In recent years, neural networks based on attention mechanisms have been increasingly widely used in speech recognition, separation, enhancement, and other fields. In particular, the convolution-augmented transformer has achieved good performance as it can combine the advantages of convolution and self-attention. Recently, the gated attention unit (GAU) has been proposed. Compared with the traditional multi-head self-attention, approaches with GAU are effective and computationally efficient. In this article, we propose a network for speech enhancement called CGA-MGAN, a kind of Metric GAN based on convolution-augmented gated attention. CGA-MGAN captures local and global correlations in speech signals at the same time through the fusion of convolution and gated attention units. Experiments on Voice Bank + DEMAND show that the CGA-MGAN we propose achieves an excellent performance (3.47 PESQ, 0.96 STOI, and 11.09dB SSNR) at a relatively small model size (1.14M).

Multi-morbidity and polypharmacy are common in older people and pose a challenge for health and social care systems especially in context of global population ageing. They are complex and interrelated concepts in the care of older people that require early detection and patient centred decision making that are underpinned by the principles of multidisciplinary led comprehensive geriatric assessment (CGA). Personalised care plans need to remain responsive and adaptable to the needs of a patient, enabling an individual to maintain their independence.

Objectives As the epidemiological transition continues in sub-Saharan Africa, we will see in future more conditions that require invasive treatment (surgery, cancer and anaesthesia, ie..). Olders patients are vulnerable to complications such as functional decline, increased length of stay and mortality as a result of this type of treatment. Comprehensive geriatric assessment (CGA) is a multidisciplinary diagnostic process aimed at identifying older people at risk of negative outcome. It is delivered by a multidisciplinary team and its benefits in reducing functional disability, incidence of mortality and other adverse health outcomes are well established. It is important to know whether this approach integrates care management strategies for older people in a context where health services for older people are scarce and staff have little training in geriatrics. The objective of this work was to review the literature on the use of CGA on invasive care (cancer, surgery, etc.) among older people in SSA. Methods A MEDLINE/PubMed database search was conducted to identify articles reporting CGA and conditions requiring invasive treatment in older patients in SSA. Results/Conclusion Taken together, the studies examined in this review show that little work has been done on the impact of CGA in older people invasive care in SSA. This review can help to assess the role of CGA in the care trajectories of older people in terms of prognosis, and thus encourage more in-depth research on this topic.

In a previous series of papers relating to the Combined Gravitational Action (CGA), we have exclusively studied orbital motion without spin. In the present paper, we apply CGA to any self-rotating material body, i.e., an axially spinning massive object, which itself may be locally seen as a gravito-rotational source because it is capable of generating the gravito-rotational acceleration, which seems to be unknown to previously existing theories of gravity. The consequences of such an acceleration are very interesting, particularly for Compact Stellar Objects. Independently of the equation of state, it is found that the minimum radius of a stable neutron star is three times its gravitational radius, R_min = 3GM_NS/c², and its critical and maximum internal magnetic field strength cannot exceed the value of 3×10¹⁸ G.
