD and SQCA groups, respectively. Nine (9.5%), 56 (58.9%), and 30 (31.6%) patients were categorized with a tumor regression score of I, II, and III respectively. In multivariable analyses, histology of the primary tumor (SQCA), lymph node size in the preoperative CT scan (>1.7cm) and absolute tumor size reduction after neoadjuvant treatment (>2.6 cm) independently predict effectiveness of tumor regression (OR, [95% Confidence interval, p-value] of 6.88 [2.40-19.77, p70 years, extended resection >one lobe, and tumor recurrence or metastasis were identified as significant independent predictors of reduced overall survival. Assessment of tumor size before and after neoadjuvant treatment might help to identify high-risk patients with decreased survival and to improve patient management and care.

The role of postmastectomy radiotherapy and regional nodal irradiation after radical mastectomy is defined in high-risk patients with locally advanced tumors, positive margins, and unfavorable biology. The benefit of postmastectomy radiotherapy in intermediate-risk patients (T3N0 tumors) remains a matter of controversy. It has been demonstrated that radiotherapy after breast-conserving surgery lowers the locoregional recurrence rate compared with surgery alone and improves the overall survival rate. In patients with 4 or more positive lymph nodes or extracapsular extension regional lymph node irradiation is indicated regardless of the surgery type (breast-conserving surgery or mastectomy). Despite the consensus that patients with more than 3 positive lymph nodes should be treated with radiotherapy, there is a controversy regarding the recommendations for patients with one to three involved lymph nodes. In patients with N0 disease with negative findings on axillary surgery, there is a trend to administer regional lymph node irradiation in patients with a high risk of recurrence. In patients treated with neoadjuvant systemic therapy and mastectomy, adjuvant radiotherapy should be administered in case of clinical stage III and/or ≥ypN1. In patients treated with neoadjuvant systemic therapy and breast-conserving surgery, postoperative radiotherapy is indicated irrespective of pathological response.

Over the past several years there has been much debate with regards to the prognostic and clinical significance of pancreatic ductal adenocarcinoma (PDAC) with lymph nodes metastasis. The PDAC gene-expression knowledge and the biologic alterations underlying the lymph node involvement convey a clinical implication in dealing with the theranostic window.To this end, we provide an original bioinformatic dissection of the gene-expression differences of PDAC according to the nodal involvement from a large public available dataset. Comprehensive transcriptomic analysis from 143 RNA-seq patient’s derived samples indicated that WNT increased activation and a peculiar immune-microenvironment identify subjects with nodal involvement.In frame of this thinking, we validated the WNT pathway role in increasing the likelihood of lymphatic dissemination in vitro. Moreover, we demonstrated for the first time in a PDAC model the potential therapeutic window that XAV-939, a specific WNT pathway inhibitor, has in re-educating a tumour permissive immune system. Finally, we outline the potential implication on bystander molecular drivers exerted by WNT molecular inhibition, providing a picture of the proteomic oncogenic landscape changes elicited by XAV-939 on PDAC cells and their clinical implication. Our findings hold the promise to identify novel immune-based therapeutic strategies targeting WNT to enhance PDAC cytotoxicity and restore anti-PDAC immunity in nodes-positive disease.

Those using the mouse for the purposes of electrophysiological research presume that the morphology of the conduction axis is comparable with the human arrangement. As yet, however, to the best of our knowledge, no direct comparison has been made between the species. By comparing our extensive histological findings in the human heart with comparable serially-sectioned datasets prepared from adult murine hearts, we aimed to provide this information. When comparing the gross anatomy, we used three-dimensional datasets of neonatal mice hearts prepared using episcopic microscopy. The overall cardiac architecture is comparable, although the mouse has a persistent left superior caval vein draining via the coronary sinus. An inferior pyramidal space, and an infero-septal recess, are both present in the murine heart, although not as well developed as in the human heart. The overall arrangement of the conduction axis is similarly comparable, albeit with subtle differences reflecting the incomplete wedging of the subaortic outflow tract in the murine heart. Most significantly, the findings in both species reveal the presence of extensive superior septal pathways, which perhaps explain the finding of base-to-apex activation of the ventricular mass known to occur in the murine heart.

