Background and objectives: Although intravenous tissue plasminogen activator (rt-PA) has been shown to be effective in the treatment of acute ischemic stroke (AIS) only a small proportion of stroke patients receive this drug. Low administration rate is mainly due to delayed presentation of patients to the emergency department (ED) or lack of a stroke team/unit in most of the hospitals. So, the aim of this study is to analyze ED time targets and the rate of rt-PA intravenous administration after the initial admission of patients with AIS in an ED from a traditional healthcare center (without a neurologist or stroke team/unit). Methods: To analyze which factors, influence administration of rt-PA we split the general sample (n=202) in two groups: group No rt-PA (n=137) and group rt-PA (n=65) based on performing or no intravenous thrombolysis. Results: Analyzing ED time targets for all sample we found that the median onset-to-ED door time was 180 minutes (IQR, 120-217.5 minutes), door-to-physician time was 4 minutes (IQR, 3-7 minutes), door-to-CT time was 52 minutes (IQR, 48-55 minutes), and door-in-door-out time was 61 minutes (IQR, 59-65 minutes). ED time targets such as door-to physician time (p=0.245), door-to-CT time (p=0.219), door-to-door-out time (p=0.24), or NIHSS at admission to Neurology department (p=0.405), or NIHSS after 24 h (p=0.9) did not have a statistically significant effect on administration or no rt-PA treatment in patients included in our study. Only highest Door-to-CT time was statistically significantly correlated with death outcome. Conclusion: In our study, rt-PA administration rate was higher than expected. Statistically significant correlation between ED time targets was found between lower Onset-to-ED door time with administration of rt-PA treatment (p<0.001) and between highest Door-to-CT time and death outcome.

After developed the formulation of a "general relativity" in C⁴ [2], we proceed with the formulation of a Hamilton-Jacobi equation in C⁴. We argue that in this consideration, the usual problems of the ADM formalism, do not exist, due to the complex time as it exists in our consideration. Specically, we can derive a suitable dispersion relation in order to work with and nd a generalised super Hamiltonian

We explore several ways, in order to include fermionic structures naturally in a physical theory in C⁴. We begin with the standard Dirac formalism and we proceed by using Cartan's property of triality as a second option. Afterwards, we suggest a new approach (in a preliminary basis), by introducing an 1-linear form, as the "square root of the geometry" derived by the usual 2-linear forms (quadratic forms). Keeping this way, we introduce n-linear forms, in order to formulate a new geometric structure, which could be suitable for the formulation of a pure geometric unied theory.

We explore the possibility to nd the usual quantum theories, within the formulation of a classic theory of mechanics in C⁴. Specically, by releasing the end-point of the integral of the action derived in C⁴, we derive the dynamic path length of the geodesic equation in C⁴. In the at case, the derived Hamilton-Jacobi equations, were identied as the usual Klein-Gordon equation, where the complex functional action S(z_i), is identied as the usual complex scalar field φ. Afterwards, we study the energy-momentum 4-d complex vector, in order to re-establish the usual covariant derivative of gauge theories.

We explore the possibility to form a physical theory in C⁴. We argue that the expansion of our usual 4-d real space-time to a 4-d complex space-time, can serve us to describe geometrically electromagnetism and unify it with gravity, in a different way that Kaluza-Klein theories do. Specically, the electromagnetic eld A_μ, is included in the free geodesic equation of C⁴. By embedding our usual 4-d real space-time in the symplectic 8-d real space-time (symplectic R⁸ is algebraically isomorphic to C⁴), we derive the usual geodesic equation of a charged particle in gravitational eld, plus new information which is interpreted. Afterwards, we explore the consequences of the formulation of a "special relativity" in the at R⁸.

Objective: We aimed to clarify the differences in the association between re-admission for heart failure (HF) and left atrial (LA) overload indices by sex in heart failure and a preserved ejection fraction (HFpEF). Methods: We analysed 898 HFpEF patients hospitalized for acute decompensated HF. Blood tests and transthoracic echocardiography were performed before discharge. The primary endpoint was re-admission for HF during the first year. Results: The ratio of diastolic elastance to arterial elastance (p=0.014), a relative index of LA pressure overload, in men and LA volume index (LAVI, p=0.020) in women were significant for re-admission for HF during the first year in the multivariable Fine-Gray analysis. Stroke volume (SV)/LA volume (LAV), another index for LAV overload, was not a significant prognostic factor of re-admission for HF during this time. Conclusion: LA overload was an important prognostic factor for HF re-readmission during the first year after enrolment in patients with HFpEF, but the indices relating to LA overload differed by sex.

