BACKGROUND: To present a new method of urethral pressures examination, and to evaluate diagnostic capabilities of three dimensional profilometry , as an alternative to classical urethral profile (UPP) .Using four channel catheters and dedicated software global urethral pressures image is obtained. The method eliminates the main limitation of classical urethral profilometry where the catheter orientation determines the pressures picture limited to only one point in the uretral circumference while we observed even 50% differences in pressure measures depending on the point of measurement. METHODS: This is a preliminary study containing method presentation and analisys of the use in varied clinical cases of either healthy patients and patients with LUTS. The article includes technique and equipment decription, full evaluation of selected cases, including three dimensional urethral pressures distribution graphics. RESULTS AND CONCLUSIONS:Three dimensional profilometry compared to classical technique is comparable regarding the time, cost, technical difficulty and patient discomfort. At the same time we obtain much more data on the urethral pressures and its distribution.The results are easy for interpretation due to 3D movable graphics created automatically through the dedicated software.

The aim of this study was to evaluate the usefulness of cystoscopic injection of Botulinum-A toxin (BTX) in the detrusor wall in the treatment of children with decreased bladder capacity due to neurogenic bladder. The prospective, randomized non placebo controlled trial is conducted in our institution since year 2006 with the approval of the local Ethics Committee. 556 cystoscopic injections of BTX were performed in 141 children with neurogenic bladder in age 1 to 18 years. In all cases decreased bladder capacity and bladder overactivity with urine incontinence were estimated. The pre-and post-treatment evaluations included determination of urinary continence status, bladder function in frequency/volume chart of catheterized urine and in urodynamic studies. Parameters measured in urodynamic investigations included maximal cystometric capacity, detrusor reflex volume, maximal detrusor pressure. Parameters were analyzed before the cystoscopy and during the follow-up examinations in 5 age groups. Values of all measured parameters improved significantly and equally after therapy in every from 5 age groups. The results obtained from the study confirmed that endoscopic administration of BTX improves function of urinary bladder in children with neurogenic bladder, and the method represents an alternative approach to conservative treatment and surgical augmentation.

Lower urinary tract dysfunction is clinically important because it may cause urinary tract infections, mainly due to accumulation of residual urine, and adversely affect renal function. In addition, it may cause urinary incontinence, strongly affecting the child’s quality of life. The function of the lower urinary tract is closely associated with function of the bowel because constipation is commonly present with bladder dysfunction. The interplay between the lower urinary tract and bowel function, coupled with common conditions such as detrusor overactivity and voiding dysfunction, requires a nuanced diagnostic approach. Detrusor overactivity, a benign but socially harmful condition, is the principal cause of daytime urinary incontinence in childhood. It needs to be differentiated from more serious conditions such as neurogenic bladder dysfunction or urethral obstruction. Voiding dysfunction, a habitual sphincter contraction during voiding, is common in children with detrusor overactivity, may be selflimiting but may also result in residual urine and urinary tract infections. It may resemble, in severe cases, the neurogenic bladder dysfunction, most often caused by spinal dysraphism which very often leads to recurrent urinary tract infections and high intravesical pressures, jeopardizing renal function. Voiding diary is crucial in the initial evaluation of lower urinary tract function in children.
