Using the sustainable livelihoods analytical framework, adaptability of cattle raising to multiple stressors (e.g. climate change and market conditions) in the dry tropics of Chiapas, Mexico was evaluated. Three case studies located in the Frailesca region of Chiapas were analyzed: (I) peasant cattle raising in a rural village in the Frailesca Valley; (II) peasant cattle raising in a rural village in a natural protected area in the Frailesca Highlands; and (III) holistic cattle raising by farmers with private land ownership in the Frailesca Valley. Adaptability was evaluated using an index on a scale of one to a hundred; average values were: case I = 20.9 ± 1.4; case II = 32.1 ± 1.8; and case III = 63.6 ± 3.5. In order to increase farms adaptability and reduce the vulnerability of cattle raising families, there is a need to modify public policy to take into account the conditions of the most vulnerable farmers (cases I and II). Given the economic, environmental, and social context of Mexico´s dry tropics, establishing ecological or organic cattle raising and silvopastoral systems may reduce the vulnerability of farm families and increase their level of adaptability of their farms to multiple stressors.

A positive fluid balance may evolve to fluid overload and associate with organ dysfunctions, weaning difficulties, and increased mortality in ICU patients. We explored whether individualized fluid management, assessing fluid-responsiveness via passive leg-raising maneuver (PLR) before spontaneous breathing trial (SBT) associated with less extubation failure in ventilated patients with high fluid balance, admitted to the ICU after liver transplantation (LT). We recruited 15 LT patients in 2023. Postoperative fluid balance was +4476 [3697,5722] mL. PLRs were conducted at ICU admission (T1) and pre-SBT (T2). Cardiac index (CI) changes were recorded before and after SBT (T3). Seven patients were fluid responsive at T1, and 12 at T2. No significant differences occurred in hemodynamic, respiratory, and perfusion parameters between fluid-responsive and fluid-unresponsive patients at any time. Fluid-responsive patients at T1 and T2 increased their CI during SBT from 3.1 [2.8,3.7] to 3.7 [3.4,4.1] mL/min/m2 (p= 0.045). All fluid-responsive patients at T2 were extubated after SBT and consolidated extubation. Two out of three of fluid-unresponsive patients experienced weaning difficulties. We concluded that fluid-responsive patients post-LT may start weaning earlier and achieve successful extubation despite high postoperative fluid balance. This highlights the profound impact of personalized assessment of cardiovascular state on critical surgical patients.

Rising humidity in old and historical buildings is a current issue that affects their structure, due to water capillarity and its propagation inside the walls, a key factor in heritage preserving and its functionality. It is also responsible for materials degradation exposed to frost and defrost cycles, biodegradation and salt crystallization. Moisture also increases the heating costs of the affected buildings, due to high thermal conductivity of the moist masonries. The paper presents an unconventional method for humidity removal or stabilisation due to infiltration of capillary water into the masonries, using electromagnetic waves systems that result in changing the water polarity inside the affected walls and pushing it out of the construction. The system consist of a small box, connected to the power supply, installed inside the building, remaining on the same location throughout the entire life of the construction. By means of this procedure, not only do the walls of the structure stay moisture-free, but also the foundations and the surrounding soil. Through this process, not only the walls of the construction remains dry, but also the foundations and the soil around them. The electromagnetic wave method was applied on five different historical buildings, located in Romania, Cluj Napoca’s historical centre, in the vicinity of Somes river, with rising damp due to water capillarity. Measurements were conducted at every six months after the system installation, for a period of minimum 24 months, using the same marked locations, at a depth of 5 cm in the walls. All buildings studied are made of stone and stone with plain brick, with different chemical, type, pore direction and porosity characteristics. The measured values show in most cases, a clear decrease of wall humidity levels or a stagnation of the capillarity rise in the monitored walls, after a minimum of 24 months after the system installation. The electromagnetic wave system, as a non-destructive method, can be applied to cases with no accidental water source or high raised groundwater, for historical buildings where no exterior works are possible or interior invasive interventions.

(1) Background: Breast cancer is the second leading cause of cancer death among women. The accurate prediction of survival intervals will help physicians make informed decisions on treatment strategies or the use of palliative care for patients; (2) Methods: The gene expression is predictive and correlates to patient prognosis. To establish a reliable prediction tool, we collected the RNA-seq data of breast cancer patients, a total of 1187 RNA-seq data (median age 58 years), in FPKM format from the TCGA database. Among them, 144 RNA-seq data with date of death information was selected to establish the SaBrcada-AD dataset. We first normalized the SaBrcada-AD dataset to transcripts per million (TPM) to build survival prediction model SaBrcada. After normalization and dimension raising, the differential gene expression data were used for testing eight different deep learning architectures. Among them, GoogLeNet performed the best. Considering the effect of age on prognosis, we examined all ages between the lower and upper quartiles of patient age for a stratified random sampling test; (3) Results: Stratifying by age based on a cut-off of 61 years of age improved the accuracy of SaBrcada compared to previous findings, resulting in an accuracy of 0.798. We also built a free website tool to provide 5 kinds of predicted survival period information for clinician reference; (4) Conclusions: We established a breast cancer survival analysis prediction model, SaBrcada, and a website tool with the same name. Through this highly reliable survival analysis model and website tool, information on survival intervals will be provided for clinicians as part of precision medicine.
