In contrast to its traditional form, in which large, powerful companies were solely and exclusively responsible for supplies, manufacturing, and distribution, the supply chain has undergone substantial change over the past half-century. Today, nearly all supply chain processes are outsourced, thanks to the efforts of innovative, modern companies. The degree of risk associated with contemporary supply chains is greater than the majority of supply chain managers acknowledge, according to an accumulation of studies, analysts, and news outlets. Vulnerabilities within the supply chain remain a significant challenge for numerous organizations. Inadequate management of these risks—which include, but are not limited to, natural disasters, acts of terrorism, cyber assaults, the credit crisis, pandemic scenarios, and war—could lead to significant declines in profitability, productivity, revenue, competitive advantage, and other metrics. Concerns regarding the risk assessment of the supply chain remain unresolved. The objective of this article is to present a risk evaluation framework that can be effectively implemented in the assessment of supply chain risks. This study makes a fundamental contribution to the body of knowledge by providing supply chain managers with a practical instrument for assessing their processes, irrespective of the mathematical underpinnings or the diverse variables employed in risk assessment. A case study from an automotive EMS production is used to compare the outcomes of multiple aggregation methods; the conclusions are then validated by risk and FMEA specialists from the same factory.

This paper proposes a model that combines multi-criteria and multi-faceted risk assessment. Using two data sources, a fuzzy Technique for Order of Preference by Similarity to Ideal Solution FTOPSIS method combined with FMEA assessment. The FMEA method presented in this paper combines the technique of prioritising preferences according to FTOPSIS similarity to the ideal solution and conviction structure to overcome the defects of traditional FMEA indicators. The paper describes the exact process and tool selection, and the results obtained in the study verified accurate data. Finally, a numerical case study was presented on hospital reorganization services and case adaptation to prioritize surgical abandonment in Poland. The problem considered in the publication is the priority of patients’ operations in hospitals. The selection of relevant criteria, their importance and patient preferences are presented. The results obtained from the method provide a viable action plan for the proposed research problem. The proposed method is multi-faceted and can be part of an information system supporting reorganization, restructuring, and modification of the operational process.

The wind industry is looking for ways to accurately predict the reliability and availability of newly installed wind turbines. Failure modes, effects and criticality analysis (FMECA) is a technique utilized for determining the critical subsystems of wind turbines. There are several studies which applied FMECA for wind turbines in the literature, but no studies so far have considered different weather conditions or climatic regions. Furthermore, various design types of wind turbines have been analyzed applying FMECA but no study so far has applied FMECA to compare the reliability of geared and direct-drive wind turbines. We propose to fill these gaps by using Koppen-Geiger climatic regions and two different turbine models of direct-drive and geared-drive concepts. A case study is applied on German wind farms utilizing the WMEP database which contains wind turbine failure data from 1989 to 2008. This proposed methodology increases the accuracy of reliability and availability predictions and compares different wind turbine design types and eliminates underestimation of impacts of different weather conditions.

This study introduces an integrated method for managing process risks in a Business Process Reengineering (BPR) project, using Robust Data Envelopment Analysis (RDEA) and machine learning (ML). The goal is to prioritize risks based on three standard factors of PFMEA: Severity, Occurrence, and Detection (S-O-D), and incorporating two additional factors (Breakdown Cost and Breakdown Duration) seen as undesirable outputs. The model also accounts for the effect of uncertainty on expert-estimated values by applying disturbance percentages in the linear PFMEA-RDEA model. A machine learning model is proposed to predict new values if partial or total modifications have been made to the processes. The approach was implemented in an au-tomotive sector company, and the results showed the impact of uncertainty on values by com-paring different approaches such as RPN, PFMEA-DEA, and PFMEA-RDEA. A new reduced risk categorization was achieved, who allowed for decision-makers to focus on necessary actions for reengineering.

The Failure Mode and Effect Analysis (FMEA) is often used to improve a system's reli-ability. This paper proposes a new approach that aims to overcome the most critical defects of the traditional FMEA. This new methodology combines the Entropy and Best Worst Method (BWM) methodology with the EDas and System Dynamics, FMECA: The EN-B-ED Dynamic FMECA. The main innovation’s point of the proposed work is the presence of an unknown factor (Cost), that allows to obtain an objective weighted factor, a risk index when a machine failure occurs. The criticality analysis has been carried out using software (Vensim PLE x64) to simulate System Dynamics models to identify corrective actions and evaluating the possible implementation of these ac-tions. The methodology proposed is applied to a case study in a relevant Italian com-pany in the agri-food sector.

