Hypertrophic obstructive cardiomyopathy (HOCM) is an autosomal dominant disorder leading to left ventricular outflow tract obstruction (LVOTO). It can present with chest pain, syncope, breathlessness, or in some cases sudden cardiac death. Primarily, it is diagnosed based on echocardiographic findings but cardiac computed tomography (CT) or cardiac magnetic resonance imaging (MRI) can be helpful in selected cases. In this case report, we discuss a case of a young-aged female patient previously diagnosed as HOCM and presented with chest pain, shortness of breath, and palpitations. Her echocardiography revealed severe asymmetrically hypertrophied left ventricle (LV) with normal function and systolic anterior motion of the mitral valve was present and a subvalvular aortic membrane was also seen. The computed tomography (CT) was also performed showing severe asymmetrical hypertrophied and thickened trileaflet tricommissural aortic valve with no calcification or significant valvular aortic stenosis but there was a subaortic membrane (concentric only sparing anteriorly). The presence of subaortic membrane with HOCM is a rare finding and it can be a diagnostic challenge and untreated cases are susceptible to progressive heart failure and worsening of the symptoms by further increasing left ventricular outflow tract obstruction (LVOTO). A thorough investigation and planning before surgical intervention is required to achieve optimal results.

In this study, the unsteady Reynolds-averaged Navier-Stokes (URANS) equations are employed in conjunction with the Menter Shear Stress Transport (SST)-Scale-Adaptive Simulation (SAS) turbulence model in compressible flow, unstructured mesh and complex geometry. Whereas, other scale-resolving approaches in space and time, such as Direct Numerical Simulation (DNS) and Large-Eddy Simulation (LES), supply more comprehensive information about the turbulent energy spectra of the fluctuating component of the flow; however, previously mentioned avenues imply computationally intensive situations, usually performed over structured meshes and relatively simply geometries. In contrast, the SAS approach is designed according to “physically” prescribed length scales of the flow. More precisely, it operates by locally comparing the length scale of the modelled turbulence to the von Karman length scale (which is prescribed according to local fluid velocity derivatives and depends on the grid point distribution in the computational domain). This length scale ratio allows the flow to dynamically adjust the local eddy viscosity in order to better capture the large scale motions (LSM) in unsteady regions of URANS simulations. While SAS may be constrained to model only low flow frequencies or waivenumbers (i.e., LSM), its versatility and computational low-cost make it attractive to obtain a quick first insight of the flow physics; particularly, in those situations dominated by strong flow unsteadiness. The selected numerical application is the well-known M219 three-dimensional rectangular acoustic cavity from the literature at two different freestream Mach numbers, M∞ (0.85 and 1.35) and a length-to-depth ratio of 5:1. Thus, we consider the “deep configuration" in experiments by Henshaw. The SST-SAS model has demonstrated a satisfactory compromise between simplicity, accuracy and flow physics description.

The output of non-ferrous metal smelters is 450,000 tons (3.1% of world output), and copper ranks 10th in the world for health impact evaluation. According to the latest information from smelters, the overall average concentration in 2020 was 8.2 ppb each of SO2, NO2, O3, PM10, and PM2.5. 7.5 ppb, 30.8 ppb, 21.3 μg/m3, and 13.2 μg/m3, and the maximum concentrations were 390 ppb, 50 ppb, 167 ppb, 148 μg/m3, and 107 μg/m3. Art Corporation, A2C (Atmosphere to CFD) model was used to investigate the resident health follow-up management (cohort study) study (SAS Enterprise Guide) as the basis for the source of diffuse pollution in the area. In this paper, corticosteroid–β-cyclodextrin (β-CD) inclusion complexes have been prepared by Supercritical AntiSolvent (SAS) precipitation to enhance the dissolution rate of dexamethasone (DEX) and prednisolone (PRED), which are poorly water-soluble drugs. The processing of the active principles in the absence of a carrier led to the almost complete extraction of the active principle (the small amount of obtained material precipitates in the form of crystals). The coprecipitation of the ingredients in the presence of β-CD was investigated at different concentrations, pressures, and molar ratios. For both the corticosteroids, the optimized operating conditions were 40 °C, 120 bar, an equimolar ratio, and a concentration in DMSO of 20 mg/mL; these conditions led to the attainment of microparticles with a mean diameter equal to 0.197±0.180 μm and 0.131±0.070 μm in the case of DEX and PRED, respectively. The Job’s method confirmed the formation of inclusion complexes in correspondence with the 1/1 mol/mol ratio. Compared to the pure ingredients, the obtained powders have an improved release rate, which is about 3 times faster in both cases.
