Very few studies, often with very small cohortes, tended to prove chemotherapy efficiency against canine agressive mammary carcinomas, either on the metastasis or on the median survival of dogs after surgery and chemo, and this was not confirmed by other studies. As a result, we lack standardized efficient protocols that exist in human cases according to their grade and stage. In this case report of a relapsing grade III solid mammary carcinoma evolving into prominent lymphatic intravascular invasion with mutifocal nodal extension (stage 4), we applied an intensive treatment combining high dose surgery and intensive adjuvant chemotherapy. This adjuvant chemo combined a carboplatin maximal-tolerated-dose chemotherapy, with doses adjusted as necessary and a metronomic chemotherapy with firocoxib, toceranib and chloraminophen, progressively added together and carefully monitored. Adapting the doses allowed to avoid adverse events and to obtain 237 days of survival with good quality of life. To our knowledge, it is the first description of such a combination of treatment. That result should be confirmed by a prospective large-scale study.

ABSTRACT Lubricants play a crucial role in reducing friction and wear between surfaces in relative motion. Engine oil, as a lubricant, is specifically designed to minimize friction and wear between the moving parts of different equipment and machinery. Engine oil blending serves the purpose of adjusting the variety of lubricants available to meet specific performance requirements. Through blending, it is possible to tailor the characteristics of the lubricating oil, such as viscosity, stability, and additive content, to optimize its effectiveness in reducing friction, protecting engine components, and enhancing overall equipment performance. By customizing lubricant formulations through blending, manufacturers can address the diverse lubrication needs of modern engines and machinery, ensuring smooth operation, longevity, and efficiency.To enhance the quality of the lubricant and achieve the desired high-performance multi-grade engine oil, a systematic approach was followed. The process began with testing the specific gravity of the oil using a hydrometer and thermometer to ensure accurate measurements. Subsequently, 5 liters of base oil were carefully measured and poured into a stainless steel reactor pot to serve as the blending vessel. Following this, 1 liter of paraffin oil (100N) was added to the base oil to introduce specific properties to the blend. To further optimize the formulation, 0.42 kg (7% of the total blend) of additives, including a selection of specific performance-enhancing compounds, was incorporated. These additives, such as Aspen, Tackifier, and other essential components like anti-wear, corrosion inhibitors, detergents, dispersants, and antioxidants, were meticulously measured and added to the mix. Through this precise blending process, the goal was to craft a high-quality multi-grade engine oil that meets the stringent performance requirements for various operating conditions and equipment types. After the melted Aspen and dissolved tackifier were added to the mixture, thorough stirring was carried out to ensure uniform distribution until a homogeneous blend was achieved. The mixture was then exposed to heat to facilitate the reaction between the base oil and additives, reaching a temperature of 70°C for optimal blending. Subsequently, 0.5 kg of viscosity index improver was introduced into the mixture and stirred continuously for a period of 5 minutes to ensure proper integration. Following this step, 5 grams of dye were carefully added to the mixture, and stirring was continued to evenly disperse the coloring agent throughout the blend. This meticulous process aimed to ensure that all components were effectively incorporated, facilitating the production of a high-quality, customized multi-grade engine oil with enhanced performance characteristics and optimal lubricating properties. After subjecting the mixture to a temperature of 70°C, the heat source was removed, allowing the product to gradually cool down to room temperature, which was estimated to be around 30°C. Following this cooling process, the product underwent a filtration step to remove any residual impurities present in the mixture. A sample of the purified product was then extracted for further analysis. Quality control tests were meticulously carried out on the sample to ensure that the product met the rigorous standards set by the Standard Organization of Nigeria (SON). The results obtained from the quality control tests were highly promising, indicating that the product indeed complied with the stringent specifications outlined by the regulatory body. This adherence to quality standards is essential in ensuring the product's reliability and performance in various applications. The successful outcome of the quality control test underscores the meticulous attention to detail and commitment to excellence in the production process. The rigorous quality control procedures implemented in the analysis of the product highlight the commitment to upholding high standards of quality and safety in the manufacturing process. Anemia is a well-known disorder in CKD dogs, but its frequency in AKI dogs has been poorly investigated. The aim of the present study was to retrospectively evaluate the frequency of anemia, its degree of severity, and regeneration rate in relation to IRIS grade, etiology, therapy, and out-come. Medical records of dogs (2017-2023) with historical, laboratory, and ultrasound findings consistent with AKI were retrospectively reviewed. According to etiology, AKI was classified as ischemic/inflammatory (IS), infectious INF), nephrotoxic (NEP), obstructive (OBS), and unknown (UK). AKI dogs were also classified according to therapeutical management (medical vs hemo-dialysis), survival to hospital discharge (survivors vs non-survivors). AKI dogs were considered anemic in case of HCT 38 g/dL). Regeneration rate was considered absent (RET< 60,000/μL), mild 61,000- 150,000/μL), and moderate (> 150,000/μL). A total of 120 AKI dogs were included the study, and anemia was found in 86/120 dogs (72%). The severity of anemia was mild in 32/86 dogs (37%), moderate in 40/86 dogs (47%), severe in 11/86 dogs (13%), and very severe in 3/86 (3%). Anemia was normo-chromic in 71/86 dogs (83%), hyperchromic in 12/86 dogs (14%), and hypochromic in 3/86 dogs (3%). Normocytic anemia was present in 56/86 dogs (65%), microcytic anemia in 27/86 dogs (31%), and macrocytic anemia in 3/86 dogs (4%). Non-regenerative anemia was found in 76/86 dogs (88%). The authors attempt to addresses the importance of timely detection and management of cervical intraepithelial neoplasia (CIN) to prevent cervical cancer. The study focuses on the potential of electrical impedance spectroscopy (EIS) as an adjunct to colposcopy, aiming to enhance the accuracy of identifying high-grade cervical lesions. Colposcopy, a widely used technique, exhibits variable sensitivity in detecting high-grade lesions, which relies on the expertise of the operator. The study's primary objective is to evaluate the effectiveness of combining colposcopy with EIS in detecting high-grade cervical lesions among patients initially diagnosed with low-grade CIN based on cytology. We employ a cross-sectional observational design, recruiting 101 women with abnormal cervical cytology results. The participants undergo colposcopy with acetic acid and subsequent EIS using the ZedScan device. The ZedScan results are categorized into color-coded probability levels, with red indicating the highest likelihood of high-grade squamous intraepithelial lesions (HSIL) occurrence. Results reveal that ZedScan exhibits a sensitivity rate of 89.5% and a specificity rate of 84% for detecting high-grade lesions. Colposcopy, on the other hand, records a sensitivity rate of 85.5% and a specificity rate of 92%. The agreement rate between ZedScan and biopsy is 79.2%, as indicated by a kappa coefficient of 0.71, while the agreement rate between colposcopy and biopsy is 74.3%, with a kappa coefficient of 0.71.

In any forest stand, the theoretically modelled output of sawlogs (the most valuable roundwood assortments) will differ from what is actually obtained. The aim of this study was to assess whether it is possible to characterize this difference by site properties or inventory parameters of forest element for birch, black alder and as-pen. The differences in yield of sawlogs varied among soil types, and was increasing with age and average diameter of the forest element. The theoretical model for assortment grading predicted a lower yield of saw-logs compared to actually obtained yield at a lower age and at a lower average diameter, but overestimated output of sawlogs as age and diameter increase. The results highlight necessity to consider decreasing wood quality with increasing age to account for damage, such as stem rot, in assortment tables.

This study aimed to identify microRNAs associated with histological grade using comprehensive microRNA analysis data obtained by next-generation sequencing in early-stage invasive breast cancer. RNA-seq data from normal breast and breast cancer samples were compared to identify candidate microRNAs with different expressions using bioinformatics. A total of 108 microRNAs were significantly differentially expressed between normal breast and breast cancer tissues. Using clinicopathological information and microRNA sequencing data of 430 patients with breast cancer from The Cancer Genome Atlas (TCGA), the differences in candidate microRNAs between low- and high-grade cases were compared. Comparing the expression of the 108 microRNAs between low- and high-grade cases, 25 and 18 microRNAs were significantly upregulated and downregulated, respectively, in high-grade cases. Clustering analysis of the TCGA cohort using these 43 microRNAs identified two groups strongly predictive of histological grade. miR-3677 is a microRNA upregulated in high-grade breast cancer. The outcome analysis revealed that patients with high miR-3677 expression had a significantly worse prognosis than those with low miR-3677 expression. This study showed that microRNAs were associated with histological grade in early-stage invasive breast cancer. These findings contribute to the elucidation of a new mechanism of breast cancer growth regulated by specific microRNAs.

High-grade gliomas are extremely fatal tumors, marked by severe hypoxia and therapeutic resistance. Autophagy is a cellular degradative process that can be activated by hypoxia, ultimately resulting in tumor advancement and chemo-resistance. Our study aimed to examine the link between autophagy markers expression in low (LGG) and high-grade gliomas (HGG). In 39 glioma cases, we assessed protein expression of autophagy markers LC3B, p62, and DRAM by immunohistochemistry (IHC) and mRNA expression of autophagy genes; PI3K, AKT, m-TOR, PTEN, ULK1, ULK2, UVRAG, Beclin 1 and VPS34 using RT-qPCR. Livak’s method was used to analyze the relative changes in gene expression and multi-variate Anova test and Spearman's correlation tests were performed for statistical significance. LC3B, p62, and DRAM expression were positive in 64.1%, 51.3%, and 28.2% of glioma cases respectively. The expression of LC3B and p62 was notably higher in HGG compared to LGG. VPS34 exhibited a significant differential expression and increased fold change and demonstrated strong positive correlations with Beclin1 (rs=0.768), UVRAG (rs=0.802), and ULK2 (rs=0.786) in HGG compared to LGG, thus highlighting the potential link between autophagy and glioma progression. High-grade advanced ovarian, fallopian tube and primary peritoneal (HGAOC) cancer are malignancies in which, apart from surgical treatment, systemic treatment plays an important role. The improvement in treatment results is associated with the introduction of polyADP-ribose polymerase inhibitors (PARPis). The aim of this study is to present a cross-section of patients treated for HGAOC, in one center, including the distribution of treatment principles following the most up-to-date guidelines. We observed patients who reported to the Department of Gyneacological Oncology between January 2018 and December 2023. We analyzed 437 women with a first diagnosis of HGAOC, who had not been treated before. The introduction of combined treatment with bevacizumab and PARPi into the first line of treatment, regardless of any remnants left during cytoreductive surgery, was introduced in November 2022. Since then, 30% (18/60) of patients (14 with olaparib and 4 with niraparib) have used the above regimen. Previously, in May 2021 - October 2022, it was 27% of patients (13/81 with olaparib and 9/81 with niraparib). Among patients starting treatment in January 2018 - April 2021, only 4.7% (5/106) received the platinum + paclitaxel + olaparib regimen. In both groups – with and without BRCA1/2 pathogenic or likely- pathogenic variant, significantly more patients used PC + PARPi or PC + BEV + PARPi in periods II and III than in period I (p<0.01). The introduction of modern treatment in our center very quickly changed the structure of patient treatment. Carriers of BRCA1/2 pathogenic or likely-pathogenic variant had previously received only chemotherapy, disappeared after the introduction of PARPi, in favor of new therapies. Most patients with no BRCA1/2 pathogenic or likely-pathogenic variant, who used PARPi in therapy in our center – received niraparib and therefore they are beneficiaries of molecular diagnostics in the field of HRD.

