Chronic myeloid leukemia (CML) is the most common form of leukemia in adults and accounts for approximately 15% of all leukemia cases. The etiology of this disease is based on the presence of the oncogenic Bcr/Abl fusion protein, which has dysregulated tyrosine kinase activity. This pathological condition is the result of a chromosomal translocation between chromosomes 9 and 22 leading to the formation of the Philadelphia chromosome (Ph). Clinical establishment of targeted therapy with Abl kinase inhibitors (AKIs) has been observed in the treatment of CML. Therapy resistance in patients with CML is due to genetic alterations in the cells of CML, specifically affecting the Bcr/Abl oncoprotein. Acquired resistance of CML patients to tyrosine kinase inhibitors (AKIs) has also been associated with the tumor microenvironment (TME). This study aimed to examine the role of hypoxic conditions in the development of chemoresistance to certain AKIs in CML. The findings of our study indicate that exposure to hypoxic conditions leads to a substantial increase in chemoresistance to crizotinib, while only resulting in a slight increase in chemoresistance to imatinib. It is worth noting that both drugs effectively block Bcr/Abl function. Furthermore, it was observed that the inclusion of the JAK1/2 inhibitor ruxolitinib resulted in an augmentation of chemoresistance to crizotinib in the context of hypoxia. Overexpression of hypoxia-inducible factor 1a (HIF1a) and silencing of JAK2 confirmed that the two proteins cooperate in mediating chemoresistance to crizotinib in CML cell lines. It is noteworthy that the compound 2-methoxy estradiol (2ME2), which is a metabolite of estradiol lacking estrogenic activity, has shown efficacy in reinstating the sensitivity of CML cells to crizotinib in the presence of hypoxic circumstances. Additionally, when used in conjunction with a JAK2 inhibitor, 2ME2 exhibited activity in restoring crizotinib sensitivity in CML cells. The findings of our study underscore the significance of selectively targeting elements of the HIF1α pathway in order to achieve total eradication of chronic myeloid leukemia (CML) cells.

Naegleria fowler, also known as brain-eating amoeba is a thermophilic microorganism that causes primary amoebic encephalitis (PAM) having >98% mortality rate. To date, there are no specific drugs identified to treat this disease. N. fowleri expresses only one form of hexokinase i.e. glucokinase (nGLK) unlike humans that express four isozymes of hexokinase including glucokinase (hexokinase IV) or hGLK that is critical for the first step of glycolysis. Until now, there have not been many studies on glucokinase as a drug target to treat PAM, hence, this study provides an exhaustive analysis of this relatively novel drug target. Thus, it was hypothesized that amoebic glucokinase can arrest carbohydrate metabolism and assist regulating its virulence. To find suitable antagonists to inhibit the activity of nGLK, we first tested the present recommended drugs by the CDC on their ability to bind nGLK. Interestingly our studies show that many of the present recommended drugs can inhibit both amoebic (nGLK) and human (hGLK) homologs of glucokinase. We identified new inhibitors for nGLK and further explored suitable hGLK antagonists for the potential treatment of diabetes. Despite significant structural differences between nGLK and hGLK, it was interesting to note that all our screened inhibitors bound with both nGLK and hGLK. Levomefolic acid; Ziprasidone; and Risperidone were found to be more suited to treat PAM while, Simeprevir; Indacaterol; Crizotinib and Mocetinostat were found to be suited for treating diabetes. The selections were based on multiple criteria including binding affinity; ADME scores; absorption; and its ability to cross the blood brain barrier. This study identifies multiple drugs that have potential not only to treat PAM but also to treat diabetes and also use for lab based biochemical assays for glycolysis inhibitors.

Background: The t (2; 5) chromosomal rearrangement and resulting nucleophosmin (NPM1) -ALK fusion was first observed in 1994 in anaplastic large cell lymphoma (ALCL), a T-cell lymphoma responsive to cyclophosphamide, abriblastine, vincristine and prednisone in approximately 80% of cases; refractory cases usually respond favorably to brentuximab-vedotin. These treatments are regarded as a bridge to allogeneic hematopoietic stem cell transplantation (allo-SCT). Nowadays, transplant procedures and monitoring of chemotherapy patients proceed very slowly because the SARS-CoV-2 pandemic has heavily clogged the hospitals in all countries. Results: A 40-year-old Caucasian woman was first seen at our clinical center in June 2020. She had ALCL ALK +, a history of failure to two previous therapeutic lines and was in complete remission after 12 courses of Brentuximab, still pending allo-SCT after two failed donor selection. Facing of a new therapeutic failure, we requested the Italian drug regulatory agency, and obtained the authorization, to administer 250 mg twice a day of Crizotinib, a drug incomprehensibly not registered for ALCL ALK +. Conclusions: The response to Crizotinib was optimal, since no adverse event occurred, and CT-PET persisted negative; this drug has proved to be a valid bridge to allo-SCT