This study investigated the breakdown trend in an automated production with an aim to recommend the application of Reliability-centered maintenance (RCM) for an improved productivity via a new preventive maintenance (PM) program. Individual Section-forming machine (ISM): a glass blowing machine for making glass bottles was used as the case study of an automated production system. The machine parts and the working mechanisms were analysed with a special focus on methods of processes and procedures that will enable the ISM maintenance department run more effective and achieve its essential goal of ensuring effective machine operation and reducing machine downtime. In this work, information is provided on the steps and procedures to identify critical components of the ISM using Failure Modes and Effect Analysis (FMEA) as a tool to develop an optimal maintenance program based on the reliability data of the equipment’s functional components. A relationship was established between the failure rate of the machine components and the maintenance costs such that using the recommended PM program, an evidence of improvement in the machine’s availability, safety and cost-effectiveness will result into an increase in the company’s profit margin.

Technology-enabled maintenance via sensors and the internet of things (IoT) technologies is seen as the “new maintenance strategy”. The aim is to use these technologies to focus on unnecessary breakdowns and increase levels of asset availability where the existing of a dedicated team to maintain or data on asset condition is limited, useless or full of noise. This paper presents the integration of various sensing nodes within an automated laser cutting machine for monitoring of critical components’ conditions. The integrated system provides a centralised platform for data collection from various sensors and provides early failure detection mechanism based on specified set points with alert mechanisms utilising a custom dashboard for data visualisation. The research highlights the required stages for the identification of the appropriate components and sensory devices for the development of efficient monitoring environment.

In the development of the safety-critical systems, it is important to perform Failure Modes and Effects Analysis (FMEA) process to identify potential failures. However, traditional FMEA activities tend to be considered difficult and time-consuming tasks. To compensate for the difficulty of the FMEA task, various types of tools are used to increase the quality and the effectiveness of the FMEA reports. This paper explains an Automatic FMEA tool which integrates the Model-based Design (MBD), FMEA, and Simulated Fault Injection techniques in a single environment. The Automatic FMEA tool has the following advantages compared to the existing FMEA analysis tool. First, the Automatic FMEA tool automatically generates FMEA reports compared to the traditional spreadsheet-based FMEA tools. Second, the Automatic FMEA tool analyzes the causality between the failure modes and the failure effects by performing model-based fault injection simulation. In order to demonstrate the applicability of the Automatic FMEA, we used the electronic fuel injection system (EFI) Simulink model. The results of the Automatic FMEA were compared to that of the legacy FMEA.

Reliability assessment in traditional power distribution systems has played a key role in power system planning, design, and operation. Recently, new information and communication technologies have been introduced in power systems automation and asset management, making the distribution network even more complex. In order to achieve efficient energy management, the distribution grid has to adopt a new configuration and operational conditions that are changing the paradigm of the actual electrical system. Therefore, the emergence of the cyber-physical systems concept to face future energetic needs requires alternative approaches for evaluating the reliability of modern distribution systems, especially in the smart grids environment. In this paper, a reliability approach that makes use of failure modes of power and cyber network main components is proposed to evaluate risk analysis in smart electrical distribution systems. We introduce the application of Failure Modes and Effects Analysis (FMEA) method in future smart grid systems in order to establish the impact of different failure modes on their performance. A smart grid test system is defined and failure modes and their effects for both power and the cyber components are presented. Preventive maintenance tasks are proposed and systematized to minimize the impact of high-risk failures and increase reliability.

Product Design is getting nowadays new challenges on developing new products, since the industry is in a rush to introduce products into the marketplace, where customers demand products faster, cheaper, and free of failures. In the meantime, global companies are always trying to improve their Product Design process to get advantages over their competitors using proven tools like FEMA and mixing methodologies like Fuzzy theories with FMEA. Even today using all this tools and combination of methodologies there is a gap to address and it is required a robust risk analysis solving current issues in the electronic industry. This document aims to reveal a novel integrated method, where Failure Mode and Effect Analysis, Pythagorean Fuzzy Sets and Dimensional Analysis are cohesive into a model that minimize the uncertainty of the ranking and prioritization over the Failure Mode and Effect Analysis execution, helping to identify risks within an accurate grade over possible failures in the Product Design process. A real practical example is used to show the proposed method, where it is identified a robust methodology integration and solid and results using a sensitivity analysis.

Manufacturing corporations has the acceptance of the Outsourcing Process (OP) to improve industrial activities as well as to archive the revenue objectives, and with this, Risk Analysis (RA) tools are constantly used to assure expected results. Failure Mode and Effect Analysis (FMEA) is one of preferred RA tools, moreover, it is proven that FMEA adds uncertainty because of the human participation at the RA, afterward it is demonstrated that Pythagorean Fuzzy Dimensional Analysis – FMEA – Value Stream Mapping (PFDA-FMEA-VSM) method removes the uncertainty in RA, likewise it aids to the stakeholders for decision making, giving more advantages improving the use of the resources on the project. This document exhibits a real case scenario in a manufacturing firm applying PFDA-FMEA-VSM method adapted for manufacturing OP. The application of PFDA-FMEA-VSM shows solid RA results, removing the human intervention uncertainty added to the risk ranking, gives advantages to the stakeholders for visualize the main risks in detailed diagram, as well as make easier to take better decisions on where to apply resources and mitigate risks during OP.