Glioblastoma is a highly aggressive brain tumor and is considered to be the most common primary one. Recurrence after treatment is a major problem. The recurrence of GBM is related to different cellular mechanisms and molecular signaling. Therapies to prolong survival are not completely effective. Epigenetics and non-coding genetic elements play a crucial role in gliomagenesis. Non-coding RNAs are novel regulatory RNAs that play key roles in different physiological processes such as gene regulation, cell differentiation, and proliferation. Interestingly, some LncRNAs have a tumor suppressor role while others are oncogenic. In this review article, we are going to illustrate the role of a well-known lncRNA HOX transcript antisense intergenic RNA (HOTAIR) in glioma and highlight the possible functions in glioma in both pre-clinical and clinically evident studies.

This study describes a novel method for grading pathological sections of gliomas. Our own integrated hyperspectral imaging system was employed to characterize 270 bands of cancerous tissue samples from microarray slides of gliomas. These samples were then classified according to the guidelines developed by the World Health Organization, which define the subtypes and grades of diffuse gliomas. We explored a hyperspectral feature extraction model called SMLME-Resnet using microscopic hyperspectral images of brain gliomas of different malignancy grades. The model combines the channel attention mechanism and multi-scale image features to automatically learn the pathological organization of gliomas and obtain hierarchical feature representations, effectively removing the interference of redundant information. It also completes multi-modal, multi-scale spatial and spectral feature extraction to improve the automatic classification of glioma subtypes. The proposed classification method demonstrated high average classification accuracy (>97.3\%) and Kappa coefficient (0.954), indicating its effectiveness in improving the automatic classification of hyperspectral gliomas. The method is readily applicable in a wide range of clinical settings, offering valuable assistance in alleviating the workload of clinical pathologists. Furthermore, the study contributes to the development of more personalized and refined treatment plans, as well as subsequent follow-up and adjustment of treatment, by providing physicians with insights into the underlying pathological organization of gliomas.

Background: Glioma surgery was remarkably renewed in the past 2 decades, with improved safety and limited but yet improved life expectations. Fluorescence-guided resection of high-grade gliomas (HGG) plays a central role in this sense, by allowing higher degrees of extent of resection (EOR). The introduction of the exoscopic guided surgery may be considered an implementation for fluorescence techniques over the traditional microscopes. We present the application and the advantages of the exoscopic guided surgery compared to the microscopic surgery in tumor resection guided by 5-ALA fluorescence in patients with HGG. Methods: ten consecutive patients underwent a surgery for HGG resection. The surgery was performed via an exoscopic-guided procedure (Olympus Orbeye) and after oral administration of Gliolan 5 hours before the procedure. During surgery, the procedure was shifted to microscope (Kinevo 900, Zeiss) view. The intensity of the fluorescence under the two different was subjectively measured in different picture samples during the surgery in a 1 to 5 (from minimum to maximum) scale. The brightness of the surgical field and the detailing of the anatomy were also analyzed comparatively. Results: among the ten patients the histopathological diagnosis was an high grade glioma in all cases. In 9 cases was possible to achieve the gross total resection. There was no perioperative mortality. The median fluorescence intensity, on a scale of 1–5, was 4.5 in the exoscope group and 3.5 in the microscope group (p<0.01). Conclusions: The exoscopic guided surgery adds advantages to the traditional fluorescence- guided surgery with 5-aminolevulinic acid. Beyond the important advantage of low cost and the possibility to perform collaborative surgeries, it adds a plain and continuous visualization of the tumor and the surgical field in the fluorescence guided glioma surgery compared to the microscopic guided one.

Introduction: We investigated the potential effect of smoking on pathological staging in clinically low-risk patients. Material-Methods: Data of 59 patients who were diagnosed with a bladder tumor for the first time and had a single lesion radiologically and endoscopically smaller than 3 cm were investigated, retrospectively. A total of 33 patients who currently smoke or smoked were classified as ever smokers group and 26 patients who did not ever smoke were classified as never smokers group. Pathological diagnoses of the patients in both groups were compared. Results: A total of 9 patients (27.3%) in ever smokers group and 18 patients (69.2%) in never smokers group had Ta disease (p<0.05). Morover, 19 patients (57.6%) in ever smokers group and 5 patients (19.2%) in never smokers group had T1 disease (p<0.05). The number of patients with low grade (LG) tumor were 8 (24.2%) and 19 (73.1%) in ever smoking and never smoking groups, respectively (p<0.05). The number of patients with high grade (HG) tumor were 25 (75.8%) and 7 (26.9%) in ever smoking and never smoking groups, respectively (p<0.05). Ta HG was detected in 9 (27.3%) patients in ever smoking group. In contrast, no patients in never smoking group had Ta HG disease (p<0.05). The number of patients with T1 HG was 17 (51.5%) in ever smoking group and 2 (7.69%) in never smoking group (p<0.05). Conclusion:Smoking seems to associate with pathologically worse stage and grade in patients with primary, single, < 3cm bladder cancer.

AbstractWe investigated the effect of cigarette smoking on pathological staging in clinically low-risk patients. The number of patients with high grade(HG) tumor were 25 (75.8%) and 7 (26.9%) in Group I and in Group II, respectively (p < 0.05). TaHG was detected in 9 (27.3%) patients in Group I. In contrast, no patients in Group II had TaHG disease (p < 0.05). The number of patients with T₁HG was 17(51.5%) patients in Group I and 2 (7.69%) patients in Group II (p < 0.05). Smoking is associated with pathologically HG and stage in patients with first time bladder tumor which is single and smaller than 3 cm.

Low-grade glioma (LGG) is a highly aggressive disease in the skull. On the other hand, anoikis, a specific form of cell death induced by the loss of cell contact with the extracellular matrix, plays a key role in cancer metastasis. In this study，anoikis-related genes (ANRGs) were used to identify LGG subtypes and to construct a prognostic model for LGG patients. In addition, we explored the immune microenvironment and enrichment pathways between different subtypes. We constructed an anoikis-related gene signature using the TCGA cohort and investigated the differences in clinical features, mutational landscape, immune cell infiltration, etc. between different risk groups. Kaplan-Meier analysis showed that the characteristics of ANRGs in the high-risk group were associated with poor prognosis in LGG patients. The risk score was identified as an independent prognostic factor. The high-risk group had higher immune cell infiltration, tumor mutation load, immune checkpoint gene expression, and ICB treatment response. Functional analysis showed that these high- and low-risk groups had different immune statuses and drug sensitivity. Risk scores were used together with LGG clinicopathological features to construct a nomogram, and DCA analysis showed that the model could enable patients to benefit from clinical treatment strategies.

Automatic ancient Roman coin analysis only recently emerged as a topic of computer science research. Nevertheless, owing to its ever-increasing popularity, the field is already reaching a certain degree of maturity, as witnessed by a substantial publication output in the last decade. At the same time, it is becoming evident that research progress is being limited by a somewhat veering direction of effort and the lack of a coherent framework which facilitates the acquisition and dissemination of robust, repeatable, and rigorous evidence. Thus, in the present article, we seek to address several associated challenges. To start with, (i) we provide a first overview and discussion of different challenges in the field, some of which have been scarcely investigated to date, and others which have hitherto been unrecognized and unaddressed. Secondly, (ii) we introduce the first data set, carefully curated and collected for the purpose of facilitating methodological evaluation of algorithms and, specifically, the effects of coin preservation grades on the performance of automatic methods. Indeed, until now, only one published work at all recognized the need for this kind of analysis, which, to any numismatist, would be a trivially obvious fact. We also discuss a wide range of considerations which had to be taken into account in collecting this corpus, explain our decisions, and describe its content in detail. Briefly, the data set comprises 100 different coin issues, all with multiple examples in Fine, Very Fine, and Extremely Fine conditions, giving a total of over 650 different specimens. These correspond to 44 issuing authorities and span the time period of approximately 300 years (from 27 BC until 244 AD). In summary, the present article should be an invaluable resource to researchers in the field, and we encourage the community to adopt the collected corpus, freely available for research purposes, as a standard evaluation benchmark.