Spacetime torsion is known to be highly suppressed at the end of inflation, called preheating. Of course this fact does not avoid that metric could affect the cosmic dynamos at preheating phase as was showed by Bassett et al, in the general relativity (GR) case. In this paper, is to show that a torsionful magnetogenesis in QED effective lagrangean, drives either a torsion damping or an amplification mechanism or dynamo effect, depending upon torsion chirality. The present extension of Basset et al theory yields a new magnetic field expression $B\sim{a^{(1\pm{\beta})}}$, obtained from dynamo equation in QED. Here a cosmic factor ${\beta}=g_{T}{\eta}k$ is present. In a second example we find Higgs inlationary fields in Einstein-Cartan gravity thick domain walls (DWs). Recently a moodified Einstein-Cartan gravity was given by M Shaposhnikov et al [PRL (2020)] to obtain higgs like inflatons as a portal to dark energy.

We derive a concentration inequality for the uncertainty in stratied random sampling. Minimising this inequality leads to an iterated online method for choosing samples from the strata. The inequality is versatile and considers a range of factors including: the data ranges, weights, sizes of the strata, as well as the number of samples taken, the estimated sample variances and whether strata are sampled with or without replacement. We evaluate the improvement this method reliably offers against other methods over sets of synthetic data, and also in approximating the Shapley value of cooperative games. The method is seen to be competitive with the performance of perfect Neyman sampling, even without prior information on strata variances. We supply a multidimensional extension of our inequality and discuss some future applications.

This work compares the multi-element analysis of human hair and nails using inductively coupled plasma mass spectrometry (ICP-MS) with an easy, fast, cheap, non-destructive method using energy dispersive x-ray fluorescence (EDX-RF). The ICP-MS-based method was more sensitive (over 30 elements could be quantified) and costly (requiring more time, sample, and chemicals). The EDX-based method required laboratory and certified reference materials made of hair for instrument calibration. It was less sensitive (16 elements could be quantified: S, Si, Ca, Br, Fe, Cu, Cr, Mg, Si, K, Mn, Ni, Zn, Se, Sr, Pb), but it allowed to replace troublesome grinding with the dissolution of keratin-based material with alkalic agent (tetramethylammonium hydroxide, TMAH) and formation of stable for days pellets. The method is simple, enables automation, and, due to the modification of wells in the autosampler of the EDX system by immersion of home-designed inserts, it requires smaller amounts of biological material and binder (down to 70 mg instead of 500 mg required by commercially available instrument) to perform analysis. It was concluded that the EDX-based method offers complementary selectivity and sensitivity to ICP-MS with the possibility of the reuse of the sample for further analysis.

TThe aim of the work is to present a newly developed stand and methodology of accelerated tests of fatigue strength of flexible propellers and fins. The paper presents the design and concepts of the measuring elements used in the research. Test methodology and water stressing were devel-oped. The mechanical properties of the following materials were presented: Arnitel EL-550 and QUEO 8230 with the addition of PP / EVA 50/50 (polypropylene / ethylene-vinyl acetate) - in the proportion of 70/30 (QUEO 8230 / additive). The constructed test stand and a new method of fa-tigue testing of biomimetic fins made it possible to determine the number of fin movement cy-cles under a given load, which may facilitate the determination of a warranty for a newly intro-duced product. The current consumption by the fin drive and video monitoring allowed for the selection of the range of the number of movement cycles and the determination of the moment of fatigue cracks until the fin structure loses its capacity. During the analysis, the test results showed that the fins made of Arnitel EL-550 did not show changes in the measured values of the current and the force generated by the fin. For a fin made of QUEO 8230, at 35,000 cycles, there is a decrease in the current and force consumed, which indicates the initiation of fatigue damage to the fin. The performed tests can be the basis for the development of standards for the fatigue strength of flexible propellers.

Cardiovascular diseases (CVDs) and type 2 diabetes mellitus (T2DM) are two of the four major chronic non-communicable diseases (NCDs), representing the leading cause of death worldwide. Several studies demonstrate that endothelial dysfunction (ED) plays a central role in the pathogenesis of these chronic diseases. Although it is well known that systemic chronic inflammation and oxidative stress are primarily involved in the development of ED, recent studies have shown that perivascular adipose tissue (PVAT) is implicated in its pathogenesis, also contributing to the progression of atherosclerosis and to insulin resistance (IR). In this review we describe the relationship between PVAT and ED, and we also analyse the role of PVAT in the pathogenesis of CVDs and T2DM, further assessing its potential therapeutic target with the aim of restoring normal ED and reducing global cardiovascular risk.
