This study proposes a cephalometric analysis that can be used for biomechanical simulation by considering the important and complex relationship between craniofacial morphology and masseter muscle (MM) size and inclination and by including muscle values. The MM line of action drawn between the Gonion (Go) and Orbital (Or) point relative to dental and skeletal references (occlusal and Frankfort planes) were analyzed. A total of 510 pre-treatment lateral cephalometric tracings (217 males, 293 females, 6-50 years) and lateral Bolton standard tracings (6-18 years) were analyzed. The study examined the following: a) skeletal-cutaneous class (linear distance in mm between the projections of points A' and B' on the occlusal plane), b) the angle between the perpendicular line to the occlusal plane and the Go-Or line at the molar occlusal point, and c) the angle between the Go-Or line and the Frankfort plane. The skeletal classes of the patients ranged from -14.5 to 15.5 mm and did not differ by sex or age; the angle between the Go-Or line and the normal to the occlusal plane averaged 39° (range 15° to 53°) and decreased with age, while the angle between the Go-Or line and the Frankfort plane averaged 42° (range 30° to 54°), increased in older patients. No effect of gender was observed. The two angles were also significantly correlated with each other, but not with sagittal jaw discrepancy. Similar results were obtained with Bolton tracings; the positional relationship of the Go-Or line to the teeth and skeletal landmarks is partially consistent with the literature. The present cephalometric analysis can be usefully used to estimate the mechanical advantage of MM in biomechanical simulations of masticatory muscle performance.

The advent of AI in medicine has transformed various medical specialties, including orthodontics. AI has shown promising results in enhancing the accuracy of diagnoses, treatment planning, and predicting treatment outcomes. With the growing number of AI applications and commercially available tools, there is an increase in their usage in orthodontic practices worldwide. This review aims to explore the principles of artificial intelligence (AI), its applications in the diagnostic process of modern orthodontic practices, and concerns associated with the implementation of AI algorithms in clinical practice. A comprehensive review of the literature was conducted, focusing on five categories where AI has been applied in orthodontics: dental diagnostics, cephalometric evaluation, skeletal age determination, temporomandibular joint (TMJ) evaluation, and extraction decision making. AI has demonstrated high efficacy in all those fields. However, variations in performance and the necessity of manual supervision indicate that AI should be used cautiously in clinical settings. Nevertheless, the high complexity and potential unpredictability of AI algorithms call for cautious implementation and regular manual validation of results. Continuous AI learning, proper governance, and addressing privacy and ethical concerns are crucial for the successful integration of AI into orthodontic practice.

Incidental findings (IFs) depicted in imaging tests during diagnostic evaluation of patients with cleft lip and/or palate (CL/P) can guide clinicians during treatment-planning procedures for effective, comprehensive cleft care. Evidence regarding IFs in different anatomical regions distant from the dentition are scarce. The aim of this study was to compare the prevalence of IFs in various anatomical areas between patients with non-syndromic CL/P and unaffected subjects. Orthopantomographs (OPT), lateral cephalometric (LC) radiographs and cone-beam computed tomography (CBCT) scans of 120 subjects (case group: n = 40, 18 females, age 14.5 ± 5.0 years; control group: n = 80, 36 females, age 14.6 ± 4.9 years) were examined and IFs were assessed by four observers. Significantly higher prevalence of IFs (p < 0.001) was reported in the case group (97.5%) compared with the control group (62.5%). The most prevalent location of IFs in the case group was the maxilla (92.5%), followed by the nasal cavity (75.8%) and the mandible (63.2%), while in the control group, the highest rates of IFs occurred in the maxilla (43.8%), mandible (41.3%) and temporomandibular joint (TMJ) (14.7%). No gender-dependent pattern for IF occurrence was detected overall. Orofacial clefts may affect the mandible. However, the reported IFs were limited compared to those in the maxilla. In conclusion, individuals with CL/P are more likely to present with a range of IFs than their unaffected peers. Understanding the association between CL/P and IFs is critical for successful interdisciplinary treatment, raising awareness of the potential need for future dental care for cleft patients, and managing extra-dental aberrations. A systematic assessment of diagnostic records is required to eliminate the risk of overlooking clinically significant IFs.

Introduction: A plethora of biological molecules regulate chondrogenesis in the epiphyseal growth plate. Disruptions of quantity and function of these molecules can manifest clinically as stature abnormalities of various etiologies. Traditionally, the growth hormone/insulin-like growth factor 1 (IGF1) axis represents the etiological center of final stature attainment. Of note, little is known about the molecular events that dominate the growth of the craniofacial complex and its correlation with somatic stature. Aim: Given the paucity of the relevant data, this review discusses available information regarding potential applications of lateral cephalometric radiography as a potential clinical indicator of genetic short stature in children. Materials and Methods: A literature search was conducted in the PubMed electronic database using the key words: cephalometric analysis and short stature; cephalometric analysis and achondroplasia; cephalometric analysis and hypochondroplasia; cephalometric analysis and skeletal abnormalities; cephalometr* and SHOX; cephalometr* and CNP; cephalometr* and ACAN; cephalometr* and CNVs; cephalometr* and IHH; cephalometr* and FGFR3; cephalometr* and Noonan syndrome; cephalometr* and “Turner syndrome”; cephalometr* and achondroplasia. Results: In individuals with genetic syndromes causing short stature, linear growth of the craniofacial complex is confined, following the pattern of somatic short stature regardless of its etiology. The angular cephalometric measurements differ from the average height normal individuals and are suggestive of a posterior placement of the jaws and a vertical growth pattern of the face. Conclusion: The greater part of the existing literature regarding cephalometric measurements in short statured children with genetic syndromes provides qualitative data. Furthermore, cephalometric data for individuals affected with specific rare genetic conditions causing short stature, should be the focus of future studies. These quantitative data are required to potentially establish cut-off values to refer for genetic testing based on craniofacial phenotypes.

Three-dimensional convolutional neural networks (3D CNN) as a type of artificial intelligence (AI) are powerful in image processing and recognition using deep learning to perform generative and descriptive tasks. The advantage of CNN compared to its predecessors is that it automatically detects the important features without any human supervision. 3D CNN are used to extract features in three dimensions where input is a 3D volume or a sequence of 2D pictures e.g., slices in a cone-beam computer tomography scan (CBCT). The main aim of this article was to bridge interdisciplinary cooperation between forensic medical experts and deep learning engineers. With emphasis activating clinical forensic experts in the field with possibly basic knowledge of advanced artificial intelligence techniques with interest in its implementation in their efforts to advance the forensic research further. This paper introduces a novel workflow of 3D CNN analysis of full-head CBCT scans. Authors explore and present 3D CNN method for forensic research design concept in five perspectives: (1) sex determination, (2) biological age estimation, (3) 3D cephalometric landmark annotation, (4) growth vectors prediction, (5) facial soft-tissue estimation from the skull and vice versa. In conclusion, 3D CNN application can be a watershed moment in forensic medicine, leading to unprecedented improvement of forensic analysis workflows based on 3D neural networks.
