The objective of this study was to investigate the impact of athletes' respiratory muscle strength, as assessed through maximal inspiratory (MIP) and expiratory pressure (MEP), on aerobic endurance, as indicated by maximal oxygen consump-tion (Max VO2) kinetics. Respiratory muscle strength was assessed using a digital spirometer (Pony FX Cosmed, Italy), while maximal oxygen utilization capacity was measured using a breath-by-breath automatic portable gas analysis system (Cosmed K5, Italy). Statistical analysis was conducted utilizing SPSS 22.0. Based on the standardized regression coefficients (β), it was found that MEP (mean ± SD: 130.95 ± 42.82) and inspiratory diaphragmatic circumference values significantly influenced ventilatory equivalent (VE), oxygen consumption (VO2), and carbon dioxide pro-duction (VCO2). Conversely, the remaining predictor variables did not exhibit a sig-nificant effect on VE (mean ± SD: 134.80 ± 36.69), VO2 (mean ± SD: 3877.52 ± 868.47 ml), and VCO2 (mean ± SD: 4301.27 ± 1001.07 ml). In contrast, measurements of chest circumference (mean ± SD: 91.40 ± 10.72 cm), MEP, and diaphragmatic cir-cumference during inspiration (mean ± SD: 95.20 ± 10.21 cm) were found to signifi-cantly impact Max VO2 (mean ± SD: 58.52 ± 10.74 ml/kg/min), while the other pre-dictor variables did not demonstrate a significant effect on Max VO2. Conclusively, the study revealed that measured values of diaphragmatic circumference during inspiration and MEP exerted a notable influence on Max VO2, VE, VO2, and VCO2. Our findings underscore the importance of considering respiratory muscle strength in assessing and enhancing athletes' aerobic performance. These insights contribute to a deeper understanding of the interplay between respiratory function and exercise capacity, offering potential avenues for optimizing training regimens and perfor-mance outcomes in athletic contexts.

Immunity has become an important aspect of concern, as the spread of corona virus, is on the rise. The strategies to boost and modulate the immunity have therefore become need of the hour. The present study was carried out to evaluate the effect of Charak immunity Tablets (CIT) on innate and adaptive immune response in healthy individuals. It was a single-blind, randomized, placebo-controlled, exploratory study. After obtaining Ethics Committee permission, 36 healthy individuals of either sex aged 18-35 years with prior consent were recruited in the study. They were randomly divided into 2 groups to receive either CIT or Placebo in 2:1 ratio. Both the interventions were given in a dose of 1 tab (500 mg) twice daily. The assessment variables were vitals [temperature, pulse, and blood pressure], respiratory health [respiratory rate, oxygen saturation and peak expiratory flow rate], questionnaire based assessment of immune status, perceived stress and quality of life along with objective assessment of immunity [CD4+, CD8+ counts, Interferon gamma (IFN γ), Tumor necrosis factor alpha (TNF-α) and Interleukin 10 (IL-10)] as well as oxidative stress; [Malondialdehyde (MDA) and Glutathione peroxidase], which were assessed at fixed time points. Of 36 recruited participants, only 18 participants completed the study. CIT treated individuals showed a statistically significant improvement in respiratory health, quality of life, perceived stress and subjective immune status. There was a decrease in the levels of serum IFN γ on day 60 compared to baseline. TNF-α and IL-10, both estimated from supernatant of Peripheral Blood Mononuclear Cells (PBMCs) stimulated with lipopolysaccharide (LPS), showed a decrease and a significant increase respectively on day 60 compared to baseline in CIT group. Further, CIT significantly decreased MDA levels.The present study indicates that CIT is an effective and safe drug to boost immunity. However, our findings need to be confirmed in larger sample size using more specific immune parameters.

Background: COVID-19 pneumonia caused by SARS-Cov-2 virus induces alveolar collapse and hypoxia that may become severe. The aim of the study is to analyze the effects of chest physiotherapy using a threshold valve in patients with acute respiratory failure due to COVID-19 pneumonia. Methods; Retrospective observational study, in hospitalized patients from March to May 2020. Breathing exercises were performed with a threshold valve of 10 cmH2O. Fraction of inspired oxygen, oxygen saturation, heart rate, respiratory rate and dyspnea were collected before and after the first session and at the end of the 5th day of chest physiotherapy treatment. Results: The final sample included 125 patients. Significant differences (p<0.01) were found in the pre-post intervention SpO2/FiO2 ratio (250±88.4 vs 275.6±97.5, p<0.001), reaching 354.4±110.2 after 5 days of therapy (p<0.001 with respect to baseline). Mean baseline respiratory, heart rate and level of dyspnea measure by the Borg scale did not change during the technique performance. In patient maneuvers with FiO2>0.4, the SaO2/FiO2 ratio increase was higher than in patients with milder severity (46.85 ± 77.69, p<0.01). Conclusions: Chest physiotherapy with a 10 cmH2O threshold valve is a safe and tolerated intervention with short-term improvement in oxygenation in patients with COVID-19 pneumonia.