Background: Coronavirus disease (COVID-19) related pneumonia is leaded by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), which infects host cells through receptors named Angiotensin-converting enzyme 2 (ACE2). Smoking is thought to be related to poor disease prognosis, as a large amount of evidence highlights the negative effects of smoking on lung health and its causal relationship with various respiratory diseases. Methods: We first evaluated the role of ACE2 expression level with tobacco effect. And correlation analyses were used to identify the related genes of ACE2 both in smoker trait and human normal multi-tissues. After intersections, hub genes were later used to further GSEA and co-regulated mechanisms were explored by GeneMANIA. We used UALCAN to perform survival curves of pan-cancers included 33 types of cancers. New clinical model of top co-occurrence cancer type was constructed and validated. Results: Effected by cigarette smoking, the expression level of ACE2 expressed statistically upwards in current smokers compared with never smokers, upwards in groups after acute smoke exposed compared with normal control. But no strong evidence detected in third-hand smoke, as poor amounts of samples, only 4. A little trend of expressing upwards became in groups after third-hand smoke compared with groups after clean air exposing. 4 genes included PIR, ADH7, AKR1C2 and AKR1C3 were identified as ACE2 related genes in smoker trait and human normal multi-tissues. Then, we made the survival curves of pan-cancers in 4 genes. Brain lower grade glioma was the co-occurrence type as both ACE2, PIR and AKR1C3 had the significantly prognostic situation. Later, we made the new clinical prediction model as the C-index was 0.827 and the Area Under Curves (AUCs) of 1-year survival, 2-year survival and 3-year survival were 0.921(95%CI, 0.882-0.961), 0.911(95%CI, 0.860-0.962) and 0.878(95%CI, 0.818-0.939). In inner validation, the AUCs of 1-year survival, 2-year survival and 3-year survival were 0.777(95%CI, 0.640-0.914), 0.916(95%CI, 0.842-0.990) and 0.888(95%CI, 0.768-1.000). In outer validation one, the AUCs of 1-year survival, 2-year survival and 3-year survival were 0.764(95%CI, 0.672-0.856), 0.848(95%CI, 0.783-0.913) and 0.748(95%CI, 0.656-0.841). And in outer validation two the AUCs of 1-year survival, 2-year survival and 3-year survival were 0.740(95%CI, 0.654-0.826), 0.766(95%CI, 0.688-0.843) and 0.794(95%CI, 0.721-0.866). Conclusions: In our study, we compared the ACE2 expression level between smokers and non-smokers crowds and identified associated hub genes. Then we explored the further role of hub genes in tumor fields to identify the correlation and influence between the avoidable host factors such as smoking on COVID-19 contamination and tumor. ACE2 and AKR1C3 are potential prognostic genes for brain lower grade glioma, and we created the web dynamic nomogram and encapsulation app through validations.

Beginning with the discovery of the BRCA ovarian cancer susceptibility genes and subsequent detailed examination of risk reduction salpingo-oophorectomy (RRSO) specimens, a new paradigm of ovarian carcinogenesis has unfolded with attention to the distal fallopian tube. The primary focus has been an early cancer in the fallopian tube which is seen in virtually all incidentally discovered high-grade serous cancers in asymptomatic women. This high-frequency of tubal involvement in early serous cancer - serous tubal intraepithelial carcinoma or STIC - has galvanized attention to this organ as a primary source of this disease. However, an enduring mystery has been the relatively low frequency of STIC in fallopian tubes of women with advanced malignancy. This paradox, a high-frequency of tubal involvement early and a low-frequency late in the disease process has spurred interest in other potential sources, such as the ovarian surface or secondary Mullerian system. However, because essentially all high-grade serous carcinomas are linked by TP53 mutations, and because fallopian tubes frequently contain early serous proliferations (ESPs) with these mutations, attention has turned to the possibility that nonmalignant but TP53 mutated tubal epithelium could be responsible for an eventual malignancy. Recent data have shown evidence of lineage continuity between ESPs and concurrent serous carcinomas prompting the concept of "precursor escape". This creates a 2nd component of the paradigm by which cells from early lesions can escape the fallopian tube and undergo future malignant transformation later, emerging suddenly as widespread malignancy. This dualistic model thus explains the paradox and opens new questions pertaining to the challenge of both early detection and prevention of this lethal malignancy.

Background Gliomas are the most prevalent primary malignant brain tumors worldwide. Recent studies highlight the potential of long non-coding RNAs (lncRNAs) in tumor progression, with LINC01354 being upregulated in various malignancies. This study investigates the role of LINC01354 in glioma prognosis to provide novel insights for early prognosis prediction.Methods This study involved four cohorts of low-grade glioma tissue samples collected from hospitals in Xinjiang, Beijing, and Guangzhou, China. Quantitative Real-Time Polymerase Chain Reaction (qRT-PCR) was used to measure LINC01354 expression levels. Clinical information and survival data were analyzed using Cox regression and ROC analysis to evaluate the association between LINC01354 expression and overall survival. Clinical comparisons were conducted to identify potential discrepancies in clinical parameters across different cohorts.Results Elevated LINC01354 levels were associated with poorer overall survival in the West China cohort (Xinjiang) but not in the North (Beijing) or South China (Guangzhou) cohorts. Clinical parameter comparisons revealed no significant differences among the cohorts that could explain the regional discrepancy. Further analysis within the West China cohort indicated that LINC01354's prognostic value was consistent across Han and non-Han ethnic groups and unaffected by dietary staples. However, LINC01354's prognostic significance was more pronounced in patients born in Xinjiang compared to those born elsewhere. A prognostic model incorporating LINC01354 expression and birth location demonstrated good predictive accuracy with ROC AUC values of 0.777 in the training cohort and 0.757 in the validation cohort.Conclusion LINC01354 is a potential prognostic biomarker for low-grade glioma in the West China population, particularly among patients born in Xinjiang.

The airflow carried by artificial aeration provides oxidant O2 for microbial growth and mineral dissolution in heap bioleaching. To investigate the effect of forced aeration rate on bioleaching of low-grade copper sulfide ores, column bioleaching experiments with six columns were conducted in the aeration range of 0~150 L/h at 30~45 ℃. The results showed that when the aeration rate was higher than 95 L/h, the proportions of heap settlement and fine particles mass were much greater than that without aeration. The final total Fe concentration and bacterial cell numbers in pregnant leach solution increased with the increase of aeration rate.The final concentrations of Cu2 + in the mother liquor under aeration conditions of 60-150 L / h were 1.411, 1.432, 1.498, 1.491 and 1.583 g / L, respectively, which were higher than 1.345 g / L under non-aeration conditions. When the aeration rate was 150 L / h, the final Cu leaching rate was 80.1 %, which was 11.4 % higher than that without aeration ( 68.7 % ). Furthermore, in the early stage of leaching, copper leaching rate with 95 L/h aeration rate remained the highest. 结论Artificial aeration accelerates the bioleaching of low-grade copper sulfide ores by improving porosity structure, temperature range of bacterial activity and microbial catalyzed dissolution of targeted minerals.

Background: A prognostic index validated for the outcomes of advanced high-grade serous ovarian cancer (HGSOC) patients with neoadjuvant chemotherapy (NACT) is still lacking. We therefore developed an ovarian neoadjuvant chemotherapy prognostic index (ONCPI) to enhance predictive accuracy. Methods: We analyzed clinicopathologic feature of advanced HGSOC patients receiving platinum-based NACT. Blood inflammatory composite markers were calculated and binary-transformed by optimal cutoffs. The omental hematoxylin and eosin (H&E) stained slides were selected for the assessment of chemotherapy response score (CRS). Logistic regression analyses and Cox proportional hazard regression model were utilized to develop a prognostic index. Results: Multivariate analysis showed that CRS and neutrophils-to-lymphocyte ratio (NLR) are independent risk factors for platinum-chemotherapy response. Meanwhile, Kaplan–Meier and Cox regression analysis revealed that CRS score was significantly correlated with PFS and OS, and NLR-high patients was associated with poor OS. We further developed an ONCPI model based on the CRS score and NLR level. Survival analysis suggested that patients with score 0 and 1 of ONCPI were significantly associated with improved PFS and OS. Conclusions: The ONCPI score emerges as a significant prognostic marker for predicting NACT outcomes in advanced HGSOC patients. Its integration into clinical practice and risk-stratified trial design is conceivable.

Chondrosarcoma, represents approximately 0.1% of all neoplasms of the head and neck, being considered a rare disease, but with a relatively good prognosis. The 5-year overall survival (OS) rate is estimated at 70-80%, being considered a disease with a low growth rate. About 13% of all cases of chondrosarcoma, are located in the region of the head and neck. We present the case of a young patient treated multimodally, lost from the oncological records less than two years after the diagnosis, without evidence of disease progression. The early loss from the oncological follow-up programs of patients with a lower risk of early repalpse, without evidence of a recurrence or metastasis could be the consequence of a major complication among which we could assume an aspiration pneumonia secondary to a dysphagia, associated with an aggressive multidisciplinary treatment. Large tumor size and positive resection margins (R1 resection) are risk factors that support an intensive adjuvant approach in order to reduce the risk of recurrence, but the low grade of tumor, associating a lower risk of recurrence as well as the adverse events (AE) of adjuvant radiotherapy and chemotherapy justifies a more reserved therapeutic approach. Taking into account the longer life expectancy of these patients, it is recommended to use a more conformal irradiation technique in order to reduce doses to radiosensitive structures as well as to omit elective neck irradiation, taking into account the lower risk of lymph node involvement. The lack of guidelines that include very rare tumors including low grade chondrosarcoma of head and neck make a unified approach difficult, but the data presented even in case reports could contribute to choosing the regimen that offers the best therapeutic ratio.

Previous studies show that women with a history of high-grade CIN have a greater risk of subsequent high-grade anogenital intraepithelial neoplasia or cancer, and the risk is greater for vaginal cancer than for anal and vulvar cancers. It is hypothesized that the laparoscopic hysterectomy procedure may cause a higher incidence of VaIN in hysterectomized women. There are few studies addressing this issue and they show mixed results. This study aimed to investigate the incidence of high-grade or worse VaIN in the population of women undergoing hysterectomy for CIN3 or benign uterine disease, illustrate treatment options and follow-up. Methods: This retrospective study was conducted on 170 women who underwent laparoscopic hysterectomy for high-grade cervical intraepithelial neoplasia (CIN3) or benign gynecological disease. The follow-up strategy included performing a cotest and colposcopy with biopsy if necessary. The median time between primary treatment and diagnosis of high-grade VaIN was 18 months. Results: High grade or worse VaIN was found in 8 patients after hysterectomy (4.7%). All cases of high-grade VaIN occurred in women with persistent HPV infection. The most frequent genotype was 16. Women hysterectomized for CIN3 showed an 8-fold greater risk than women hysterectomized for benign disease of developing high-grade VaIN. The risk of VaIN is low in women hysterectomized for benign disease. The risk of developing VaIN is greater in women with viral persistence Conclusion. All these elements suggest that it is the history of HPV-related disease of the lower genital tract and viral persistence, rather than hysterectomy itself, that should be considered a risk factor for the development of high-grade VaIN. After hysterectomy, patients with a history of CIN should undergo annual screening with vaginal dome cytology and HPV testing.

