Background: a 17 years-old girl with a Mitrofanoff appendicovesicostomy developed a large bladder clot due to hematuria after a surgical cystolithotomy in an augmented bladder. Trans-urethral approaches were not feasible due to a previous surgical closure of the bladder neck. Methods: after an unsuccessful trans-appendicovesicostomy bladder washing, the endoscopic evaluation was performed using a 14 Ch rigid cystoscope, surrounded by its own urethral sheath. The clot was progressively fragmented through the cystoscope and a urethral catheter under direct vision. Clot fragments were aspirated obtaining a complete evacuation; Results: the urethral sheath prevented damages to the appendicovesicostomy, allowing at the same time repeated accesses of the cystoscope into the neobladder and ensuring the success of the procedure; Conclusions: the use of urethral sheath 14 Ch though an appendicovesicostomy preserves both the stoma and the channel, making possible endoscopic procedures as blood clot evacuation into the neobladder.

BACKGROUND: To present a new method of urethral pressures examination, and to evaluate diagnostic capabilities of three dimensional profilometry , as an alternative to classical urethral profile (UPP) .Using four channel catheters and dedicated software global urethral pressures image is obtained. The method eliminates the main limitation of classical urethral profilometry where the catheter orientation determines the pressures picture limited to only one point in the uretral circumference while we observed even 50% differences in pressure measures depending on the point of measurement. METHODS: This is a preliminary study containing method presentation and analisys of the use in varied clinical cases of either healthy patients and patients with LUTS. The article includes technique and equipment decription, full evaluation of selected cases, including three dimensional urethral pressures distribution graphics. RESULTS AND CONCLUSIONS:Three dimensional profilometry compared to classical technique is comparable regarding the time, cost, technical difficulty and patient discomfort. At the same time we obtain much more data on the urethral pressures and its distribution.The results are easy for interpretation due to 3D movable graphics created automatically through the dedicated software.

Primary urethral malignancies are rare in clinical practice and elderly female are prone to suffer. The cause of PUC is unknown and may be related to long-term chronic inflammation, infection, sexual intercourse, pregnancy and irritation of the urethra. Incipient symptom of PUC is not distinctness and can easily lead to misdiagnosis. The final diagnosis relied on transvaginal ultrasound-guided mass biopsy and urethral biopsy. Early tumor treatment is mainly based on surgical resection. For advanced tumors, it is recommended to adopt a comprehensive treatment plan combining surgery, radiotherapy and chemotherapy We report this case with the aim of bringing attention to that primary urethral cancer progresses rapidly and has a poor prognosis, it is urged to call for a need of diagnostic and treatment specification.

Urethral prolapse (UP) is a rare condition in which the distal urethral mucosa protrudes circularly from the external urethral meatus. Urethral prolapse is seen in two age groups: prepubertal girls and postmenopausal women. The fact that it is seen in older women shows that it may be acquired. Causes of acquired UP include weakening of smooth muscle layers secondary to estrogen deficiency, chronic cough, chronic constipation, trauma, recurrent urinary tract infections, pelvic radiotherapy, and sexual abuse. The priority in treatment is conservative management, such as manual reduction, sitz baths, topical estrogen creams, and antibiotics. The general consensus is to switch to surgical treatment in patients who do not benefit from a conservative approach. In this case report, we describe the surgical treatment of a 56-year-old postmenopausal female patient with necrotic UP who presented with urinary retention.

The study aims to compare the efficacy and safety of bulking agents and single-incision slings in the treatment of urinary incontinence in 159 patients during a 29-month follow-up period. Of the 159 patients suffering from stress urinary incontinence, 64 were treated with bulking agents and 75 with a single incision sling. The ICIQ-UI-SF, PISQ-12, FSFI, FSDS, and PGI-I were used to assess efficiency and quality of life. In the 29-month follow-up, the bulking agents showed significantly increased efficacy and safety. Compared to the ones treated with a single-incision sling, the patients treated with bulking agents showed less and mild postoperative complications. Furthermore, at our follow-up there was no discernible difference in terms of effectiveness or quality of life between patients treated with bulking agents and those treated with single-incision slings

Urethral pain syndrome (UPS) is still a pathology in which the diagnosis is formulated as a "diagnosis of exclusion". The exact pathogenetic mechanisms are not yet fully understood and clear recommendations for the prevention and treatment of UPS are absent. The goal of the study was to assess the condition of the tissues in the female urethra in UPS, by using transvaginal ultrasound (TVUS) and cross-polarization optical tomography (CP OCT). TVUS showed an expansion in the diameter of the internal lumen of the urethra, especially in the proximal region compared with the norm. Compression elastography revealed areas with increased stiffness (presence of fibrosis) in urethral and surrounding tissues. When studied with CP OCT it was shown that with UPS, the structure of the tissues in most cases was changed: trophic alterations in the epithelium (hypertrophy or atrophy) and fibrosis of underlying connective tissue were observed. The proximal fragment of the urethra with UPS underwent changes identical to those of the bladder neck. This paper showed that the introduction of new technology — CP OCT — in conjunction with TVUS will allow verification of structural changes in tissues of the lower urinary tract at the level of their architectonics and will help doctors understand better the basics of the UPS pathogenesis.
