Background/Objectives: Chronic postoperative pain remains a significant barrier in postoperative management, particularly in patients who have undergone shoulder surgery. We evaluated the predictors of chronic pain following shoulder arthroscopy. Methods: This was a retrospective, observational study of patients who underwent shoulder arthroscopy at a university hospital between 2012 and 2017. Telephone contact was established with 178 patients who met the study criteria. Demographic data, comorbidities, preoperative interscalene block application, intraoperative opioid use, and records of postoperative patient-controlled analgesia were obtained by reviewing patient files. Upon contact, each patient's psychological state and pain level were assessed. Results: Interscalene block was administered to 33 (18.5%) of the patients, while patient-controlled analgesia was provided to 97 (54.5%). The analgesic method of 48 patients’ were not achieved from the records. Chronic pain was identified in 92 patients (51.7%). Body weight, comorbidities, and the combined use of opioids and non-opioids were found to be significant risk factors (p-values of 0.024, 0.016, and 0.010, respectively) for chronic pain. Multivariate Logistic Regression analysis revealed that the risk of chronic pain in patients with comorbidities and combined opioid-non-opioid use was 9.27 times higher than in those without comorbidities. In the presence of comorbidities, the risk of chronic pain was found to be 7.18 times higher in patients who did not use a combination of opioids and non-opioids. Conclusion: This study indicates that higher body weight, the presence of comorbidities, and the use of both opioids and non-opioids are significant predictors of increased chronic postoperative pain following shoulder arthroscopy.

This paper reviews the appearance of joint lesions during arthroscopic surgery of inter-est to the arthroscopic surgeon and their interpretation within the continuum of joint disease. It also points out the relevance of surgical findings as well as giving directions to the surgeon as to how to approach an appropriate decision-making process during an arthroscopic procedure and what therapeutic options are optimal.

Current treatments for medial meniscus lesions in association with CCL ruptures [1–5] have shown disappointing long-term outcomes [6]. Meniscal suturing may improve the outcome, but their clinical application remains limited [7–9]. This study aims to assess the practicability of a minimally invasive DNT to facilitate meniscal suturing in dogs. Ten stifles of eight client-owned dogs with arthroscopically confirmed CCL disease and medial meniscal tears in the abaxial third of the meniscus were included. The described suture technique was applied under joint distraction using the Titan joint distractor.[8] Surgical stabilization of all stifles was then accomplished through a X-Porous TTA procedure [10]. The DNT allowed for the precise placement of meniscal sutures. Minor intraoperative complications included reduced arthroscopic visibility (n=2) and suture breakage during its passage through the meniscus (n=3). No complications related to the meniscal sutures were noted throughout the six-month follow-up. The described DNT proved to be a viable and effective method for suturing lesions of the caudal horn of the medial meniscus, provided there is adequate visualization during the procedure. Appropriate stifle stabilization postoperatively is mandatory for protecting the suture and avoiding concomitant meniscal lesions due to joint instability.

Temporomandibular disorders (TMD) affect a high percentage of children and adults worldwide. Surgery may be indicated in severe or recalcitrant case. Several recent advancements in the TMD and temporomanidbular joint (TMJ) surgery have elevated understanding and ability to treat affected patients. We discuss recent advances in TMD epidemiology, juvenile idiopathic arthritis (JIA) of the TMJ, and surgical techniques and technologies. Technical advancements have been identified in TMJ arthroscopy, treatment of TMJ subluxation and dislocation, and extended prosthetic total TMJ reconstruction (eTMJR). Overall, this review provides valuable insights into significant recent advancements of TMJ disorders and their surgical management.