Gastric cancer is among the top 5 causes of cancer-related death worldwide. Preoperative chemotherapy has been established as an option in patients with locally advanced gastric cancer. However, chemotherapy yields variable results, owing to the cellular and molecular heterogeneity of this disease. Identifying patients who did or did not respond to preoperative therapy can allow clinicians to alter treatment modalities and provide important information related to prognostication. Pathologic response to preoperative therapies, called Tumor Response Grade (TRG), has been evaluated to quantify treatment response. Multiple systems for TRG have been established. However, literature has demonstrated inconsistent results for TGR systems and prognosis, possibly due to variability in interpretation of tumor response between systems and interobserver variability. Radiographic response to preoperative therapies using RECIST 1.1 criteria and endoscopically-assessed tumor response have demonstrated association with survival; however their use in gastric cancer remains challenging given the inability to accurately and consistently identify and measure the tumor, especially in the setting of neoadjuvant therapy where treatment-related changes can obscure the gastric wall layers. This review is focused on summarizing the available literature related to evaluating TRG in gastric cancer, as well as providing a brief overview on the use of radiographic and endoscopic methods to assess response to preoperative therapies. Lastly, we outline future directions regarding the use of a universal TRG system to guide care and assist with prognosis.

The paradigm of the Somatic Mutation Theory (SMT) is failing and a new paradigm is in the making but not yet established. What is being challenged is a conceptual approach that involves the entire human biology and the development of chronic diseases. The behavior of breast cancer is well compatible with the concept that the primary tumor is able to control its microscopic metastases, in the same way that an organ (e.g., the liver) is able to control its physiological size. This finding suggested that breast cancer and its metastases may behave as an organoid. The new paradigm under construction considers the origin of tumors as a disturbance in the communication network between tissue cell populations and between cells and extracellular matrix, and supports a systemic approach to the study of both healthy and pathologic tissues. The commentary provides a rationale for the role of physical exercise in the control of tumor dormancy according to a human evolutionary perspective.

Despite the surge in the research of cannabis chemistry and its biological and medical activity, only a few cannabis-based pharmaceutical-grade drugs have been developed and marketed to date. Not many of these drugs are Food and Drug Administration (FDA)-approved and some are still going through regulation processes. Active compounds including cannabinergic compounds (i.e., molecules targeted to modulate the endocannabinoid system) or analogs of phytocannabinoids (cannabinoids produced by the plant) may be developed into single-molecule drugs. However, since in many cases treatment with whole plant extract is preferred over treatment with a single purified molecule, some more recently developed cannabis-derived drugs contain several molecules. Different combinations of active plant ingredients (API) from cannabis with proven synergy may be identified and developed as drugs to treat different medical conditions. However, possible negative effects between cannabis compounds should also be considered, as well as the effect of the cannabis treatment on the endocannabinoid system. FDA registration of single, few or multiple molecules as drugs is a challenging process and certain considerations that should be reviewed in this process, including issues of drug-drug interactions, are also discussed here.

The present study analyses the influence of scratches and fatigue on the mechanical characteristics of aluminium alloys commonly used in the aviation industry: 2024-T4, 6061-T4, 6061-T4 uncoated, 6061-T6 uncoated, 7075-T0, and 7076-T6. 120 specimens were used for the study. Out of these, 60 samples were scratched, and the other 60 samples were also scratched under the same circumstances and then subjected to low-cycle fatigue (LCF) before undergoing the tensile test. Based on the statistical examination of the measured data, mathematical prediction models have been established. Compared to their unscratched counterparts, the results indicate a significant decrease in the mechanical properties of both scratched and scratched-fatigued sample sets. The LCF pre-treatment contributes to the negative impacts of scratches, resulting in a reduced value for the ultimate tensile strength and elongation at break. The results conclude that surface integrity plays an essential role in preserving aircraft components' structural strength.

Wagyu beef is gaining worldwide popularity, primarily due to the fineness of its marbling. Cur-rently, the evaluation of this marbling is performed visually by graders. This method has several issues: varying evaluation standards among graders, reduced accuracy due to long working hours and external factors causing fatigue, and fluctuations in grading standards due to the grader's mood at the time. This paper proposes the use of fractal analysis for the grading evaluation of beef marbling to achieve automatic grading without the inconsistencies caused by human factors. In the experiments, cross-sectional images of the parts used for visual judgment were taken, and fractal analysis was performed on these images to evaluate them using fractal dimensions. The results confirmed a correlation between the marbling evaluation and the fractal dimensions, demonstrating that quantitative evaluation can be achieved, moving away from qualitative visual assessments.

Obesity is associated with chronic low-grade inflammation. Polyunsaturated fatty ac-ids such as omega-3 (n-3), are essential in the pro-resolution of inflammation. An 8-week double-blind randomized clinical trial was conducted with a n-6:n-3, 5:1 ratio, along with daily 1.8 g of n-3 supplementation vs. placebo. A total of 40 subjects were included, 21 in omega 3 group and 19 in placebo group. The n-3 supplementation had a superior effect compared to placebo increasing levels of Resolvin D1 [129.3 (-90.1 - 193.5) vs. -16.8 (-237.8 - 92.5) pg/mL, p=0.041], IL-10 [1.4 (-0.7 - 4.6) vs. -2.0 (-5 - 0.05) pg/mL, p=0.001], and decreasing IL-6 [-0.67 (2.72 - -0.59) vs. 0.03 (-0.59 - 1.84) pg/mL p=0.015], and MCP-1 [-29.6 (-94.9 - 5.50) vs. 18.3 (-97.3 - 66.35) pg/mL, p=0.040]. In con-clusion, n-3 supplementation promoted resolution of inflammation by increasing sys-temic levels of Resolvin D1, IL-10, and decreasing IL-6 and MCP-1.

In this paper, a low-grade copper oxide ore with highly alkaline was treated. The thermodynamic model of CuO-(NH4)2SO4-NH3-H2O was established in the leaching system of tenorite (CuO) under the condition of double conservation of mass and charge. The fitting function of MATLAB and the diff and solve functions were used to calculate the optimal ammonia concentration and total copper ion concentration of tenorite under different ammonium sulfate concentrations. The effects of different ammonia-ammonium salt (ammonium sulphate, ammonium carbonate, ammonium chloride) solution on copper leaching rate were investigated. Results show that under the conditions of ammonia concentration of 1.2 mol/L, ammonia-ammonium ratio of 2:1 and liquid-solid ratio of 3:1, the copper leaching rate from the ammonium sulphate and ammonium chloride solution reaches 70%, slightly higher than that of ammonium carbonate. Therefore, ammonia ammonium sulfate system is selected to leach low-grade copper oxide due to its lower corrosion to the equipment than the chlorination system.

Ovarian cancer (OC) is an umbrella term for cancerous malignancies affecting the ovaries, yet treatment options for all subtypes are predominantly derived from high-grade serous ovarian cancer, the largest subgroup. The concept of "functional precision medicine" involves gaining personalized insights on therapy choice, based on direct exposure of patient tissues to drugs. This especially holds promise for rare subtypes like low-grade serous ovarian cancer (LGSOC). This study aims to establish an in vivo model for LGSOC using zebrafish embryos, comparing treatment responses previously observed in mouse PDX models, cell lines, and 3D tumor models. To address this goal, a well characterized patient-derived LGSOC cell line with the KRAS mutation c.35 G > T (p.(Gly12Val)) was used. Fluorescently labelled tumor cells were injected into the perivitelline space of 2 days post fertilization zebrafish embryos. At 1 day post-injection xenografts were as-sessed for tumor size, followed by random allocation into treatment groups with trametinib, luminespib, and trametinib + luminespib. Subsequently xenografts were euthanized and analysed for apoptosis and proliferation by confocal microscopy. Tumor cells formed compact tumor masses (n=84) in vivo and with clear Ki67 staining, indicating proliferation. Zebrafish xenografts exhibited sensitivity to trametinib and luminespib, individually or combined, within a two-week period, establishing them as a rapid and complementary tool to existing in vitro and in vivo models for evaluating targeted therapies in LGSOC.

Nickel is widely used in industrial processes and plays a crucial role in many applications. However, most of the nickel resource mainly exists as nickel oxide in laterite nickel ore with complex composition, resulting in difficulty in upgrading the nickel content using physical separation methods. In this study, high-grade ferronickel concentrates were obtained through a carbothermal reduction and magnetic separation using laterite nickel ore and anthracite as raw materials. The effects of different types of additives (CaF2, Na2SO4 and H3BO3), carbon ratio (nc/no in metal oxides) and grinding time on the recoveries and grades of ferronickel concentrates were experimentally investigated, along with the microstructure and chemical composition of the products. CaF2 was proved to be the primary active additive in the aggregation and growth of the ferronickel particles, and the improvement of the grade of the product. Under the optimal conditions of CaF2 addition of 12 wt.%, carbon ratio of 1.4, and grinding time of 240 s, high-grade magnetically separable ferronickel concentrate with nickel grade 8.93 wt.% and iron grade 63.96 wt.% was successfully prepared. This work presents a practical method for the highly efficient recovery and utilization of iron and nickel from low-grade laterite nickel ore, contributing to the development of strategies for the sustainable extraction and utilization of nickel resources.

: In the automotive industry, high-strength plates have been increasingly used to reduce the vehicle body weight due to stringent regulations on fuel economy and safety. Such plates achieve tensile strengths as high as 1.5 GPa owing to the hot-stamping process. However, these plates suffer from scale generation and surface decarburization because of the surface oxidation of the material. Recently, studies on materials subjected to high-frequency induction heating have reported strength improvement owing to the coil shape and cooling method. This study presents the surface temperature changes, mechanical properties, and microstructures of boron steels subjected to longitudinal heating, which uses eddy currents generated from high-frequency induction heating. The surface temperature data were analyzed under different high-frequency induction heating powers (15, 18, 21, 24, 27, and 30 kW) and distances from the specimen (6, 9, and 15 mm). The mechanical properties and microstructures were also analyzed in different high-frequency induction heating coil directions. The correlations between the high-frequency induction heating power and the distance from the specimen with the maximum tensile strength were determined.