Sublobar resection is a standard surgical procedure for small-sized non-small cell lung cancer (NSCLC). However, clinical role of adjuvant chemotherapy for small-sized NSCLC with pathological lymph node (LN) metastasis after sublobar resection is unknown. National Cancer Database was queried for NSCLC patients between 2004 and 2018. Eligibility included sublobar resection with pathological LN metastasis, R0 resection, Charlson comorbidity score = 0, clinical stage T1a-b, and tumor size ≤ 20 mm. Kaplan-Meier method with log-rank test and multivariable Cox proportional hazards analyses were used for assessing survival. Samples were evaluated before and after propensity score matching (PSM) with respect to age, sex, histologic type, and pathological LN status. Of 810 patients who met the eligibility criteria, 567 (70.0%) underwent adjuvant chemotherapy. After PSM, patients with adjuvant chemotherapy had a significantly longer survival than those without (median survival: 64.3 vs. 34.0 months, hazard ratio for death: 0.61, p < 0.0001). Multivariate analyses after PSM showed that younger age (p = 0.0206), female (p = 0.0005), and adjuvant chemotherapy (p < 0.0001) were independent prognostic factors for longer survival. Adjuvant chemotherapy has a prognostic impact in patients with small-sized NSCLC and pathological lymph node metastasis who undergo sublobar resection.

(1) Background: Sentinel lymph node (SLN) mapping represents an accurate and feasible technique for the surgical staging of endometrial and cervical cancer. This is commonly performed by conventional laparoscopy or robotic-assisted laparoscopy, but in recent years, a new retroperitoneal transvaginal natural orifice transluminal endoscopic surgery (vNOTES) approach has been described. This technique provides easy visualization of lymphatic afferent vessels and pelvic lymph nodes, early SLNs assessment, and a coherent mapping methodology following the lymphatic flow from caudal to cranial. However, only a few publications have reported it. Following the IDEAL (Idea Development Exploration Assessment Long-term follow-up) framework, research concerning this technique is at stage 2a, with only small case series as evidence of its feasibility. Its standardized description appears necessary to provide the surgical homogeneity required to move further. (2) Methods: description of a standardized approach for retroperitoneal pelvic SLN mapping by vNOTES. (3) Results: we described a 10-step approach to successfully perform retroperitoneal vNOTES SLN mapping, including pre-, intra-, and postoperative management. (4) Conclusions: This IDEAL stage 2a study could help other surgeons approach this new technique, and it proposes a common methodology necessary to evolve through future IDEAL stage 2b (multi-center studies) and stage 3 (randomized controlled trials) studies.

Aims: The aim of our work was to demonstrate the possibility, in carefully selected cases, to perform operations under local anesthesia with a benefit to the patient and savings for national health care. Method: 123 patients with breast cancer underwent quadrantectomy or wide tumorectomy and sentinel lymph node removal under local anesthesia in out-patient surgery. Results: Of the 123 patients, 9 had micro-metastases and 4 had macro-metastases and only three of this had metastasis in other lymph nodes. Conclusions: Our results demonstrate that it is possible in selected cases to operate on breast cancer in out-patient surgery, with an advantage for the patient and savings for national health care.

Background: Tuberculosis (TB) manifests itself primarily in the lungs as pulmonary disease (PTB) and sometimes disseminates to other organs to cause extra-pulmonary TB, such as lymph node TB (LNTB). This study aimed to investigate the role of host genetic polymorphism in immunity related genes to find a genetic basis for such differences. Methods: Sixty-three, Single nucleotide polymorphisms (SNPs) in twenty-three, TB-immunity related genes including eleven innate immunity (SLCA11, VDR, TLR2, TLR4, TLR8, IRGM, P2RX7, LTA4H, SP110, DCSIGN and NOS2A) and twelve cytokine (TNFA, IFNG, IL2, Il12, IL18, IL1B, IL10, IL6, IL4, IL1RA, IL8 and TNFB) genes were investigated to find genetic associations in both PTB and LNTB as compared to healthy community controls. The serum cytokine levels were correlated for association with the genotypes. Results: PTB and LNTB showed differential genetic associations. The genetic variants in the cytokine genes (IFNG, IL12, IL4, TNFB and IL1RA and TLR2,4 associated with PTB susceptibility and cytokine levels but not LNTB (p &lt; 0.05). Similarly, genetic variants in LTA4H, P2RX7, DCSIGN and SP110 showed susceptibility to LNTB and not PTB. Pathway analysis showed abundance of cytokine related variants for PTB and apoptosis related variants for LNTB. Conclusions: PTB and LNTB outcomes of TB infection have a genetic component and should be considered for any future susceptibility and functional studies.

Through field research, the village is investigated and analyzed, and the street scale, spatial structure, functional characteristic streets, internal commercial distribution forms, and functions of Cuiwei Historic District are studied. analysis. On this basis, based on the convex space method and the line-of-sight analysis method in the space syntax theory, with the help of Depthmap software, the complex street node, that is, the space of the two nodes A and B, is established through the establishment of a visual field model. It includes the analysis of the integration degree of the horizon, the concentration of the horizon, the connection value of the horizon and the spatial characteristics of the historical area, as well as the traditional buildings that are the most representative of the village, namely the Three Kings Temple and the Webster's Mansion. In this paper, the methodology using Insect Intelligent Building (I^2B) technology for establishing energy consumption monitoring system of public buildings is prevailed. The computing process node and distributed algorithm are utilized to implement the energy consumption collection and data transmission and data pre-processing. Taking a commercial building as a case study, CPNs are applied to set up the building energy consumption monitoring system, with the Spanning Tree Algorithm for generating network topology，and BPNN method for solving abnormal data and recovering missing data. The research results demonstrate the proposed method can effectively improve the performance of plug-and-play and self-identified and self-configuration of energy consumption monitoring system.