Arthroscopic capsular release (ACR) is used for persistent shoulder stiffness after an index surgery. No cases of post-ACR humeral head osteonecrosis have been reported to date. A 56-year-old male patient underwent open reduction and internal fixation using a hook plate for acromioclavicular joint dislocation. Despite hardware removal, the patient presented with unresolved shoulder pain and range-of-motion (ROM) limitation. He had a history of hypertension, chronic hepatitis B infection, and alcohol consumption. Preoperative ROM was forward flexion 90°and internal rotation sacral level. Preoperative functional status was visual analog scale (VAS) score 4, American Shoulder and Elbow Surgeons (ASES) score 51, and Constant–Murley (CMS) score 48, and normal radiography and magnetic resonance imaging. Standard ACR was performed with 360° release of the joint capsule via electrocautery ablation. Six-months post-ACR, ROM (forward flexion: 135°, abduction: 135°, external rotation: 70°, internal rotation: T10 vertebra) and functional outcomes (VAS 2; ASES79; CMS 75) were significantly improved, without interval change in radiographic assessment. However, 21 months later, he had recurrent shoulder pain and decreased ROM. Radiography revealed humeral head osteonecrosis. Patients with intrinsic or extrinsic risk factors related to humeral head circulation disturbance should be monitored for humeral head osteonecrosis post-ACR.

Background: The objective of the present study was to evaluate the formation of biofilm in bone patellar tendon bone grafts (BPTB grafts) and to compare it to the formation of biofilm formation in quadrupled hamstring anterior cruciate ligament grafts (4xHt graft). Methods: A descriptive in vitro study was conducted. One 4xHt graft and one BPTB graft were prepared. They were then contaminated with a strain of S. epidermidis. Later, a quantitative analysis was carried out by means of microcalorimetry and sonication with plating. Additionally, a qualitative analysis was carried out by means of electron microscopy. Results: No significant differences were found between the bacterial growth profiles of 4xHt graft and BPTB graft in microcalorimetry and colony counting. In the samples analyzed with electron microscopy, no specific biofilm growth pattern was identified upon comparing BPTB graft to 4xHt graft. Conclusions: There were no significant differences at either the quantitative or qualitative level when comparing bacterial growth in BPTB graft to that in 4xHt graft. Therefore, the presence of sutures in 4xHt graft cannot be established as a predisposing factor for increased biofilm growth in this in vitro study.

Septic arthritis (SA) represents an orthopedics urgency and mainly affects the knee joint. Due to its devastating effects on cartilage, immediate management is crucial. SA is characterized by an an-nual incidence of 2 to 10 cases per 100,000 individuals, with mortality rates fluctuating between 0.5% and 15%, with a substantially higher mortality rate observed in older people (15%) in con-trast to younger cohorts (4%). The etiology of septic arthritis is multifactorial: a spectrum of Gram-positive and Gram-negative bacteria can contribute to the development of this condition, especially Staphylococcus aureus. The treatment involves urgent (arthroscopic or arthrotomic) debridement associated with adequate antibiotic therapy. Intra-articular antibiotic carriers can also be used to increase their local concentration and effectiveness. The case of a 67-years-old woman affected by knee SA from methicillin-susceptible S. aureus is presented. She was treated with an arthroscopic debridement enhanced by intra-articular antibiotic-loaded calcium sulphate beads, together with antibiotic therapy. At 2-year follow-up, the infection has been eradicated and the patient fully recovered. This is the first description, to our knowledge, in English Literature of the use of antibiotic-loaded calcium sulphate beads as an adjuvant in the surgical treatment of SA of a native knee joint.

Rotator cuff (RC) repair leads to less than optimal results when dealing with massive lesions, so the use of scaffolds as biological support has been proposed to improve RC repair site biology. The present study aims to evaluate the clinical and radiographical outcomes in a series of patients suffering from massive or irreparable RC tears treated with patch (porcine or human scaffolds) repair (augmentation or bridging). Twenty-three patients with a minimum follow up of 24 months were subjectively with 3 patient-related outcome measurements (PROMs) (American Shoulder and Elbow Surgeons score, Constant score, visual analog scale), clinically and radiographically assessed before and after surgery. Regardless the technique (augmentation or bridging) or type of scaffold (porcine or human) employed, at follow up all patients experienced a statistically signif-icant improvement in all PROMs, and clinically. Patch repair represents a valid salvage procedure in massive or irreparable RC tears improving the quality of life and eventually delaying the need for prosthetic replacement.