Asphalt binder is a temperature sensitive material, whose performance is greatly affected by the climate change of the environment. Improper selection of asphalt will cause a lot of damage and affect the durability of the road. The establishment of asphalt pavement performance zoning in Xinjiang, a vast area with great temperature difference, will provide reference for the selection of asphalt suitability, the refinement of pavement design and the sustainable development of road engineering. In this study, 11 years of temperature data in Xinjiang region were collected and analyzed, and 98% reliability of pavement design temperature was used to draw a performance grading map of asphalt pavement in Xinjiang region based on ArcGIS platform. Finally, Xinjiang region was divided into 9 performance zones. At the same time, the performance grade (PG) of five kinds of asphalt binders in Karamay were explored. The result shows that there is little difference in continuous PG span between different matrix asphalt binders, the lower penetration grade, the better high temperature performance, the worse low temperature performance. After adding SBS modifier, the continuous PG span can be higher than the matrix asphalt in 20 ℃. All these provide the basis for reasonable selection of asphalt binders in different areas of Xinjiang.

Due to the accuracy of numerical calculation of fluid flow inside a hydrocyclone can be obtained using Computational Fluid Dynamics (CFD), highly modified super computers are used to simulate the fluid flow and track particle motion inside a hydrocyclone. This paper deals with the numerical study using three multiphase models viz. Volume of fluid, Mixture and Eulerian model. The dimensions of the hydrocyclone taken into consideration for numerical analysis is same as considered by Rajamani. Validation of axial and tangential velocities at different strategically decided axial stations, RMS axial and tangential velocity profiles of the hydrocyclone is done using Reynolds Stress Model (RSM). The hydrocyclone model has been designed in Creo 3.0 using the same dimensions which later was imported to CFD for meshing. Fine hexagonal mesh numbering up to 5 lacs were constructed to obtain optimum results. Fluid flow was allowed to be developed in ANSYS FLUENT 16.2. Entire simulation took 96 hours to generate results and track particle movements inside the hydrocyclone. The particle tracking has been done using three multiphase model. The first being the volume of fluid was used for validation purposes and the comparison of the Mixture and Eulerian model are the basic focus of this research work. Conclusive results indicate that usage of different multiphase model does not result in variation in particle motion. The slight variation in grade efficiency values are hardly noticeable. The Mixture model and Eulerian model predict lower separation efficiency as compared with Volume of fluid multiphase model.

High-grade gliomas present a significant challenge in neuro-oncology because of their aggressive nature and resistance to current therapies. Boron neutron capture therapy (BNCT) is a potential treatment method; however, the boron carrier compounds used — such as 4-borono-L-phenylalanine (L-BPA) — have limitations. This study evaluated the use of boron-conjugated 4-iodophenylbutanamide (BC-IP), a novel boron compound in BNCT, for the treatment of glioma. Using in vitro drug exposure experiments and in vivo studies, we compared BC-IP and BPA, with a focus on boron uptake and retention characteristics. The results showed that although BC-IP had a lower boron uptake than BPA, it exhibited superior retention. Furthermore, despite lower boron accumulation in tumors, BC-IP showed significant survival improvement in glioma-bearing rats compared to controls. These results suggest that BC-IP, with its unique properties, may be an alternative boron carrier for BNCT. Further research is required to optimize this potential treatment modality, which could significantly contribute to advancing the treatment of high-grade gliomas.

The possibility of using additive manufacturing (AM) in the medicine area has created a new opportunities in health care. This has contributed to a sharp increase in demand for 3D printers, their systems and materials that are adapted to strict medical requirements. We described herein a medical-grade thermoplastic polyurethane (S-TPU), which was developed and then formed into a filament for Fused Deposition Modeling (FDM) 3D printers during a melt-extrusion process. S-TPU consisting of aliphatic hexamethylene 1,6-diisocyanate (HDI), amorphous α,ω-dihydroxy(ethylene-butylene adipate) (PEBA) and 1,4 butandiol (BDO) as a chain extender, was synthesized without the use of a catalyst. The filament properties were characterized by rheological, mechanical, physico-chemical and in vitro biological properties. The tests showed biocompatibility of the obtained filament as well as revealed no significant effect of the filament formation process on its properties. This study may contribute to expanding the range of medical-grade flexible filaments for standard low-budget FDM printers.

Low-grade endotoxemia by lipopolysaccharide (LPS) has been detected in COVID-19 and could favour thrombosis via eliciting a pro-inflammatory and pro-coagulant state. The aim of the study was to analyse the mechanism accounting for low-grade endotoxemia and its relationship with oxidative stress and clotting activation thrombosis in COVID-19. We measured serum levels of sNOX2-dp, zonulin, LPS, D-dimer and albumin in 175 patients with COVID-19, classified as having or not acute respiratory distress syndrome (ARDS), and 50 healthy subjects. Baseline levels of sNOX2-dp, LPS, zonulin, D-dimer, albumin and hs-CRP were significantly higher in COVID-19 compared to controls. In COVID-19 patients with ARDS, sNOX2-dp, LPS, zonulin, D-dimer and hs-CRP were significantly higher compared to COVID-19 patients without ARDS. Conversely, concentration of albumin was lower in patients with compared with those without ARDS and inversely associated with LPS. In COVID-19 cohort, the number of patients with ARDS progressively increased according to sNOX2-dp and LPS quartiles; a significant correlation between LPS and sNOX2-dp, and LPS and D-Dimer was detected in COVID-19. In a multivariable logistic regression model, LPS/Albumin levels and D-dimer predicted thrombotic events. In COVID-19 patients, LPS is significantly associated with a hypercoagulation state and disease severity. In vitro, LPS can increase endothelial oxidative stress and coagulation biomarkers, that were reduced by the treatment with albumin. In conclusion, impaired gut barrier permeability, increased NOX2 activation and low serum albumin may account for low-grade endotoxemia, and they may be implicated in thrombotic events in COVID-19.

This review explores the nuanced decision-making surrounding postoperative radiotherapy for diffuse low-grade glioma (DLGG) patients, emphasizing the need for personalized treatment to balance the benefits of tumor control with the risks of long-term side effects. Radiotherapy is a cornerstone of DLGG treatment, offering significant improvements in progression-free survival, but its potential for cognitive decline and vascular lesions necessitates careful consideration of individual patient factors. The review delves into the complexities of radiotherapy timing, revealing that while early radiotherapy enhances progression-free survival, it does not significantly impact overall survival and may lead to earlier onset of cognitive decline. Conversely, delayed radiotherapy may mitigate cognitive decline but may result in shorter progression-free survival and the onset of neurological symptoms. The review advocates for a patient-centered approach, emphasizing the importance of weighing individual risks and benefits, and highlights the potential of artificial intelligence in predicting cognitive decline and the need to revisit previous research findings in light of the 2021 WHO (World Health Organization) classification update, underscoring the evolving nature of DLGG treatment and the ongoing quest for optimal therapeutic strategies.

In the early stage of new vehicle development, the requirements of maximum performance of the air conditioning (AC) system are obtained through Amesim software simulation, but the calibration control of the AC system is usually carried out in the prototype-build vehicle test stage, and the test time is spanned up to 10 months after spring (autumn), summer and winter, of which up to 2 months are for environment chamber tests. Due to the long calibration cycle of AC control in the road test, which seriously affects the vehicle launch time, virtual calibration technology of air conditioning system is developed to shorten the calibration cycle of AC systems and ensure the quality of vehicles. In the project development stage, the calibration control of the vehicle AC system can be realized to carry out the functional test of the AC system in advance. In that, the grade of maturity for the AC system control can be improved, the on-road calibration test cycle can be shortened, and the development cost can be saved.

An algorithm is presented for a micro-model of a mining dump truck movement, which allows determining the optimal speed and acceleration of a vehicle, considering the change of the road grade. The structure of the simulation macro-model of an excavator-and-dump truck complex has been developed. The procedure for integrating a micro-model of a dump truck movement into a simulation model is shown. As a result of a series of experiments with a macro-model, the influence of open pit roads grade on the performance and efficiency of excavator-and-dump truck complexes was established. The necessity of considering in simulation excavator-and-dump truck complexes the speed regulation of dump trucks depending on the change in road grade is shown.

Accurately distinguishing between indolent and aggressive tumors is a crucial unmet need in the clinical management of prostate cancer (PCa). The traditional Gleason grading system has been utilized for this purpose; however, there is often ambiguity in classifying tumors with a Gleason grade of 7. Clinicians commonly resort to using secondary Gleason grades, such as 3+4 or 4+3, to classify these tumors as indolent or aggressive, respectively. Unfortunately, such classifications are prone to misinterpretation, leading to erroneous diagnoses and prognoses. To address this challenge, we investigated the application of Machine Learning (ML) techniques to classify PCa patients based on gene expression data sourced from The Cancer Genome Atlas. By comparing gene expression levels between indolent and aggressive tumors, we sought to identify distinctive features for developing and validating a range of ML algorithms and stacking techniques. The stacking based model achieved an impressive accuracy of 96% for all samples encompassing primary Gleason grades 6 to 10. Notably, when excluding Gleason grade 7 from the analysis, the accuracy further improved to 97%. This study underscores the effectiveness of the stacked ML algorithm for accurately classifying indolent versus aggressive PCa. Leveraging gene expression data and employing a combination of classifiers, this approach offers a powerful solution to address the unmet need in robustly distinguishing between different types of PCa tumors. Future implementation of this methodology may significantly impact clinical decision-making and patient outcomes in the management of prostate cancer.