In this study we discuss critical issues in modelling the structure and function of lymph nodes (LNs), with emphasis on how LN physiology is related to its multi-scale structural organization. In addition to macroscopic domains such as B-cell follicles and the T cell zone, there are vascular networks which play a key role in the delivery of information to the inner parts of the LN, i.e., the conduit and blood microvascular networks. We propose object-oriented computational algorithms to model the 3D geometry of the fibroblastic reticular cell (FRC) network and the microvasculature. Assuming that a conduit cylinder is densely packed with collagen fibers, the computational flow study predicted that the diffusion should be a dominating process in mass transport than convective flow. The geometry models are used to analyze the lymph flow properties through the conduit network in unperturbed- and damaged states of the LN. The analysis predicts that elimination of up to 60–90 % of edges is required to stop the lymph flux. This result suggests a high degree of functional robustness of the network.

The delineation of the clinical target volumes (CTVs) for radiation therapy is time-consuming, requires intensive training and shows high inter-observer variability. Supervised deep learning methods depend heavily on consistent training data, thus state-of-the-art research focuses on making CTV labels more homogeneous and strictly bounding them to current standards. International consensus expert guidelines standardize CTV delineation by conditioning the extension of the clinical target volume on surrounding anatomical structures. Training strategies that directly following the construction rules given in the expert guidelines or the possibility to quantify the conformance of manually drawn contours with the guidelines are still missing. 71 anatomical structures that are relevant for CTV delineation in head and neck cancer patients according to the expert guidelines were segmented on 104 CT scans to assess the possibility to automate their segmentation by state-of-the-art deep learning methods. nnU-Net models were trained to automatically segment those 71 structures on planning CT scans. We report the DICE, HD and sDICE for 71 + 5 anatomical structures for most of which no previous segmentation accuracies have been reported. For those structures, for that prediction values have been reported, our segmentation accuracy matched or exceeded the reported values. The predictions from our models were always better than those predicted by the TotalSegmentator. The sDICE with 2mm margin was larger than 80% for almost all structures. Individual structures with decreased segmentation accuracy are analyzed and discussed with respect to their impact on the CTV delineation following the expert guidelines. No deviation is expected to affect the rule-based automation of the CTV delineation.

Some concepts/hypotheses have been proposed to explain the general rules behind the complexity of tumorigenesis. Characterization of the property of cancer cells and neural stem cells indicates that neural stemness underlies tumorigenicity and pluripotency, leading to the proposal that tumorigenesis represents a process of progressive loss of original cell identity and gain of neural stemness. This reminds of a most fundamental process required for the development of the nervous system and body axis during embryogenesis, i.e., embryonic neural induction. The principle of neural induction is that, in response to extracellular signals that are secreted by the Spemann-Mangold organizer in amphibians or the node in mammals and inhibit epidermal fate in ectoderm, the ectodermal cells assume the neural default fate and turn into neuroectodermal cells. These cells further differentiate into the nervous system and also some non-neural cells via interaction with adjacent tissues. Failure in neural induction leads to failure of embryogenesis, and ectopic neural induction due to ectopic organizer or node activity or activation of embryonic neural genes causes a formation of secondary body axis or conjoined twins. A similar principle underlies tumorigenesis. Increasing evidence has demonstrated that the core property of cancer cells is neural stemness. Therefore, cancer cells are cells with the loss of original cell identity and gain of neural stemness, and consequently tumorigenicity and pluripotency, due to various intra-/extracellular insults in postnatal animals. Unlike that pluripotent cells (embryonic pluripotent cells, neural stem cells and cancer cells) can differentiate and integrate into embryonic development, cancer cells are capable of self-renewal and differentiation, but cannot integrate into normal tissues in a 2 postnatal animal, ultimately leading to tumor formation. Neural induction and the unique property of neural stemness provide an inclusive explanation for embryogenesis, conjoined twin formation and tumorigenesis. Based on these findings, I discuss about some confusion in cancer research, e.g., epithelial-mesenchymal transition, and propose to distinguish the causality and associations, and the causal and supporting factors involved in tumorigenesis, and suggest revisiting the focus of cancer research. Integration of evidence from developmental and cancer biology indicates that neural stemness determines tumorigenicity and pluripotency, and neural induction drives embryogenesis in gastrulating embryos but a similar process drives tumorigenesis in a postnatal animal.

