Under batch experiment conditions, this work seeks to successfully remove Diclofenac-Na (DCF-Na) from an aqueous solution utilizing a composite sorbent made of Bentonite, Kaolinite clay, and Worm casting (BKW). This study investigated the structural modification of the H₃PO₄ Modified Clay by X-ray fluorescence and the effect of selected adsorption factors – DCF-Na concentration and modified BKW composite dosage. The concentration equilibrium data was used to study six isotherm models. Freundlich isotherm model better explained the adsorption of DCF-Na onto modified BKW composite with a correlation coefficient close to 1. Kinetics models were examined, and the Elovich model gave a better fit than other kinetic models studied. Mass diffusion mechanisms and thermodynamics studies were successfully carried out. The enthalpy change values evaluated were negative, which revealed the spontaneity of DCF-Na remediation onto modified BKW, and that DCF-Na adsorption is exothermic and occurred through a physisorption process.

The present work describes the synthesis, characterization and photomineralization activity of synthesized Ce1-xZnxO2-x solid solution catalysts allowing the degradation of Diclofenac as a model of anti-inflammatory medicines in water. Oxygen deficient photocatalysts Ce1-xZnxO2-x (CeZnx), produced with mixing ZnO and CeO2 have been characterized for their crystallographic parameters, specific surface area and morphology. Photomineralization activity determinations using TOC analysis have shown efficient diclofenac photooxidation under sunlight. Moreover, results indicate that coexistence of Zn2+, Ce4+ and oxygen vacancies rate in CeZnx solid solution are key factors for strong drug mineralization. Finally, CeZn0.1 which is one of the photocatalysts synthesized in the present work represents a cheap and efficient reagent for organic matter photomineralization in wastewater.

Introduction: Diclofenac is the most prescribed non-steroidal anti-inflammatory drug worldwide and used to reliev pain and inflammation for inflammatory arthritis. Diclofenac do not slows disease progression and cartialge damage of Rheuamtoid Arthritis individuals. Moreover, it associated with seriuos adverse effects even using regular dose regimens. Drug delivery systems can overcome this issues reducing adverse effects and optmizing efficacy. Objectives: to evaluate the activity of a lipid-core nanocapsule loaded of Diclofenac (DIC-LNC) in an experimental model of adjuvant-induced arthritis and its anti-arthritic properties at the joint components. Methods: The diclofenac nanoformulation was obtained by self-assembling methodology. The stereology analysis aproach was applied for morphological quantification of the volume, density and cellular profile count of the metatarsophalangeal joints of rats induced to adjuvant arthritis. Proinflamatory cytokines and biochemical profile was also obtained. Results: DIC-LNC is able to reduce arthitis compared to control group (p<0.0001) and DIC group (p=0.009). The TNF and IL1 cytokine as well as C-reative protein and Xanthine-oxidade were efficiently reduced by DIC-LNC. Additionally, DIC-LNC reduces synovites and condrocytes lossing compared to DIC (p<0.05)and control group (p<0.05). The synovial space volume was higher for DIC-LNC compared to DIC (p<0.05) and Control (p<0.05). These data are suggesting that DIC-LNC is showing anti-arthritic actvity preserving deep joint components. Conclusion: DIC-LNC is a promissing nanoformulation for clinical use, since is able to reduce joint inflamation and synovits, avoiding damage of cartilage and synovial space at advjuvant athrits. Further studies and developments are necessary to achieve future clinical use.

Stem cell-based regeneration strategies have shown therapeutic efficacy in various fields of regenerative medicine. These include bone healing after bone augmentation, often complicated by pain, that is managed by using non-steroidal anti-inflammatory drugs (NSAIDSs). Information is limited about how NSAIDSs affect the therapeutic potential of stem cells. Therefore, we investigated the effects of ibuprofen and diclofenac in low and high therapeutic doses on the properties of human mesenchymal stromal cells isolated from the dental pulp (DPSCs) and cultured in vitro. Ibuprofen and diclofenac significantly reduced the viability of DPSCs, while the expression of mesenchymal stem cell surface markers was unaffected. Both ibuprofen and diclofenac treatment significantly upregulated the expression of HGF, while the expression of VEGFA remained unchanged. Ibuprofen significantly altered the expression of several apoptosis-related genes, including upregulation of CASP9 and BCL2, with decreased CASP3 expression. BAK, CASP3, CASP9, and BCL2 expressions were significantly increased in the diclofenac-treated DPSCs, while no difference was demonstrated in BAX expression. Our results suggest that concomitant use of the NSAIDSs ibuprofen or diclofenac with stem cell therapy may negatively impact cell viability and alter the expression of apoptosis-related genes, which can affect the efficacy of stem cell therapy.

Many studies have been conducted using pectin for modified drug delivery. In this regard, the goal of the work was to obtain interpolyelectrolyte complexes based on citrus or apple pectin and the Eudragit® EPO and study properties from the standpoint of using these carriers for oral drug delivery. Turbidity, gravimetry, viscosity, elementary analysis and FTIR spectroscopy, DSC analysis were used for physicochemical characterization. Diffusion-transport characteristics were studied to assess the swelling ability of the matrices and the release of diclofenac sodium. Mathematical modeling of the release parameters was carried out using the Krosmayer-Peppas equation. During the turbidity study, stoichiometry compositions were selected for the studied IPECs EPO/PeсA and EPO/PeсC at pH values = 4.0, 5.0, 6.0, 7.0. FTIR spectra of the complexes are characterized by an increase in the intensity of the bands at 1610 cm-1 and 1400 cm-1. According to the DSC analysis IPEC has a certain Tg = 57.3°C. The highest release rates are shown by IPEC EPO/PecC_1 and EPO/PecC_4. The mechanism of drug transport from the matrices IPEC EPO/PecC, IPEC EPO/PecA_3 and EPO/PecA_4 can be characterized as Super Case II. Anomalous (non-Fickian release) is typical for IPEC EPO/PecA_1 and EPO/PecA_2. Thus, the resulting systems are suitable for colon-specific drug delivery systems.

Breast cancer is the most common malignant disease in women. Preclinical studies have confirmed that the local anesthetic levobupivacaine has a cytotoxic effect on breast cancer cells. We examined whether postoperative wound infiltration with levobupivacaine influences survival in 120 patients who were operated on for breast cancer and underwent quadrantectomy or mastectomy with axillary lymph node dissection. Groups with continuous levobupivacaine wound infiltration, bolus wound infiltration, and diclofenac analgesia were compared. Long-term outcomes examined were quality of life, shoulder disability, and hand grip strength (HGS) after one year, and survival after 5 and 10 years. Groups that had infiltration analgesia had better shoulder function compared to diclofenac after one year. Levobupivacaine PCA group had the best preserved HGS after 1 year (P=0.022). The most significant predictor of the 5-year outcome was HGS (P=0.03). Although the best survival after 5 and 10 years was registered in the bolus levobupivacaine group, statistical significance was not reached (P=0.36). The extent of the disease at the time of surgery is the most important predictor of long-term survival. A larger prospective clinical study could better confirm the effect of levobupivacaine wound infiltration on outcomes after breast cancer surgery observed in this pilot study. Trial number NCT05829707