Based on the daily observation data of 2254 meteorological stations in China from 1961 to 2021 after homogenization and correction, the extreme value analysis method that combines relative threshold and absolute threshold was used to identify extreme climate events in the mainland of China, namely extreme high temperature, extreme low temperature, extreme drought, extreme precipitation and extreme typhoon events. The frequency and linear variation trends of individual extreme events in the past 60 years were investigated. On this basis, the multi extreme events composite risk grade index (MXCI) was constructed, which can objectively reflect the comprehensive characteristics of multi extreme climate events in different regions. The results show that the high value areas of MXCI were mainly located in Southeast China and Southwest China. The MXCI presented a decreasing trend in most of the western and southeastern regions of China, while an increasing trend from Southwest China to the south of Northeast China, due to the frequent occurrence of extreme high temperature, extreme drought and other extreme events in these regions.

Purpose: This study investigates the impact of different intensity normalization (IN) methods on the overall survival (OS) radiomics models’ performance of MR sequences in primary (pHGG) and recurrent high-grade glioma (rHGG). Methods: MR scans acquired before radiotherapy were retrieved from two independent cohorts (rHGG C1: 197, pHGG C2: 141) from multiple scanners (15, 14). The sequences are T1 weighted (w), contrast-enhanced T1w (T1wce), T2w, and T2w-FLAIR. Sequence-specific significant features (SF) associated with OS, extracted from the tumour volume, were derived after applying 15 different IN methods. Survival analyses were conducted using Cox proportional hazard (CPH) and Poisson regression (POI) models. A ranking score was assigned based on the 10-fold cross-validated (CV) concordance index (C-I), mean square error (MSE), and the Akaike information criterion (AICs), to evaluate the methods’ performance. Results: Scatter plots of the 10-CV C-I and MSE against the AIC showed an impact on the survival predictions between the IN methods and MR sequences (C1/C2 C-I range: 0.62-0.71/0.61-0.72, MSE range: 0.20-0.42/0.13-0.22). White stripe showed stable results for T1wce (C1/C2 C-I: 0.71/0.65, MSE: 0.21/0.14). Combat (0.68/0.62, 0.22/0.15) and histogram matching (HM, 0.67/0.64, 0.22/0.15) showed consistent prediction results for T2w models. They were also the top-performing methods for T1w in C2 (Combat: 0.67, 0.13; HM: 0.67, 0.13), however, only HM achieved high predictions in C1 (0.66, 0.22). After eliminating IN impacted SF using Spearman’s rank-order correlation coefficient, a mean decrease in the C-I and MSE of 0.05 and 0.03 was observed in all four sequences. Conclusion: The IN method impacted the predictive power of survival models. Thus, performance is sequence-dependent.

Economic expedience of waste heat recovery systems (WHRS), especially for low temperature difference applications, is often questionable due to high capital investments and long pay-back periods. By its simple design isobaric expansion (IE) machines could provide a viable pathway to utilize otherwise unprofitable waste heat streams for power generation and particularly for pumping liquids and compression of gases. Different engine configurations are presented and discussed. A new method of modelling and calculation of the IE process and efficiency is used on IE cycles with various pure and mixtures as a working fluid. Some interesting cases are presented. It is shown in this paper, that the simplest non-regenerative IE engines are efficient at low temperature differences between a heat source and heat sink. Efficiency of non-regenerative IE process with pure working fluid can be very high approaching Carnot efficiency at low pressure and heat source/heat sink temperature differences. Regeneration permits to increase efficiency of the IE-cycle to some extent. Application of mixed working fluids in combination with regeneration permits to significantly increase the range of high efficiencies to much larger temperature and pressure differences.

The Amoebic Self Theory is a concept of the social psychology, which postulates that humans have a psychological boundary. As the authors of the concept propose [1], the function of the boundary is to allow psychological separation of one from the others. In this study, we examined how sen-sitivity to violation of the boundary, measured by an amoebic self scale, is connected with differ-ent types of procrastination sensu lato, measured by seven procrastination subscales. Only two of the seven procrastination aspects, i.e. the preference for pressure and outcome satisfaction, were negatively and significantly related to the spatial-symbolic domain of the amoebic self scale. The other purpose of this research was to examine the connection between the students’ grade point average (GPA) and scores obtained in the procrastination subscales. Only the non-adaptive aspect of procrastination predicted significantly the GPA. That is an important detail, because pointing out the gap between one’s self-opinion and the real, non dependent of the opinion, academic achievement. All these findings were considered in the academic context and consequences of these results were discussed.

Although sarcopenia is characterized by a loss of muscle strength and skeletal muscle mass, few studies have evaluated the effect of muscle strength on hepatocellular carcinoma (HCC) patients. We separately evaluated the impact of sarcopenia-related factors (grip strength [GS] and the skeletal muscle index [SMI]) on the survival among lenvatinib-treated unresectable HCC (u-HCC) patients. This single-center cohort study was conducted at a university hospital. The study population included 63 lenvatinib-treated u-HCC patients managed between April 2018 and April 2020. A decreased GS and decreased SMI were found in 21 (33.3%) and 22 (34.9%) patients, respectively. The overall survival (OS) of the normal GS group was significantly higher than that of the decreased GS group, while that of the normal and decreased SMI groups did not differ markedly. There were no significant differences in the progression-free survival between the normal GS and decreased GS groups or the normal SMI and decreased SMI groups. A multivariate Cox proportional hazards model showed that ALBI2b (hazard ratio [HR] 4.39) and a decreased GS (HR 3.55) were independently associated with an increased risk of poor prognosis. In addition to the hepatic functional reserve, a decreased GS was a poor prognostic factor in lenvatinib-treated u-HCC patients.

The origin and significance of the tonalite–trondhjemite–granodiorite (TTG) units and the familiar metabasite xenoliths they host in the Yangtze Craton, China, remain controversial, and resolving these issues is important if we are to understand the evolution of the early Yangtze Craton. We focused on biotite–tremolite schist xenoliths in the Archean TTG units of the Kongling high-grade metamorphic terrane, and U–Pb dating of their zircons yielded ²⁰⁷Pb/²⁰⁶Pb ages of ca. 3.00 Ga, which provides a minimum age for the formation of the pre-metamorphic basic igneous rock. The host TTGs and late intrusive granitic dikes yield three groups of upper intercept ages at 2.87–2.88, 2.91–2.94, and 3.07 Ga, and a concordant age at 2.94 Ga, which suggest that the Yangtze continental nucleus underwent three important metamorphic–magmatic events in the Mesoarchean at ca. 3.00, 2.94, and 2.87 Ga. The biotite–tremolite schists have high ratios of K₂O/Na₂O and high contents of CaO, Cr, and Ni, thus showing the characteristics of high-K calc-alkaline island-arc volcanic rocks (basalt–andesite) that form by the partial melting of subducted oceanic crust. The data also provide further proof that a Mesoarchean metamorphic basement exists in the Yangtze Plate. Derivation of the magmatic protoliths of the biotite–tremolite schist enclaves from an oceanic crust during slab subduction, and the presence of these xenoliths within the TTG suite, indicate the existence of the initiation of plate tectonics during the Mesoarchean (≤2.94 Ga).

Background: Despite recent advances in epithelial ovarian carcinoma (EOC) treatment, recur-rence and mortality rates have not improved significantly. DNA hypermethylation has generally been associated with ominous prognosis and chemotherapy resistance, but the role of DNMTs in EOC remains to be investigated. Methods: In the current study, we systematically retrieved gene expression data from patients with EOC and studied the immunohistochemical expression of DNMTs in 108 primary and 26 relapsed tumors. Results: Our results showed that DNMT1, DNMT3A, DNMT3B and DNMT3L RNA levels were higher and DNMT2 lower in tumors compared to non-neoplastic tissue, and DNMT3A and DNMT2 expression decreased from Stage II to Stage IV carcinomas. Proteomic data also suggested that DNMT1 and DNMT3A levels were increased in tumors. Similarly, DNMT1, DNMT3A and DNMT3L protein levels were overex-pressed and DNMT2 expression was reduced in high grade carcinomas compared to non-neoplastic tissue and low-grade tumors. Moreover, DNMT1 and DNMT3L were increased in relapsed tumors compared to their primaries. DNMT3A, DNMT1 and DNMT3B mRNA lev-els were correlated with overall survival. Conclusions: Our study demonstrates that DNMT1 and DNMT3L are upregulated in primary high grade EOC and further increase in relapses, whereas DNMT3A is upregulated only in the earlier stages of cancer progression. DNMT2 downregulation highlights a presumptive tumor-suppressor activity of this gene in ovarian car-cinoma.

A right frontoparietal neoplastic lesion was found in a young-aged patient with analgesic refractory headache. Surgical resection was performed, and the pathologist analysis turned out a cellular ependymoma with signs of anaplasia. In the follow-up, a MRI showed a suspicious lesion, so a [18F]Fluorocholine PET/CT was performed. An increased uptake was described in the right parietal region on the margin of residual cystic lesion. The patient got a complete resection which was confirmed later by MR. In the pathology analysis, a focally congestive cerebral parenchyma with a central histiocytic reaction to foreign body area was described. Foreign body reaction in brain tissue is a very rare immune response that has not been well studied. Haemostatic material has been reported as a possible trigger of this response in other organs and could be detected by [18F]FDG PET/CT. Following the experience of the current case report, [18F]Fluorocholine PET/CT could also show a false positive related to foreign body reaction.

Although the relations between sleep and academic performance have been extensively examined, how sleep predicts future academic performance (e.g., 2 -3 years) remains to be further investigated. Using wearable smartwatches and a self-report questionnaire, we tracked sleep activities of 45 college students over a period of approximate half a month to see whether their sleep activities predicted their academic performance, which was estimated by grade point average (GPA). Results showed that both nighttime sleep awakening frequency and its consistency in the tracking period were not significantly correlated with the GPA for the courses taken in the semester during sleep tracking (current GPA). However, both nighttime sleep awakening frequency and its consistency inversely predicted the GPA for the rest of the courses taken after that semester (future GPA). Moreover, students with more difficulty staying awake throughout the day obtained lower current and future GPAs, and students with lower inconsistency of sleep quality obtained lower future GPA. Together, these findings highlight the importance of nighttime sleep awakening frequency and consistency in predicting future academic performance and emphasize the necessity of assessing the consistency of sleep measures in future studies.