The ability to predict the skin sensitization potential of small organic molecules is of high importance to the development and safe application of cosmetics, drugs and pesticides. One of the most widely accepted methods for predicting this hazard is the rodent local lymph node assay (LLNA). The goal of this work was to develop in silico models for the prediction of the skin sensitization potential of small molecules that go beyond the state of the art, with larger LLNA data sets and, most importantly, a robust and intuitive definition of the applicability domain, paired with additional indicators of the reliability of predictions. We explored a large variety of molecular descriptors and fingerprints in combination with random forest and support vector machine classifiers. The most suitable models were tested on holdout data, on which they yielded competitive performance (Matthews correlation coefficients up to 0.52; accuracies up to 0.76; areas under the receiver operating characteristic curves up to 0.83). The most favorable models are available via a public web service at https://nerdd.zbh.uni-hamburg.de/skinDoctor/ that, in addition to predictions, provides assessments of the applicability domain and indicators of the reliability of the individual predictions.

In this study we summarized our work on various aspects of colorectal adenocarcinomas, 1 focusing on tumor distribution (left- sided, right- sided, and rectal cancers), histologycal type, patient 2 age, gender and several factors affecting prognosis. These predictive and prognostic factors include 3 peritumoral budding, lymphovascular ant perineural invasion, lymph node metastasis, mismatch 4 repair status, and corellations with some frequently known mutations in colorectal adenocarcinomas, 5 detected using Next- Generation Sequencing (NGS) platform. This comprehensive approach allowed 6 us to explore how these factors interplay and influence the clinical outcome of patients with colorectal 7 adenocarcinoma. Additionaly, our findings contribute to the understanding of how genetic mutations 8 may correlate with histological features and prognostic markers, potentially guiding personalized 9 treatment strategies in the future. In our study, we do not claim to have exhausted the research on 10 the topic, and we fully recognized the need for more extensive and in-depth studies in this area of 11 tumor pathology.

Ultra-wideband wavelength division multiplexed networks enable operators to use more effectively the bandwidth offered by a single fiber pair and thus make significant savings, both in operational and capital expenditures. The main objective of this study is to minimize optical node resources, such as transponders, multiplexers and wavelength selective switches, needed to provide and maintain high quality of network services, in ultra-wideband wavelength division multiplexed networks, at low cost. A model based on integer programming is proposed, which includes a detailed description of optical network nodal resources. The developed optimization tools are used to study the ultra-wideband wavelength division multiplexed network performance when compared with the traditional C-band wavelength division multiplexed networks. The analysis is carried out for realistic networks of different dimensions and traffic demand sets.

This work introduces the mathematical framework of the novel “First-Order Features Adjoint Sensitivity Analysis Methodology for Neural Ordinary Differential Equations” (1st-FASAM-NODE). The 1st-FASAM-NODE methodology produces and computes most efficiently the exact expressions of all of the first-order sensitivities of NODE-decoder responses with respect to the parameters underlying the NODE’s decoder, hidden layers, and encoder, after having optimized the NODE-net to represent the physical system under consideration. Building on the 1st-FASAM-NODE, this work subsequently introduces the mathematical framework of the novel “Second-Order Features Adjoint Sensitivity Analysis Methodology for Neural Ordinary Differential Equations (2nd-FASAM-NODE).” The 2nd-FASAM-NODE methodology yields and computes most efficiently the exact expressions of the second-order sensitivities of NODE decoder-responses with respect to the NODE parameters. Since the physical system modeled by the NODE-net necessarily comprises imprecisely known parameters that stem from measurements and/or computations subject to uncertainties, the availability of the first-and second-order sensitivities of decoder responses to the parameters underlying the NODE-net are essential for performing sensitivity analysis and quantifying the uncertainties induced in the NODE-decoder responses by uncertainties in the underlying uncertain NODE-parameters.

This review article is focused on the analysis of the state of the art of sensors for guided 9 ultrasonic waves for the detection and localization of impacts, therefore of interest for the structural 10 health monitoring (SHM). The recent developments in sensor technologies are then reported and 11 discussed through the many references in recent scientific literature. The physical phenomena re-12 lated to impact event and the main physical quantities are then introduced to discuss their im-13 portance in the development of the hardware and software components for SHM systems. An im-14 portant aspect of the article is the description of the different ultrasonic sensor technologies cur-15 rently present in the literature and what advantages and disadvantages they could bring, in relation 16 to the various phenomena investigated. In this context, the analysis of the front-end electronics is 17 deepened, the type of data transmission both in terms of wired and wireless technology and in terms 18 of online and offline signal processing. The integration aspects of sensors for the creation of net-19 works with autonomous nodes with the possibility of powering through energy harvesting devices 20 and the embedded processing capacity is also studied. Finally, the emerging sector of processing 21 techniques using deep learning and artificial intelligence concludes the review by indicating the 22 potential for the detection and autonomous characterization of the impacts.
