Background: To describe the effectiveness and safety of COVID-19 vaccine in a series of patients with rheumatoid arthritis (RA). Methods: Retrospective single-center study of RA patients, fulfilling the ACR 1987 or ACR/EULAR 2010 classification criteria, that received first COVID-19 vaccine between December 2020 and October 2021, had post-vaccination serology and subsequent follow-up of at least 6 months in a university hospital. Vaccine effectiveness was evaluated by the serological response and the incidence of post-vaccine COVID-19, and safety by the incidence of adverse events (AE) and RA flares. Adjusted logistic and linear multivariate regression analyses were carried out with Stata® to identify factors associated with vaccine response. Results: We included 118 patients with RA (87.2% women, age 65.4±11.6 years, evolution 12.0±9.6 years). 95.8% received complete vaccination schedule. Most patients (88.1%) developed adequate humoral immunogenicity, and the degree of serological response was significantly related to younger age and previous COVID-19 infection. After vaccination, 18.6% presented mild SARS-CoV-2 infection. Vaccine AE (19.5%) were mostly mild and inversely associated with age (OR 0.95). RA flares were anecdotal (1.7%) and inversely related to age (OR 0.95). Conclusion: Our results suggest that COVID-19 vaccine induces adequate humoral immunogenicity, with an acceptable safety profile in RA patients.

Objective: To assess the spectrum of PET-CT-related large vessel vasculitis (LVV) in a Spanish tertiary center and to determine whether FDG uptake by PET-CT differs between giant cell arteritis (GCA) with predominant cranial or extracranial phenotypes. Methods: The spectrum of patients diagnosed with LVV by PET-CT in a tertiary referral hospital that cares for 450,000 people over a period of two years was reviewed. Moreover, differences in FDG uptake between LVV-GCA with predominantly cranial and extracranial phenotype were analyzed. Results: Eighty patients were diagnosed with LVV by PET-CT. Most were due to systemic vasculitis (n=64; 80%), especially GCA (n=54; 67.5%). Other conditions included the presence of rheumatic diseases (n=4; 3.2%), tumors (n=9; 7.2%), and infections (n=3; 2.4%). LVV-GCA patients with predominant extracranial GCA phenotype were younger (mean ± SD: 68.07 ± 9.91 versus 75.46 ± 7.64 years; p= 0.017) and had a longer delay to the diagnosis (median [interquartile range] 12 [4-18] versus 4 [3-8]; p=0.006), but had PMR symptoms more frequently than those with predominantly cranial GCA phenotype (46.3% versus 15.4%, p= 0.057). When FDG uptake was compared according to the two different disease patterns, no statistically significant differences were observed. However, patients with extracranial LVV-GCA showed a non-significantly higher frequency of vasculitic involvement of lower extremity arteries. Conclusion: Regardless of the predominant phenotype, LVV identified by PET-CT is more commonly due to GCA in the Spanish population. In these GCA patients, younger age, PMR and a higher frequency of lower extremity artery vasculitis suggest the presence of LVV.

Background/Objectives: Patients with rheumatoid arthritis (RA) have an increased risk of infections. The risk of presenting herpes zoster (HZ) is 1.5-2 times higher than in immunocompetent individuals and disseminated presentation is more frequent. Our aim was to analyze the prevalence and general features of HZ in RA patients. Methods: Prospective study of 392 RA patients included in the vaccination program of our hospital between 2011 and 2016, and followed-up until December 2020. Diagnosis of HZ was made according to clinical manifestations: skin rash, blisters, paresthesia and local pain in one or more dermatomes. Results: We studied 392(309women/83men), mean age 59±13years. Every patient was followed-up during a mean period of 137±110 months (range: 42months- 42years). HZ infection was observed in 30 of 392 (25women/5men) patients; age (mean±SD) 64.7±11.8years. Prevalence was 7.65% in this period and incidence rate was 13.22/1000patients/year. Three patients had facial involvement, one with optic involvement and one patient presented a disseminated HZ. Seven patients presented postherpetic neuralgia treated with gabapentinoids. The main features of RA of these 30 patients were: positive RF(n=17; 56.6%), positive anti-CCP(13; 43.3%) and erosive disease(10; 33.3%). At HZ infection they were in treatment with glucocorticoids (19; 63.3%), conventional DMARDs (15; 50%), biological DMARDs (15; 50%), tofacitinib (2; 6.6%) and upadacitinib (1; 3.3%). Conclusions: HZ is a relative frequent viral complication of RA patients. In our series, one patient presented a disseminated HZ and nearly 25% of patients had post-herpetic neuralgia. Include HZ vaccine in our vaccination program in RA patients may be beneficial.