The meshless Petrov-Galerkin (MLPG) method is developed to analyse 2-D problems for flexoelectricity and thermoelectricity. Both problems are multiphysical and scale dependent. The size-effect is considered by the strain- and electric field-gradients in the flexoelecricity and higher-grade heat flux in the thermoelectricity. The variational principle is applied to de-rive the governing equations considered constitutive equations. The order of derivatives in governing equations is higher than in equations obtained from classical theory. The coupled governing partial differential equations (PDE) are satisfied in a local weak-form on small fic-titious subdomains with a simple test function. Physical fields are approximated by the mov-ing least-squares (MLS) scheme. Applying the spatial approximations in local integral equa-tions a system of algebraic is obtained for the nodal unknowns.

In the environments of all kinds open coal storage sites , mining-affected coalbeds, and goafs, parts of coal body at low moisture content (≤8%) are prone to spontaneous combustion under the influence of some external environmental factors. In order to examine the influence effect of low moisture content on the spontaneous combustion tendency of coals with different metamorphic grade, we conducted temperature programmed oxidation (TPO) experiment and differential scanning calorimetry (DSC) experiment to study the spontaneous combustion characteristics of coals with different metamorphic grade at four different low moisture content, and comparatively analyzed the change laws of the characteristic parameters of four different metamorphic grade coals at four different low moisture content. The experimental results indicate that: 1) Compared other low moisture content, anthracite and fat coal at low moisture content of 1.2% show stronger tendency to undergo spontaneous combustion, long flame coal and lignite at low moisture content of 3.4% and 5.6% are more prone to spontaneous combustion. 2) Four different metamorphic grade coals at low moisture content of 7.8% are less prone to spontaneous combustion; 3) Coals with different metamorphic grade have different tendency to undergo spontaneous combustion.

This paper addresses the mixed convective flow and heat transfer in combustible third grade fluid through a slant channel that is filled with a permeable materials. The fluid layer in contact with the lower plate is exposed to convective heating with the ambient while at the upper plate, a constant heat flux is experienced. We employ the spectral quasilinearisation method (SQLM) to the coupled nonlinear flow governing equations. Fluid velocity and temperature profiles, local entropy generation and irreversibility ratio are computed and analysed quantitatively and qualitatively. A residual error analysis demonstrated high accuracy and convergence of the numerical method. The results on flow and thermal effects, entropy generation rate and Bejan number revealed fascinating manifestations that have profound implications in design of thermo-mechanical systems. In particular, the results of the thermal analysis are pertinent to optimal designs of systems that achieve efficient energy utilization.

Glucocorticoid administration is a common clinical practice in attempt to decrease inflammation associated with, and improve resectability of, canine mast cell tumors (MCTs). However, the impact of neoadjuvant glucocorticoids on histological features and proliferation indices of canine MCTs is unknown. The objective of this study was to evaluate changes in tumor grade, mitotic count, Ki67, AgNOR, and AgNORxKi67 scores following short-course, anti-inflammatory neoadjuvant prednisone in canine patients with MCTs. This was a prospective, single-arm pilot study. Client-owned dogs with treatment-naïve, cytologically-confirmed MCTs were enrolled. Patients underwent an initial incisional biopsy followed by a 10-14 day course of anti-inflammatory prednisone, and surgical resection. All histological samples were randomized, masked, and evaluated by a single pathologist. Unstained paired pre- and post-treatment samples were submitted to a commercial laboratory for Ki67 and AgNOR immunohistochemical analysis. There were 11 dogs enrolled with 11 tumors. There were no statistical differences between the pre- and post-treatment histological parameters of mitotic index, Ki67, AgNOR, or Ki67xAgNOR. There were no clinically-significant alterations between pre-treatment and post-treatment in the assignment of tumor grades. A short-course of anti-inflammatory prednisone does not appear to alter the histological parameters that affect grade-determination, nor significantly alter the proliferation indices in canine MCTs.

The Sanaga River Valley is a deep-seated structure of about 220 km in width, bounded on its northern and southern sides by 025-0550 fault zones. Its internal structure has been very little described before this study. It is characterized by the presence of surface features where a thick stratum of shale and argillaceous sandstones have been documented. The use of electrical resistivity tomography for near-surface imaging and hydrogeological prospecting within shale succession is widespread. However, several methods of spatial modeling of resistivity contours are generally proposed for the interpretation of inverse resistivity model sections. To determine the most appropriate method for interpreting resistivity tomographies in the context of subsoil marked by significant heterogeneity, logarithmic contour intervals, and linear contour interval processing were implemented on data collected over five areas. The data consists of outcrops data, cuttings samples from groundwater boreholes, and resistivity sections ranging in length from 240 to 1000 m. Well-control of the inverse resistivity model sections derived from both processes shows that the linear contour intervals modeling gives a more realistic picture of the reservoir size of the wa-ter-bearing sand in both shale and argillaceous sandstone succession. The sedimentary architecture over Sanaga fault breccia is marked by vertical intrusion anomalies up to 80 m high. The shape of these anomalies, as well as the very low resistivity values that characterize them, suggests that they could be related to mud intrusion.

Consuming fish sustainably means consuming less and better, considering environmental, social, and economic impacts. This demands more knowledge to make informed decisions. In this study, the main goal was to understand how sustainability education can be used as a tool to raise awareness and promote new habits regarding the sustainable consumption of fish in 4th-grade students at primary school. During the COVID-19 pandemic, face-to-face and online learning methodologies were used in two different groups of students to understand whether the teaching method impacted learning. A sequential explanatory mixed methodology was applied, with a pre-test/post-test design, combined with focus group sessions. Between the pre and post-tests three learning modules were performed. The results showed that the students were unaware of many aspects related to fish sustainable consumption, but the developed activities significantly increased their knowledge of the subject and impacted students to change their consumption behaviours. It was also found that the construction of knowledge by the students after the activities was achieved either by face-to-face or online learning. Interviews from focus group sessions showed that students got to know new fish species, learned about the concept of minimum landing size and closure season and that fish are labelled like other types of food.

Introduction: OS2966 is a fist-in-class, humanized and de-immunized monoclonal antibody which targets the adhesion receptor subunit, CD29/β1 integrin. CD29 expression is highly upregulated in glioblastoma and has been shown to drive tumor progression, invasion, and resistance to multiple modalities of therapy. Here, we present a novel Phase I clinical trial design addressing several factors plaguing effective treatment of high-grade gliomas (HGG). Study Design: This 2-part, ascending-dose, Phase I clinical trial will enroll patients with recurrent/progressive HGG requiring a clinically-indicated resection. In Study Part 1, patients will undergo stereotactic tumor biopsy followed by placement of a purpose-built catheter which will be used for intratumoral, convection-enhanced delivery (CED) of OS2966. Subsequently, patients will undergo their clinically-indicated tumor resection followed by CED of OS2966 to the surrounding tumor-infiltrated brain. Matched pre- and post-infusion tumor specimens will be utilized for biomarker development and validation of target engagement by receptor occupancy. Dose escalation will be achieved using a unique concentration-based accelerated titration design. Discussion: The present study design leverages multiple innovations including: 1) the latest CED technology, 2) 2-part design including neoadjuvant intratumoral administration, 3) a first-in-class investigational therapeutic, and 4) concentration-based dosing. Trial registration: A U.S. Food and Drug Administration (FDA) Investigational New Drug application (IND) for the above protocol is now active. A Phase I trial is registered at clinicialtrials.gov (NCT04608812).

This work studied the corrosion of welded pipes and how welding destroyed surface film of pipes. Surface reaction of a welded pipe is key to understanding phenomena and important factors during the corrosion. This paper presents experiment and CFD modeling approaches of a welded pipe corrosion under salt vapor condition. The pipes were welded at currents of 60 A,70 A and 80 A to observe the effect of welding current on corrosion. A welded pipe is a stainless-steel ASTM A312 grade 304L and period of experiment about 0-600 hours that they are tested in vertical and horizontal alignments. In CFD software, there is not direct model of corrosion but it can use surface reaction and create add-on species and chemical reaction technique for imitating the corrosion mechanism. The modeling approaches of corrosion have presented in 3-dimensional transient times in CFD simulation. Surface reactions were performed by Species Model which involve site species. Site species in Species Model took place at gas-solid interfaces and in this case are salt vapor and surface pipe. Chemical reaction rate on the surface controls lost weight of a welded pipe and the model can be validated with experiment. In conclusion, in period 0-600 hours error between CFD modeling and experiment have error trend decreased. The error at 600 hours is 6% both of vertical pipe and horizontal pipe test. The modeling approaches closely with the performed experiment and can be accepted. Moreover, the model is able to predict corrosion of a welded pipe of different sizes and their lost weight after 600 hours without experiment. Also the model can predict lifetime of pipe.

The performance of the presurgical test (preSurg) for temporal lobe epilepsy (TLE) was evaluated via video-electroencephalography (VEEG), electroencephalography (EEG), 99mTc-HmPAO single-photon emission tomography (SPECT) and 1.5 T magnetic resonance imaging (MRI) in a group of 112 men (37.0 ± 1.1 years) and 106 women (39.7 ± 1.1 years) operated on for TLE. The epileptic zone (EZ) was adequately identified to determine whether the patient reached an Engel I grade (EI) at least one year postop. Accuracy was evaluated by the coefficient α, ranging from 3 (when result = EZ) to 2 (result in the same hemisphere as the EZ), 1 (noninformative result) or 0 (EZ in the contralateral hemisphere). The simplicity of diagnosis was defined as the number of preSurg surgeries needed to identify the EZ. EI was obtained in 85.8% of patients even though 42.2% of patients had noninformative MRI results. For preSurg α was (mean ± SEM) = VEEG (2.83 ± 0.04) &gt; MRI (2.14 ± 0.08) &gt; EEG (1.87 ± 0.09) &gt; SPECT (1.71 ± 0.09). The accuracy (combination of sensitivity and specificity) was calculated as follows: VEEG (0.797) &gt; MRI (0.518) &gt; EEG (0.446) &gt; SPECT (0.360). The likelihood positive ratio was greater, and the likelihood negative ratio was lower for VEEG. The most relevant factor for the regression model was VEEG (3.106) &gt; MRI (2.558) &gt; EEG (1.905), and the factors were not significantly related to SPECT. EZs in EI patients with low simplicity were identified mainly by VEEG. A very good postoperative outcome can be obtained even in TLE patients with no lesions on MRI. The VEEG is the most reliable preSurg test and may be the only reliable test for patients with very low simplicity (i.e., difficult).single paragraph of about 200 words maximum. Every day around 93% of children under the age of 15 (1.8 billion children) breathe outdoor air that is so polluted it puts their health and development at serious risk. Due to the pandemic, however, ventilation of buildings using outdoor air has become an important safety technique to prevent the spread of COVID-19. With the mounting ev-idence suggesting that air pollution is impactful to human health and educational out-comes, this contradictory guidance may be problematic in schools with higher air pol-lution levels, but keeping kids COVID-19 free and in school to receive their education is now more pressing than ever. To understand if all schools in an urban area are ex-posed to similar outdoor air quality and if school infrastructure protects children equally indoors, we installed research grade sensors to observe PM2.5 concentrations in indoor and outdoor settings to understand how unequal exposure to indoor and out-door air pollution impacts indoor air quality among high- and low-income schools in Salt Lake City, Utah. Based on this approach, we found that during atmospheric inver-sions and dust events, there was a lag ranging between 35 to 73 minutes for the out-door PM2.5 concentrations to follow a similar temporal pattern as the indoor PM2.5. This lag has policy and health implications and may help to explain the rising concerns re-garding reduced educational outcomes related to air pollution in urban areas. These data and resulting analysis show that poor air quality may impact school settings, and the potential implications with respect to environmental inequality.

Development of effective treatments for high-grade glioma (HGG) is hampered by 1) the blood-brain barrier (BBB), 2) an infiltrative growth pattern, 3) rapid development of therapeutic resistance, and, in many cases, 4) dose-limiting toxicity due to systemic exposure. Convec-tion-enhanced delivery (CED) has the potential to significantly limit systemic toxicity and in-crease therapeutic index by directly delivering homogenous drug concentrations to the site of disease. In this review, we present clinical experiences and preclinical developments of CED in the setting of high-grade gliomas.

As new treatment modalities are being explored in neuro-oncology, viruses are emerging as a promising class of therapeutics. Virotherapy consists of introduction of either wild-type or engineered viruses to the site of disease, where they exert anti-tumor effect. These viruses can either be non-lytic, in which case they are used to deliver gene therapy, or lytic, which induce tumor cell lysis and subsequent host immunologic response. Replication-competent viruses can then go on to further infect and lyse neighboring glioma cells. This treatment paradigm is being explored extensively in both preclinical and clinical studies for a variety of indications. Virus-based therapies are advantageous due to the natural susceptibility of glioma cells to viral infection, which improves therapeutic selectivity. Furthermore, lytic viruses expose glioma antigens to the host immune system and subsequently stimulate an immune response that specifically targets tumor cells. This review surveys the current landscape of oncolytic virotherapy clinical trials in high-grade glioma, summarizes preclinical experiences, identifies challenges associated with this modality across multiple trials, and highlights potential to integrate this therapeutic strategy into promising combinatory approaches.

This article provides a literature review on tubal carcinoma to offer an updated insight into its preventative strategies, diagnosis, treatment, and oncological surveillance. In addition to the search string utilized, the authors' focus extended to key scientific studies, consensus statements, guidelines, and relevant case reports essential for the proper clinical management of the disease, providing a methodologically well-structured literature review combined with the practical expertise in the oncological field. The article also includes two emblematic clinical cases that emphasize the importance of understanding the physiopathology and the current state of the art in the anatomopathological advancements of tubal/ovarian/peritoneal carcinoma, often assimilated into a single clinical entity.

A regional modern river-sand geochemical mapping study in the majority of the Timor-Leste territory, including the research area, has not been previously reported. This study demonstrated the potential of modern river sand geochemical mapping as an effective method for lithological mapping, particularly in areas that face challenges in conducting efficient geological fieldwork and have limited geological information. A total of 53 modern river sand samples were collected and analyzed. Before examining the samples using wavelength-dispersive X-ray fluorescence for ten major elements, the loss-on-ignition method was employed. In the Clerec drainage basin, SiO2, CaO, and K2O showed positive correlations, and CaO was positively correlated with MnO. Similarly, in the Sahe River catchment, a positive relationship was observed between CaO, MnO, and TiO2. These findings suggested the presence of quartz, manganese, secondary K-bearing minerals, and secondary Ti-bearing minerals in carbonate rocks that have undergone silicification and other alteration processes. The principal component analysis results corroborated the SiO2/Al2O3 and K2O/Na2O ratios and the ICV findings. PC1 was related to the variations in Ca-poor and Ca-rich rocks; PC2 indicated notable inputs from SiO2-rich and SiO2-poor rocks; and PC3 revealed the possibility of contributions from secondary mineral assemblages related to silicification and low-grade metamorphic processes.

Background: This practice guideline was developed by the Chronic Kidney Disease (CKD) Task Force, which was composed of clinical and methodological experts. The Saudi Arabian Ministry of Health and its Health Holding Company commissioned this guideline project to support the realization of the Vision 2030's healthcare transformation pillar. Methods: The guideline synthesis was guided by the GRADE-ADOLOPMENT methodology. Results: The final adoloped guideline addressed 12 clinical questions on blood pressure management in patients with CKD through a set of recommendations and performance measures. The recommendations included antihypertensive agents in children, non-renin angiotensin system inhibition (Non-RASi) vs RASi in adults, intensive vs standard blood pressure targets, early vs late assessment for kidney replacement therapy (KRT), late vs early preparation strategy for KRT, CKD symptoms during assessment for KRT or conservative management, initiation of KRT in patients with deteriorating CKD, choice of KRT modality or conservative management in certain CKD patient groups, changing or discontinuing KRT modalities, review frequency for KRT or conservative management, and Information, education and support. Conclusions: These conditional recommendations were based on low to very low certainty of evidence that highlights the need for high-quality randomized trials comparing different antihypertensive agents in patients with CKD.

Patients with pT1 high-grade (HG) bladder cancer (BC) and a very high risk of progression might benefit from immediate radical cystectomy (RC), but this option remains controversial. Validation of a standardized method to evaluate the extent of lamina propria (LP) invasion (with recognized prognostic value) in transurethral resections (TURBT) specimens is still needed. The Rete Oncologica Lombarda (ROL) system showed a high predictive value for progression after TURBT in recent retrospective studies. Our aim was to validate ROL system on a large mono-institutional prospective series of primary urothelial carcinomas. From 2016 to 2020, we adopted ROL for all patients with pT1 HG BC on TURBT. We employed a 1.0-mm threshold to stratify tumors in ROL1 and ROL2. A total of 222 pT1HGBC were analyzed. Median age was 74 years, with male predominance (73.8%). ROL was feasible in all cases: 91 cases were ROL1 (41%) and 131 ROL2 (59%). At a median follow up of 26.9 months (IQR 13.8-40.6), we registered 80 recurrences and 40 progressions. ROL was a significant predictor of tumor progression at both univariable (HR 3.53; CI 95% 1.56 – 7.99; p&lt;0.01) and multivariable (HR 2.90; CI 95% 1.25 – 6.75; p=0.01) Cox regression analyses. At Kaplan-Meier estimates, ROL showed correlation with both PFS (p=0.0012) and RFS (p=0.0167). Our results confirmed the strong predictive value of ROL for progression on a large prospective series. We encourage the application of ROL for reporting the extent of LP invasion, substaging T1 HG BC, and improving risk tables for urological decision making.

The tryptophan (TRP)-kynurenine (KYN) metabolic pathway is a main player of TRP metabolism through which more than 95% of TRP is catabolized. The pathway is activated by acute and chronic immune responses leading to a wide range of illnesses including cancer, immune diseases, neurodegenerative diseases, and psychiatric disorders. The TRP-KYN pathway synthesizes multifarious metabolites including oxidants, antioxidants, neurotoxins, neuroprotectants, and immunomodulators. The immunomodulators are known to facilitate the immune system towards a tolerogenic state, resulting in chronic low-grade inflammation (LGI) that is commonly present in obesity, poor nutrition, exposer to chemicals or allergens, prodromal stage of various illnesses, and chronic diseases. KYN, kynurenic acid, xanthurenic acid, and cinnabarinic acid are aryl hydrocarbon receptor ligands that serve as immunomodulators. Furthermore, TRP-KYN pathway enzymes are known to be activated by the stress hormone cortisol and inflammatory cytokines, and genotypic variants were observed to contribute to inflammation and thus various diseases. The tryptophan 2,3-dioxygenase, the indoleamine 2, 3-oxygenases, and the kynurenine-3-monooxygenase are main enzymes in the pathway. This review article discusses the TRP-KYN pathway with special emphasis on its interaction with the immune system and the tolerogenic shift towards chronic LGI and overviews the major symptoms, pro- and anti-inflammatory cytokines, and toxic and protective KYNs to explore the linkage between chronic LGI, KYNs, and major psychiatric, including depressive disorder, bipolar disorder, substance use disorder, post-traumatic stress disorder, schizophrenia, and autism spectrum disorder.

We report a case of colloid carcinoma (CC) arising from an intestinal-type intraductal papillary mucinous neoplasm with high-grade dysplasia (IPMNHGD) of the pancreas, diagnosed with serial pancreatic juice aspiration cytological examination (SPACE). A rapidly growing intraductal papillary mucinous neoplasm (IPMN) in a 71-year-old Japanese man accelerated his hospitalization in our institute. Clinically, a large, ruptured pancreatic cyst was suspected. Cytologically, several mucin-positive signet-ring cells were scattered in the inflammatory, necrotic, or mucinous background. Signet-ring cells in cell block specimens were immunoreactive for MUC2, MUC5AC, maspin, S100P, and claudin-18. The final cytologic diagnosis was CC arising in an intestinal-type IPMNHGD with intraperitoneal penetration. The patient died two months postoperatively. The cytologic diagnosis was achieved through SPACE, and the presence of signet-ring cells was characteristic. Anti-claudin-18.2-specific monoclonal antibody therapy will likely be used to treat patients with IPMNHGD in the future. To our knowledge, this is the first report in English on mucin-positive signet-ring cells of CC arising in an intestinal-type IPMNHGD evaluated by SPACE cytology.
